CLUES' Adult Mental Health Targeted Case Manager collaborates with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. The position is responsible for promoting behavioral health services both internally within CLUES and externally within the community, while also assisting staff in areas including but not limited to quality, outcome-based design, service relevance, delivery, effectiveness, and evaluation, all in alignment with CLUES mission. Respond promptly to client crisis situations and seek support from supervisor in high-risk situations. Collaborate with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. Participate in individual and group supervision as required within the team and the department. Attend other meetings as required by contract or agency.
