Essential Duties and Major responsibilities: * Schedules Nurse visits with patients as appropriate * Orders appropriate laboratory & Diagnostic testing according to established guidelines * Prepares patients for visits with providers by performing and recording vital signs (including: blood pressure, temperature, pulse, respirations, weight, and other procedures as needed)
* Reviews patients' medical records for completeness and updates prior to patients' visit
* Assists in routine physical examinations by preparing the patient, the equipment, and supplies, and assisting the provider as needed
* Assists in preparing patients for routine diagnostic procedures (i.e. urine specimens) and performing selected screening tests (including: urine dipsticks, urine hCG, and rapid strep throat cultures)
* Assists in preparing routine diagnostic request forms
* Books patients for tests and appointments with outside facilities and/or specialists
* Executes medical orders for specific drugs, treatments, and other diagnostic or therapeutic procedures
* Administers and records medication administration consistent with his/her knowledge of pharmacology in accordance with health center policy
* Administers routine vaccines. Is aware of current vaccine schedules, catch up schedules & complies with vaccine storage guidelines as set by the MA DPH
* Participates in preventive health teaching and education pertinent to procedures being conducted
* Provides case management type oversight to an identified patient population as assigned by the Nursing Coordinator
* Provides guidance to non-clinical staff regarding clinical issues (i.e. phone triage)
* Near visual acuity to observe changes in health/illness status of patients. Manual skills required include good eye and hand coordination in combination with finger dexterity for administration of injections, treatments, etc. Good color vision is required for perceiving changes in skin color and colors of medicines, solutions, etc
* Assists & yearly HEDIS reporting as assigned
* Work is of medium demand, walking and standing.
* Occasional lifting of patients with assistance, reaching, handling and fingering of instruments and equipment and caring of patient needs.
* Hearing to distinguish difference in heartbeat and breathing of patients and to hear instructions.
* Near visual acuity to read gauges and dials on equipment.
* Color vision for perceiving changes in patient's skin color or color of solutions.
* Able to lift 40 lbs.
* Managing patient care is a team effort that involves clinical and non-clinical staff interacting with patients

Minimum Qualifications:
* Current Registered Nurse or LicensedPracticalNurseLicense the Commonwealth of Massachusetts and its Board of Registration in Nursing required,
* Bachelor of Science Nursing a plus.
* 2 years similar work, internship, or volunteer experience required
* Work experience equivalent to one year as an RN or LPN in an ambulatory medical office environment preferred
* Bilingual in Spanish and/or Portuguese a plus
* Computer skills for accurate data entry
* AHA BLS Health Care Provider certification or equivalent
* Demonstrated interpersonal relationship skills
* Demonstrated written and verbal communication skills in English
* Demonstrated ability to work in a fast-paced medical office environment

Summary: The LicensedPracticalNurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. In addition the incumbent with provide advocacy and liaison with community health care providers, monitoring medical records, and medication administration.
Schedule: Monday - Friday 3:00 PM - 11:00 PM (40 hours)
Essential Duties and Responsibilities Include:
Provide direct specialized nursing and other day-to-day care to individuals.
Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements. Abide by all agency policies and procedures.
Function under the supervision of the residential manager, in collaboration with the registered nurse and Director of Health Care.
Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care. Help staff identify appropriate community health resources.
Provide in-service education to individuals and/or staff on health issues.
Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures.
Administer medications to assigned individuals following MAP policies and procedures
Provide quality assurance reviews of medication systems and MAP compliance.
Work collaboratively with the residential manager and registered nurse evidenced by meeting weekly or as needed for medical updates.
Work collaboratively with the nursing/healthcare staff of Bridgewell by participating in monthly nursing staff meetings and agency-wide healthcare planning.
Collaborate with the residential manager and registered nurse to assess, organize, update, and maintain health care and medication needs. This should include, but is not limited to:
Individual client assessment
Review of routine and other physical exams
Review of physician-ordered treatments
Assurance of treatment follow-up and coordination of medical and psychiatric care
Maintenance of medical charts.
Other duties as assigned
In Addition, your Direct Support Responsibilities Include:
Provide support and assist individuals with all activities of daily living which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
Maintain the comfort, privacy, and dignity of each individual in the delivery of services to them.
Observe residents for changes in condition or behavior and promptly report these changes to the Residential Manager.
Provide services to individuals according to DDS, CARF, and all applicable regulatory standards.
Provide support and assist individuals with all medical, clinical, and financial needs by identifying needs and securing appropriate services as needed.
Transport individuals as needed to and from medical, recreational, and other activities outside the home.
Assist with housekeeping and cleaning duties as needed to maintain a safe and clean living environment.
Maintain progress notes according to DDS and all applicable regulatory standards.
Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in the ISP meeting and other relevant meetings for the individuals.
Assist the individuals with accessing resources and recreational opportunities in the community.
Comply with Bridgewell Awake Overnight Call-in Procedures if applicable to assigned shift.
Maintain core training certifications including CPR/FA, OSHA, and PABC, as well as completing training in MAP.
Required Education & Experience:
Graduation from an accredited and approved licensedpracticalnurse program.
Must have current Massachusetts Nursinglicense.
Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
Experience working with adults with DD and behaviors is a plus.
Must have a Driver's license for at least one year with a good driving record history.
Must be able to drive a 12-passenger van. We will provide training.

Essential Duties and Responsibilities Include:
Provide direct specialized nursing and other day-to-day care to individuals.
Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements. Abide by all agency policies and procedures.
Function under the supervision of the residential manager, in collaboration with the registered nurse and Director of Health Care.
Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care. Help staff identify appropriate community health resources.
Provide in-service education to individuals and/or staff on health issues.
Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures.
Administer medications to assigned individuals following MAP policies and procedures
Provide quality assurance reviews of medication systems and MAP compliance.
Work collaboratively with the residential manager and registered nurse evidenced by meeting weekly or as needed for medical updates.
Work collaboratively with the nursing/healthcare staff of Bridgewell by participating in monthly nursing staff meetings and agency-wide healthcare planning.
Collaborate with the residential manager and registered nurse to assess, organize, update, and maintain health care and medication needs. This should include, but is not limited to:
Individual client assessment
Review of routine and other physical exams
Review of physician-ordered treatments
Assurance of treatment follow-up and coordination of medical and psychiatric care
Maintenance of medical charts.
Other duties as assigned
In Addition, your Direct Support Responsibilities Include:
Provide support and assist individuals with all activities of daily living which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
Maintain the comfort, privacy, and dignity of each individual in the delivery of services to them.
Observe residents for changes in condition or behavior and promptly report these changes to the Residential Manager.
Provide services to individuals according to DDS, CARF, and all applicable regulatory standards.
Provide support and assist individuals with all medical, clinical, and financial needs by identifying needs and securing appropriate services as needed.
Transport individuals as needed to and from medical, recreational, and other activities outside the home.
Assist with housekeeping and cleaning duties as needed to maintain a safe and clean living environment.
Maintain progress notes according to DDS and all applicable regulatory standards.
Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in the ISP meeting and other relevant meetings for the individuals.
Assist the individuals with accessing resources and recreational opportunities in the community.
Comply with Bridgewell Awake Overnight Call-in Procedures if applicable to assigned shift.
Maintain core training certifications including CPR/FA, OSHA, and PABC, as well as completing training in MAP.
Required Education & Experience:
Graduation from an accredited and approved licensedpracticalnurse program.
Must have current Massachusetts Nursinglicense.
Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
Experience working with adults with DD and behaviors is a plus.
Must have a Driver's license for at least one year with a good driving record history.
Must be able to drive a 12-passenger van. We will provide training.

The Care Coordinator performs an integral role in the team-based approach to care, developing collaborative relationships with providers, patients and their significant others and other members of the care team. He /she ensures that high risk patients receive care that is consistent with national, state and health center standards and policies. The Care Coordinator supports patients and families in self-management, through evidenced-based approaches as appropriate, engages in health center projects aimed at specific patient population needs, and utilizes effective and appropriate communication strategies for issues such as health literacy and cultural norms when working with patients.
