    Job Description QUALIFICATIONS: Master's level in Counseling or Social Work (Other license-eligible Masters Degrees will be considered). At least one year of experience in individual, family, and group therapy methods preferred. Act 33/34 & PA FBI clearances required. Child Protective Service Law Training (CPSL) completion required. Evidence and demonstration of cultural humility required. Maintain a current driver's license and insurance coverage for vehicle. Must have own transportation. Must possess excellent communication and interpersonal skills. RESPONSIBILITIES: School Based Provides individual counseling, group counseling, and care management for children and adolescents with mental disorders. Assesses strengths and therapeutic needs of children and families. Maintains timely and adequate documentation (including reports, summaries, and letters) on each client assigned. Requires travel to school-based locations in Mercer County Provides community education as time permits. This involves public speaking, and planning and participating in education workshops, seminars, etc. Participates in the utilization/peer review process, audits, supervision, and treatment team meetings. Includes child as a participant in his/her own treatment. Reports directly to assigned Clinical Supervisor. Outpatient Provides individual counseling, group counseling, and care management for children and adolescents with mental disorders. Assesses strengths and therapeutic needs of children and families. Maintains timely and adequate documentation (including reports, summaries, and letters) on each client assigned. This position may require work in satellite office (Grove City and Greenville). Recommends and refers for special evaluations and services, such as psychiatric assessment or psychological testing. Involves and encourages participation of collateral supports (i.e., family, friends, other providers) in treatment and treatment planning. Reports directly to assigned Clinical Supervisor. May be required to work evening hours. May also be required to work non-traditional hours if needed. Supervisor will determine this according to need. Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work. Adheres to agency policies and procedures/job duties or will be subject to disciplinary action. Performs other duties as assigned. Can perform the essential functions of this position with reasonable accommodations, if necessary. Participates in CCC CQI plan to ensure quality of service to CCC consumers.

This member of the ACT multidisciplinary team has the responsibility to carry out treatment, rehabilitation and support functions, education, and provide crisis intervention to participants and support and consultation to families. As essential personnel may be required to report to work during emergencies including inclement weather. SPECIFIC DUTIES: Provide service coordination (case management) for assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT; provide individual supportive therapy and symptom management; ensure immediate changes; are made in treatment plans as clients' need change, educate and support clients' families; and advocate for clients' rights and preferences. Conduct comprehensive assessment of psychiatric history (e.g. onset, course an effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment, social development and functioning; activities of daily living (e.g., care-care, living situation, nutrition, money management); and family structure and relationships. Provide ongoing assessment of client's mental illness symptoms and clients' responses to treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put clients at risk (e.g., suicidality). Provide symptom education to enable clients to identify their mental illness symptoms. Provide direct clinical services including individual supportive therapy and psychotherapy to clients on an individual, group and family basis in the office and in community settings to teach behavioral symptom- management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stresses. Provide treatment, rehabilitation, and support services, with some interventions directed or performed by staff with specialty training and skills (e.g., vocational specialists). Perform shift management in coordination with other shift managers according to established policies and procedures. Provide on-call crisis intervention covering nighttime hours and serve as a back-up to evening and weekend staff. Document client progress to maintain a permanent record of client activity according to established methods and procedures. Candidates receiving job offers must pass a medical examination (when applicable) before the date of hire. * Written: None required for this position. * Performance: None required for this position. * Medical: None required for this position. * SCHEDULE: The standard schedule is Monday-Friday, 8:00 a.m.-4:30 p.m. However, shift times may be adjusted in response to call volume or emerging community needs. * Practices OCH&S values and principles, emphasizing harm reduction, person-centered, and trauma-informed approaches. * Adheres to principles of social work and professional counselor code of ethics and consults Program Manager when navigating and resolving ethical dilemmas. * Provides mobile crisis response services by responding to calls for service across City of Pittsburgh neighborhoods, responding to calls dispatched through County 911, and intervening as requested after public safety personnel have cleared the scene for safety, done via vehicle or other mobile unit. * Conducts on-scene de-escalation, stabilization, risk assessment, and safety planning in the least restrictive environment possible. * Delivers stabilization and support during mental and behavioral health crises in the least restrictive environment possible. * Assesses biopsychosocial factors related to the immediate crisis, develops stabilization strategies, and coordinates follow-up care and referrals. * Regularly communicates with the Program Manager and other OCHS leadership as needed regarding ongoing work and to facilitate problem-solving. * Engages in multidisciplinary case conferencing and collaborates with internal and external partners to ensure effective support. * Facilitates referrals to internal OCH&S programs and community-based services * Documents program participants' interactions and calls thoroughly and promptly. * Communicates with individuals, families, and providers via in-person, phone, or electronic methods to assess concerns and determine needs. * Collaborates with public safety and community partners to enhance coordinated crisis response. * Participates in team and staff meetings as required. * Engages in daily participant and program outreach via vehicle, and on foot through frequent walking, and other means necessary to render services. * Performs other related duties as assigned.
