A Brief Overview
Responsible for obtaining prior authorization, facilitating patient access to medication, coordinating patient care to include documentation, dispensing and distribution under the supervision of licensed pharmacist meeting regulatory and accrediting agency requirements. The role will be responsible for medication authorizations to optimize patient access to their medication therapy. The Pharmacy Care Coordinator will also answer calls, handle verification and identification of covered and non-covered medications, explanations of benefits structures, as well as necessary follow-up with families, providers, and payors regarding referral issues, authorization questions, insurance eligibility issues, copay assistance programs, and medication delivery as applicable. The Pharmacy Care Coordinator will cross cover other areas as needed.
What you will do
* Provides information of Pharmacy benefit structures to patients/caregivers, clinical team and physicians as well as perform the following responsibilities:
* Eligibility verification
* Authorizations
* Expected out-of-pocket costs and copay assistance programs
* Introduces and explains scope of Pharmacy services to the patient/caregiver to include: medication history, refill coordination, access to emergent services, and pharmacist escalation as appropriate.
* Provides customer service when answering incoming patient calls, communicating with patients on urgent requests, and communicating with medical/nursing staff to obtain key information needed
* Assists with coordinating and scheduling the prescription delivery process.
* Prepares new and refill prescriptions accurately, through the proper use of equipment and techniques, based on CHOP Policy for prescription entry and dispensing.
* Participates in departmental process improvement initiatives.
* Follows up with insurance companies and families of patients identified as ineligible or non-covered
* Contacts family and/or responsible party, as necessary, to inform them of any insurance problems or restrictions, ensuring that insurance information is clearly relayed to and understood by family and/or responsible party. Contact clinical office with all information that requires follow-up.
* Performs other duties as assigned by Pharmacy Management.
* Create and/or update patient record by verifying demographic, insurance information, or proper study identification in the registration system
* Ensure specific registration fields are accurately inputted into registration system, including but not limited to visit notes, referral/authorization numbers, eligibility review, proper payor plan entries for research and standard-of-care visits
* Verify coverage through third party benefit coverage/authorization vendors.
* Educate and inform family of outstanding financial obligations if appropriate.
* Electronically collect or facilitate payment of outstanding balances.
Education Qualifications
* High School Diploma / GED - Required

Responsibilities:

Under the supervision of the Hospital Manager of Medical Staff Services/DUHS CVO, the Medical Staff Services Coordinator is responsible for coordination of aspects of DUHS Medical Staff Services office by providing essential verification and quality control functions to support DUHS's goals for credentialing and privileging health care providers while ensuring adherence to legal, regulatory, and delegated requirements. Assignments are planned and executed with considerable independence in compliance with established policies and procedures, bylaws, regulations and laws.
Duties and Responsibilities of this Level
* Organizing and directing of a comprehensive and specialized credentialing program to assure continued accreditation compliance by the Joint Commission, NCQA, and AAAHC, as well as any other regulatory organizations.
* Interpretation, explanation and following all regulatory guidelines, including medical staff bylaws, fair hearing plan, rules and regulations and policies.
* Maintaining accuracy in the Credentialing database to identify (and disseminate to others), medical staff members and their clinical privileges; prepares and maintains other related records and reports. Responsible for the accuracy and integrity of the credentialing and privileging database system and for data entry performed in the medical staff office.
* Receives application/reappointment application from the CVO, oversees the completion of the credentialing process for medical, professional, and advanced practice professional staff and granting of clinical privileges/scopes practice for the DUHS Medical Staff.
* Coordinating, monitoring and maintaining the credentialing and re- credentialing process. For reappointment applications, this includes gathering of additional data for incorporation into hospital-specific processes (i.e., QI elements, FPPE/OPPE process, etc.) and evaluation/analysis of confidential healthcare provider credentials for the initial appointment and reappointment processes prior to presentation for the Department Chair, Division Chief, and Credentials Committee review and action.
* Works with medical staff leadership and hospital clinical staff for the approval and ongoing review of credentialed practitioners.
* Monitoring and revising credentialing and privileging/scope of practice forms as needed. Ensures maintenance of medical staff, advanced practice professional staff, and professional staff files.
* Scheduling and supporting (prepares agenda, attends and prepares minutes) the Credentials Committee. Provides support to the Credentials Committee chair as needed. May participate on other Hospital committees as requested. Uses discretion to escalate identified red flags to the Manager, Medical Staff Services/CVO Office.
* Communicating medical staff information to appropriate departments; including approval notices to the provider, and other entities (if applicable). In addition, prepares reports to the respective boards to comply with their deadlines.
* Demonstrates effective leadership skills and coordination of intra- department activities and inter-department integration. Assures necessary revisions are communicated with the Manager, Medical Staff Services/CVO Office for the ongoing development and implementation of policies and procedures that guide and support the provision of services.

A caregiver in this position works 8:00am - 5:00pm. This position requires travel to skilled nursing facilities in the area and Cleveland Clinic locations for occational meetings.
A caregiver who excels in this role will:

Minimum qualifications for the ideal future caregiver include:
Associate's Degree in communication, psychology, sociology or a related field and 3 years of related experience in customer relations with diverse patient/culture populations
OR A high school diploma/GED and 5 years of progressively responsible experience may substitute the education requirement

A Brief Overview
Provide basic purpose or primary function of the job: The Patient Services Representative is responsible for greeting patents and families, collecting & entering demographic and insurance information, collecting payments associated with services rendered and reconciliation of daily services. In addition, other duties necessary to facilitate efficient and timely services to patients and their families, such as scheduling appointments, answering phones, and obtaining appropriate insurance referrals and authorizations as needed per insurance guidelines. Responsibilities may include working with medical records, following HIPAA regulations. Other duties and administrative support and projects as required.

What you will do
* Provide welcoming, prompt and respectful customer service which includes scheduling appointments, inform of delays, address family complaints and escalate as necessary.
* Perform EPIC functions such as check in, check out, enter demographic and insurance information while ensuring accuracy, run daily eligibility verifications.
* Reconciliation of daily services to include collection and processing of payments received, reimbursements, create and/or obtain insurance referrals and authorizations needed.
* Answer phones promptly with minimal downtime between calls, accurately verifies and documents contact information and messages, provides appropriate information to callers as warranted. Communicates in a courteous, caring manner to all customers as needed. Check voicemails.
* Call Management Screens callers' reasons for call by using the high priority list and critical thinking skills to prioritize calls to be triaged: backline to nurse, high priority in queue, or regular queue. Triages and documents CHOP home care calls. Contacts on call providers directly for calls from those requesting to speak to them, such as E.D.'s, home care nurses, other physicians, and parents. Assists in the distribution of calls to on call providers when the volume/staffing warrants it. Seeks resources to assist in decision making to ensure safe, high quality telephone triage and customer service as needed.
* Communicates and provides essential information updates during shifts such as change in on call providers and signing out by offices. Communicates concerns and ideas for improvement to the Nursing Supervisor/Manager as needed.
* Perform administrative tasks such as sort and deliver mail, faxes, medical records and other correspondence, photocopies, and faxes documents.
* Participate in team huddles and meetings.
* Daily maintenance of patient records and provider template and daily schedules while following HIPPA guidelines.
* Periodic rounding to inform patients of delays.
* Work necessary billing workques, EPIC pools and various other reports.
* If located in an area where 24/7 coverage is needed, will need to rotate shifts.
* ED/Admissions specific tasks as assigned include: SART paperwork processing, Nurse Call communications, Trauma B-line and arrivals.
Education Qualifications
* High School Diploma / GED - Required
Experience Qualifications
* At least one (1) year patient services representative experience in a role that consisted of health care, registration, physician billing, medical records, medical office work and relatable customer service - Preferred
