Duties:

Monitors progress of the plan of care and communicates any changes with the care team.
To the extent possible, provides integrated - mind, body, spirit - service to the patient and family through a mutually agreed upon plan of care and patient advocate relationship.
Supports System processes related to the health management of patients across the continuum of care, e.g. QI and/or clinical practice team membership, clinical research, quality monitoring, community networking.
Assesses needs of patients/families and conceptualizes problem situations within social work scope of service using appropriate age-related skills.
Provides direct social work services and counseling to patients and their families. Interprets social, psychological, emotional, financial and family issues for attending physicians and other health care team members.
Completes documentation required in patient's medical record on department forms and reports, and in MIDAS+ and in Cerner.
Demonstrates continued enhancement of professional skills.
Complies with department and hospital policies and procedures.
Supports organization goals to provide the best/optimal care to our patients and families, and community to our highest customer satisfaction.

NLSLA is seeking a dedicated SocialWorker to support survivors of domestic violence through our SUP Domestic Violence Supportive Services (DVSS) grant. This role provides short-term supportive services, case management, crisis intervention, counseling, and resource navigation for survivors and their families.
The SocialWorker will engage in trauma-informed, client-centered practice while collaborating closely with attorneys, case managers, and community partners to reduce barriers and support client safety and stabilization.
In this role, you will report to the Social Services Supervisor. This position is exempt.
What You'll Do
· Conduct intakes and comprehensive assessments to identify client needs, strengths, and barriers related to domestic violence.
· Create individualized service plans in collaboration with clients, including establishing goals, timelines, follow-up, and reassessment.
· Provide short-term counseling, psychoeducation, crisis intervention, and safety planning for DV survivors.
· Facilitate or co-facilitate support groups, life-skills workshops, and domestic violence education groups.
· Provide client advocacy services, which may include:
Court accompaniment
DCFS advocacy
Assistance obtaining police reports and medical records
Coordination with community mental health, financial assistance, and housing resources
· Provide case management, resource navigation, and direct referrals to community-based services.

Under the supervision of the Adult Day Health Care Program Director, the Bilingual SocialWorker leads and delivers the social services component of the L'Chaim ADHC program. Responsibilities
Work with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as, history of trauma, co-occurring conditions and suicide.
Assist clients in identifying and managing personal, psychological and interactive problems, and in achieving effective personal, occupational, marital, and family development and adjustments.
Provide comprehensive social work services in the form of casework and case management by consulting on or developing discharge plans for complicated cases, psychosocial assessments and mental status examinations on behalf of beneficiaries.

· Knowledge of trauma-informed care, domestic violence dynamics, and strengths-based frameworks.
· Experience facilitating support groups or psychoeducational workshops.
· Experience working collaboratively with attorneys, court staff, DCFS, shelters, and crisis teams.
· Familiarity with Los Angeles County resources, CalVCB, CalWORKs, shelters, and DV service networks.
