Primary Responsibilities (including, but not limited to):
Identify all local influence centers (hospitals, nursing homes, assisted living with rehabilitation or respite care, doctor's offices and other health providers) to determine lead sources.
Build and maintain database of potential referrers.
Conduct training sessions to professionals and families that focus on dementia care and Parkinson's disease care.
Personally visit and arrange meetings with people responsible for or in a position to refer clients; this includes private, public, and non-profit organizations.
Attend trade shows, conferences and networking events representing Visiting Angels services. Network with others in the industry to develop additional lead sources.
Manage periodic on-call calls with internal staff.
Maintain and manage a (CRM) Customer Relationship Management System to track leads and follow-up on leads.
Achieve weekly visit frequency with key targets and deliver messaging as per our marketing program.
Use tracking sheets to record activity and submit to manager weekly.
Meet with manager weekly to discuss opportunities, referrals status, activity, etc.
Base Pay is $55k to $65k per year, plus commission

Position Summary:
Full-time Community Resource Navigator position available for our Managed Care Department in Mount Vernon, WA. The role of the Community Resource Navigator is to provide information and assistance to individuals seeking health coverage, health care and other local resources that aid them in maintaining a healthy lifestyle. Helps in educating eligible individuals on how to navigate health insurance options, public programs, and Sea Mar services and assist with the application processes. Aid newly assigned and high-risk patients in scheduling their initial appointment for medical, dental and care management services. Provides case management and or participate in intervention strategies that remove barriers that may result in appointment no shows. In addition, they are responsible for meeting all Washington State and Sea Mar testing and certification requirements.
Duties and Responsibilities:
Pro-actively aid in increasing public awareness regarding health coverage enrollment opportunities by planning and conducting outreach activities within the community, participating at both Sea Mar sponsored and non-Sea Mar sponsored events.
Assist in the development and distribution of informational/promotional materials, and establishes and maintains referral relationships with local organizations, agencies, institutions and businesses as directed by the Managed Care Department.
They will routinely work uninsured lists from Sea Mar dental, behavioral health, and ancillary service sites to advance enrollment of eligible individuals onto health coverage and inform non
eligible individuals about health care options.
Assist consumers with online public health coverage and program applications, provide the necessary information for consumer to navigate health plan options and make well informed decisions.
Serve as a service navigator for consumers wishing to establish Sea Mar as their medical home,
by providing information regarding services available, and consumer rights and responsibilities.
Be responsible for assisting high risk patients in scheduling a care manager appointment and
facilitate processes to ensure they've established care. In using a case management model
incorporating home visits if necessary to assist patients who fail to keep their initial medical visit
by removing impediments by identifying and linking them to necessary resources.
Be responsible for maintaining daily activity records, submitting progress reports in a timely
manner, and assist in identifying areas for quality improvement and efficiency to the Managed
Care Department.
Assists in identifying, monitoring and tracking uninsured individuals and advocating on their
behalf regarding barriers to coverage and or access of health care services.
Performs all duties in a professional manner, participates at all mandatory state and corporate
trainings and meet all competency testing requirements, on health coverage policy changes
impacting consumers and or processes.
Perform any and all other duties as may be assigned while adhering to all corporate and
departmental policies and procedures.
Qualifications and/or Requirements:
Must possess good organizational and time-management skills
Demonstrate good judgment and have excellent problem-solving abilities
Maintain a professional demeanor at all times and maintain confidentiality at all times.
Must be proficient at public speaking.
Must have good spelling, grammar and written communication. Legible handwriting required
Must have the ability to read and interpret complex documents, reports as well as data.

Education and Experience:

This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.

Our Outreach team is looking for a part-time staff member to join our team. We are looking for candidates who want to travel throughout the county to connect and help our unhoused community members. The outreach specialist will be expected to engage individual's experiencing homelessness and assist them in accessing self-identified needs, such as, health, housing, behavioral health and basic needs. We are looking to fill 1 part time position at 20-25 hours per week.
JOB DUTIES AND RESPONSIBILITES INCLUDE:
Build rapport and connect with individuals who are living unsheltered, with the goal of connecting them
to supportive services.
Refer clients into case management, coordinated entry, or other agency programs, as appropriate.
Work with program coordinator to develop strategies on reducing the stigma regarding individuals with
opioid use disorder in the community.
Input and maintain information regarding referrals in database. Keep detailed case notes of client
interactions.
Build effective relationships with treatment providers and community partners for warm hand-offs, as
well as, assisting individuals in accessing appropriate resources related to pursuing recovery and
attending appointments.
Provide transportation resources to access treatment services and/or support in getting to an
appointments.
Provide on-going education and advocacy for individuals experiencing opioid use disorder, including
information on overdosing, Narcan, and Fentanyl testing strips.
Assist clients in navigating the intake system for treatment services.
Coordinate with case management staff to meet the individual needs of new and existing program
participants;
Assist program coordinator with collection, entry, and tracking of impact data.
Assist with recruiting, training, and onboarding volunteers.
Attends Community Action staff meetings and other meetings and trainings as required.
Other related duties as assigned by management.
QUALIFICATIONS INCLUDE:
Education & Experience:
Lived experience can be helpful and will be considered as a qualification alongside education,
work, and volunteer experience.
High school diploma or equivalent . Degree in Human Services or related field desired.
Experience working with homeless population, and those with substance abuse and mental
health disorders, preferred.
Or a combination of education and experience providing the knowledge, skills, and abilities to successfully
perform the work.
License(s) & Certification(s):
Must have valid driver license and auto liability insurance, if personal vehicle is used for work-related
travel.
Recovery Coach and Diversion Training preferred.
Training necessary (within 6 months of hire): Blood borne Pathogens, First Aid/CPR, Safety,
Mandated Reporting, Confidentiality, HIPAA, 42 CFR Part 2, Harm Reduction, Conflict Resolution,

Position Summary:
Full-time Community Resource Navigator position available for our Managed Care Department in Bellingham, WA. The role of the Community Resource Navigator is to provide information and assistance to individuals seeking health coverage, health care and other local resources that aid them in maintaining a healthy lifestyle. Helps in educating eligible individuals on how to navigate health insurance options, public programs, and Sea Mar services and assist with the application processes. Aid newly assigned and high-risk patients in scheduling their initial appointment for medical, dental and care management services. Provides case management and or participate in intervention strategies that remove barriers that may result in appointment no shows. In addition, they are responsible for meeting all Washington State and Sea Mar testing and certification requirements.
Duties and Responsibilities:
Pro-actively aid in increasing public awareness regarding health coverage enrollment opportunities by planning and conducting outreach activities within the community, participating at both Sea Mar sponsored and non-Sea Mar sponsored events.
Assist in the development and distribution of informational/promotional materials, and establishes and maintains referral relationships with local organizations, agencies, institutions and businesses as directed by the Managed Care Department.
They will routinely work uninsured lists from Sea Mar dental, behavioral health, and ancillary service sites to advance enrollment of eligible individuals onto health coverage and inform non
eligible individuals about health care options.
Assist consumers with online public health coverage and program applications, provide the necessary information for consumer to navigate health plan options and make well informed decisions.
Serve as a service navigator for consumers wishing to establish Sea Mar as their medical home,
by providing information regarding services available, and consumer rights and responsibilities.
Be responsible for assisting high risk patients in scheduling a care manager appointment and
facilitate processes to ensure they've established care. In using a case management model
incorporating home visits if necessary to assist patients who fail to keep their initial medical visit
by removing impediments by identifying and linking them to necessary resources.
Be responsible for maintaining daily activity records, submitting progress reports in a timely
manner, and assist in identifying areas for quality improvement and efficiency to the Managed
Care Department.
Assists in identifying, monitoring and tracking uninsured individuals and advocating on their
behalf regarding barriers to coverage and or access of health care services.

THIS IS NOT A FULL-TIME JOB. THIS IS A FULL-TIME VOLUNTEER POSITION WITH A MODEST LIVING ALLOWANCE STIPEND.

Position Summary
Patient Navigators work with various health services providers to assist patients to access appropriate care, establish a medical home, assure pre- and post-visit needs are met and facilitate brief health education encounters related to chronic conditions, medically complicated cases and patients with multiple barriers to clinical care. Navigators provide translation/interpretation services as needed. Duties may vary as each position is customized to meet the specific needs of the host clinic.

Essential Duties and Responsibilities include but not limited to the following:
Schedule appointments and navigate patients through health system barriers
Initiate communication with patients and assist with support in making referrals to address the full spectrum of patient needs
Participate in mandatory trainings, service projects and team building activities throughout their 10-11 month full time term of service.
Create informational materials and available resources to have at Sea Mar sites. These include posters, promotional programs, healthy diet and recipes, exercise information, handouts/take-away, as well as resource binders that provide information about available Sea Mar and community resources.
Create a community resource guide to share with patients and the AmeriCorps-Sea Mar group.
Review electronic health record charts and lists to identify patients in need of important health screenings and services. Schedules appropriate appointments according to patient needs.
Prepare mail outs with appropriate materials and patient identifier information.
Collaborate with Sea Mar medical clinic staff, quality improvement staff, and other Sea Mar departments as needed to identify patients in need of screenings, to inform care teams about preventative care, and to reach out to these identified patients.
Work with Sea Mar staff to roll-out additional preventive health screening and recall efforts to patients.
Participate in an AmeriCorps site health project. This includes community outreach, health interventions, and health events regarding, but not limited to, the following health topics: smoking cessation, diabetes education, nutrition, and healthy lifestyles, stress reduction, managing chronic conditions, and adolescent topics.
