Greenbriar Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Assistant to join our team. Thi position will work regular hours Normal hours 7:00 am - 3:30 pm, must be flexible for early or late appointments on occasion.
PURPOSE/BELIEF STATEMENT:
The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.
JOB DUTIES & RESPONSIBILITIES
Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.
Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.
Assist with transporting residents to and from scheduled in-house activities.
Provide one on one monitoring for residents deemed an elopement risk on outside visits.
Perform basic nursing care procedures for residents as required.
Create a clean, comfortable, and safe environment for residents and assure that their needs are met.
Maintain resident documentation records.
Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.
Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.
Perform other related activities as assigned or requested.
QUALIFICATIONS & EXPERIENCE REQUIREMENTS
High school graduate or G. E. D. equivalent
Must possess a current, valid Driver's License in State of employment.
Graduate of an approved Nurse Aide Training Program or be eligible to enter such a program and graduate and pass a state test within 120 days from date of employment.
Must possess Red Cross First Aid Certificate.
Prior work/life experiences involving independent decision making
Prior work/life experiences, preferably in healthcare or a service industry

You will help provide assistance to licensed nursing personnel in the provision of patient care by providing and documenting activities of daily living; and providing a clean, healthy, and safe environment.
Qualifications Knowledge/Skills & Abilities
High school graduate or G. E. D. equivalent.
Current State Tested Nursing Assistant Certification
1 Year LTAC experience preferred, but not required.
JOB DUTIES & RESPONSIBILITIES
As STNA, you will:
Participate in and receive resident reports at start/end of shift.
Provide personal/nursing care in accordance with resident care plan.
Facilitate and conduct activities.
Monitor & respond to resident requests and needs in a dignified and respectful manner.
Maintain a clean and pleasant environment for residents.
Assist in preparing residents for various activities within and outside the center.
Ensure that all resident care is provided in a dignified and respectful manner.
Perform basic nursing care procedures for residents as required.

Ensure that resident's food service needs are met in a timely and compassionate manner.

Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.

MINIMUM QUALIFICATIONS:
Current State Tested Nursing Assistant certification preferred. One to two years previous healthcare experience is required. Current BLS and immunizations (Hep B and TB screening) are required.

JOB DUTIES & RESPONSIBILITIES
As STNA, you will:
Assists the registered nurse by gathering data on patient conditions, direct patient observation, obtaining vital signs, recording oral intakes, monitoring input and output, weighing patients, turning patients, recording/charting patient information and interacting with patient and family members.
Reports changes in patient medical and/or emotional condition to the registered nurse.
Assists nursing personnel in responding to various emergency patient situations.
Assists patient in activities of daily living by feeding, bathing, toileting and other personal hygiene regimes.
Demonstrates sensitivity to patient comfort and privacy and expresses interest in patient progress.
Answers call lights. Maintains confidentiality and Documents interventions and patient responses according to policy.
Cares for and maintains patient's environment by avoiding environmental hazards and consistently using infection control procedures and barriers and other standards of safety.

Graduate of an approved Nurse Aide Training Program
6 month to year of Lockdown and memory care unit experience.
JOB DUTIES & RESPONSIBILITIES
As STNA, you will:
Participate in and receive resident reports at start/end of shift.
Provide personal/nursing care in accordance with resident care plan.
Facilitate and conduct activities.
Monitor & respond to resident requests and needs in a dignified and respectful manner.
Maintain a clean and pleasant environment for residents.
Assist in preparing residents for various activities within and outside the center.
Ensure that all resident care is provided in a dignified and respectful manner.
Perform basic nursing care procedures for residents as required.
Create a clean, comfortable, and safe environment for residents and assure that their needs are met.
Ensure that resident's food service needs are met in a timely and compassionate manner.
Maintain resident documentation records for activities of daily living, restorative programs, and resident specific forms.
Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards
Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.
