Under supervision, clinician provides a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, group facilitation, and/or case management services in an integrated, multi-disciplinary primary care setting, located in a community health center. Works collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.
Responsibilities:
Provides individual, group, and family therapy to clients as appropriate to the position; prepares treatment plans, and case management including discharge planning; provides therapeutic crisis stabilization services as required.
Collects data relating to patients through interview, case history, psychological assessments, and/or observational techniques; evaluates data to identify root causes of problems and determines proper the appropriate therapeutic approach or referral to other providers, as appropriate.
Consults with supervisor, or other behavioral health and medical staff, as appropriate, in the performance of therapeutic and/or case management services; refers clients to appropriate service agencies based on medical necessity.
Provides and/or arranges for therapeutic interventions as appropriate for patients or clients requiring crisis stabilization or level of care change, specifically for those with acute Mental Health and/or active substance use disorder.
Refers clients to appropriate community/social service agencies for financial assistance and/or other necessary services.
Provide outcome measurements as required to evaluate treatment progress and adjust treatment plan as needed.
Establishes and maintains all aspects of client case file including clinical documentation beginning at intake and throughout client discharge, including closing treatment plans for clients completing day treatment and/or transitioning to outpatient behavioral health services.
Maintains relationships with referral partners; collaborate with outside treatment providers agencies in relation to patient/client records, rights, and responsibilities while engaged in day treatment.
Works alongside community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools.
If appropriate, provide clinical supervision to Master's level therapists and/or graduate social work students/interns, and/or education or consulting services to other patient care professionals.
As appropriate to the position, perform various administrative functions such as group preparation, assist with scheduling, chart audits for quality improvement, utilization review, and outreach to referral sources, etc.
Participate in weekly clinical supervision and twice monthly group supervision, attend staff meetings, morning rounds, and multi-disciplinary team meetings as appropriate to the role.
Provide psychoeducation, consultation, and in-service training to staff, if/when required.
Demonstrate a comprehensive understanding and commitment to the health center Mission, Values & Standards.
Provide a description of Day Treatment to new clients.
Perform miscellaneous job-related duties as required.
Minimum Qualifications:
Master's degree in Counseling or Social Work
Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC) or equivalent or licensure pending, as documented by temporary license.
Ability to make administrative/procedural decisions and judgments.
Ability to observe, assess, and record symptoms, reactions, and progress.
Knowledge and understanding of clinical social work and mental health counseling including competencies, principles, methods, procedures, ethics, and standards.
Interviewing and psychological/developmental evaluation skills.
Knowledge of clinical operations and compliance guidelines.
Familiarity as well as collaboration with community mental health resources.
Ability to maintain emotional stability during crisis or emergent situations.
Strong interpersonal skills including the ability to engage and redirect group members as needed and the ability to work effectively with a wide range of constituencies in a diverse community
Strong verbal and written communication skills including creating, composing, and editing written materials.
Knowledge of crisis intervention techniques and ability to de-escalate in stressful situations.
Ability to evaluate the progress of therapeutic interventions and adjust treatment plans as needed.
Knowledge of legal and ethical issues related to patients' rights in MA.
Ability to lead and train staff and/or students, as needed
Physical Requirements:
Work is normally performed in a typical interior/office work environment.
Minimal/limited physical effort needed

Job Description
Minimum Qualifications:
1. Master's degree in Social Work from an accredited institution.
2. Licensed Independent Clinical Social Worker (LICSW).
3. Five (5) years of related experience.
Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Provides psychosocial assessment of the individual and family, including limitations, deficits and strengths in their current situation and environment, as well as functional level. May conduct a psychiatric social work assessment according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Develops treatment plans with appropriate and competent interventions.
2. Responsible for psychosocial assessment and intervention in crisis situations that often involve issues of suicide, homicide, family violence, elder or child abuse.
3. Demonstrates sound clinical judgment in assessing the patient's needs for long-term care, including evaluating the home and family situation, helping the patient and family to develop an in-home care plan, exploring alternatives to in-home care and arranging for placement if necessary.
4. Provides treatment, therapy, psycho-education and/or counseling utilizing individual, family or group modalities.
5. Provides information, referral and creative resource acquisition for specific individual and family needs both within the Hospital and in the community as indicated. Assists patients/families to understand psychosocial factors impeding their maximal utilization of hospital and community resources.
6. Collaborates with and provides psychosocial consultation to the health care team to promote a smooth, coordinated plan of care. Aids the team in understanding and integrating the significance of psychosocial factors in relation to patient's illness, treatment and recovery. Identifies psychosocial issues that may impede progress. Makes recommendations to the team regarding patient/family care and management. Assists health care team to assist patient/family level of understanding to make informed decisions.
7. Provide medical social services to the patient's family member or caregiver on a short-term basis when necessary to remove a clear and direct impediment to the effective treatment of the patient's medical condition or rate of recovery.
8. Treats patients and families with dignity and respect at all times. Offers time for questions and answers; demonstrates empathy in communication while working toward optimal health goals.
9. Provides outreach, case finding/screening for high-risk issues that may impact on patient's progress, participation in plan, discharge or ability to utilize resources.
10. Assists with discharge planning issues to ensure continuity of care. Formulates, coordinates and implements the psychosocial components of outpatient care.
11. Consults, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources.
12. Serves as a patient/family advocate with a variety of systems both internally and externally as required. May be called upon to testify in court.
13. Ensures clinical documentation is complete and according to departmental standards. Performs other administrative documentation and record keeping, such as accountability and statistical reports, billing forms, student intern evaluations, performance evaluations, etc. in complete and timely manner.
14. Initiates policy and program development in specific service area or team, utilizing knowledge of state-of-the-art programs and promoting social work values such as patient self-determination and social justice. Initiates, participates in and supports policy and program development in the Department of Social Work Services.
15. Actively engages in the regularly scheduled meetings, programs and activities of the Department. Serves on departmental committees. Represents the profession and Department in hospital and community meetings and committees.
16. Participates in social work professional education, supervision and teaching activities; supervises or trains staff social workers, student interns, house staff, nurses, medical students, and volunteers as required.
17. Maintains collaborative, team relationships with peers and colleagues in order to effectively contribute to the working groups achievement of goals, and to help foster a positive work environment
18. Continually monitors, assesses and modifies his/her own social work practice and programs. Conducts quality improvement activities and research regarding service delivery and effectiveness of interventions, as required.
19. Conducts or participates in professional organizations, grant writing, teaching, public presentations, seminars and workshops. May supervise staff social workers, social work associates, social work interns, parent consultants, house staff, nurses, medical students and volunteers as required.
20. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.
Physical Requirements:
1. Normal office setting.

3. Frequent sitting, occasional standing & walking, and lifting of 5-10 lbs.
4. Requires manual dexterity using fine hand manipulation to operate computer keyboard.

1. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
2. Knowledge of casework process and community resources in relation to health (including mental health), illness, and disability.
3. Knowledge of current social work theory and practice, the ability to apply treatment and case management methods, and familiarity with community issues and resources.
4. Understanding of organizational theory and practice in large systems and the knowledge of social and health care policies and systems.
5. High degree of diagnostic, treatment, interpersonal, organizational, and communication skills.
6. Analytical skills required assessing patient needs, to develop associated treatment modalities and to provide effective counseling.
7. Computer skills and knowledge of general office programs, spreadsheet programs, database programs, and presentation programs (Word, Excel, Access, PowerPoint).
8. Ability to read/write and communicate in English.
9. Excellent interpersonal skills.
10. Ability to function well in very busy situations.
11. Responsible and reliable.
12. Good organization skills.
13. Ability to engage a wide variety of patients and collaterals in the acute treatment and discharge planning process.
14. Ability to identify and access community resources.

A Mental Health Counselor is an integral part of our multidisciplinary treatment team. As a Mental Health Counselor, you will:
Conduct patient observation and rounds.
Assists staff with patient care while following treatment plans.
Observe and monitor patient's progress and reports observations to staff.
Develop a therapeutic relationship with patients while maintaining professional boundaries.
Assist patients with activities of daily living skills, as necessary.
Provide appropriate supervision of patients' activities.
Responsible for safety monitoring of patients of the unit.
Process admissions belongings.
Complete documentation in the EMR system.
Facilitate patient groups.

Mental Health Counselor candidates will have the following:
High School Diploma. Bachelor's degree in psychology or human services field preferred.
Previous PCT/PCA, CNA or Medical Assistant experience required.
Bachelor's degree may be considered in lieu of experience.
Effective oral and written communication skills; ability to develop a therapeutic relationship while maintaining professional boundaries.
Previous experience in psychiatric or substance inpatient, restraint reduction de-escalation, and crisis intervention is preferred.
Pay Range

The Counselor Supervisor will supervise the Counselors and Counselor Interns who provide treatment and casework services to patients in compliance with all applicable laws and regulations and with BayMark's policies and procedures. The position may include participation in, and support of, the Rubenstein Unit-Based Exercise (RUBEX) program and exercise supervision of clients enrolled in Hebrew Rehabilitation Center's "Get Up & Go" Outpatient exercise program.

Job Responsibilities & Essential Functions
Participates in treatment planning under the direction of clinical and nursing staff

Due to the type of work and client needs, this position may need to work beyond a forty-hour work week. Additionally, essential staff may be required to remain on shift or work an additional shift until relief staff is available.
For overnight positions, staff are required to stay awake for the entirety of their shift.

The GLFHC Behavioral Health Team is currently seeking an experienced Mental Health Counselor. Under the supervision of a LICSW, works collaboratively with all members of the interdisciplinary healthcare team, provides consultation to the clinical site team and provides individual and group counseling to GLFHC patients referred by primary care clinicians
Scope of Duties:
Serves as liaison between patients, family member and health care team. Helps to educate patients and their family about patients' behavioral health treatment. Explains treatment in a clear, concise manner, with sensitivity to patients' level of understanding, culture, age and language. Documents all teaching materials given to the patients, or family members including their response, comprehension and need for further follow-up.
Upon referral from the care team, reviews the patient's biopsychosocial history, and completes the assessment and care plan. Maintains accurate and timely records according to policy.
Provides short term individual, group and family intervention. Collaborates with community behavioral health providers in the coordination of behavioral health care.
Develops and facilitates support groups as determined by patient/client need, interest and team member recommendation.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Is able to identify and categorize each patient's age specific grouping of needs. This may include children, couples, families, and adults.
Collaborates with GLFHC clinical staff and administration to develop and maintain referral resources throughout service area. Refers patients to services within GLFHC and to other community agencies.
Develops working relationship with leaders and community agencies, representing GLFHC at meetings as indicated.
Demonstrates clinical competence and professional commitment by adhering to established standards of practice and utilizing appropriate scientific principles.
Develops and maintains systems for patient educational materials and staff.
On a regular basis participates in ongoing quality improvement activities related to individual, team, and organizational performance improvement.

Works collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.
Responsibilities include:
* Conduct intake assessments to evaluate individuals' appropriateness for our program.
* Use clinical tools (urine drug screens, treatment plans, counseling, etc.) to monitor clinical progress and promote and support recovery.
* Lead weekly group therapy sessions that build recovery skills and foster community by utilizing the outlined curriculum.
