As a Field Based CommunityHealthWorker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team - based structure and spend the majority of their time in the community engaging directly with members. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and / or behavioral health conditions is important for this role.
Expect to spend about 50% of your time in the field visiting our members in their homes or in long-term care facilities, in the King, Pierce, or Snohomish Counties, WA region. You'll need to be flexible, adaptable and, above all, patient in all types of situations.
Scheduled: Monday - Friday with some evening and weekend hours to attend community events.
If you reside near King, Pierce, or Snohomish counties, you will have the flexibility to telecommute* as you take on some tough challenges.

The CommunityHealthWorker (CHW) works in a population-based practice setting in Clark County. The CHW, a paraprofessional classification in the STI HIV Prevention and Harm Reduction Program, provides culturally competent community/public health services and works in a population-based practice setting in Clark County. The majority of health and outreach services are provided on-site at the Harm Reduction Center (HRC) located at the Center for CommunityHealth or other community locations according to protocols, and under the oversight of a Program Coordinator (PC), Program Manager (PM), and the Health Officer (HO). This position is responsible for providing in-person harm reduction services, inclusive of staffing the Harm Reduction Syringe Services Program and community outreach events, providing operational support during STI HIV clinic, as well as other programmatic duties as assigned.
Organizational responsibilities include understanding and promoting the public health mission of the Department; providing courteous, respectful, efficient customer service to all Health Department clients; honoring diversity of all department employees and constituents; participating in Health Department training; and striving for personal excellence in public health work.
This position reports directly to the Program Manager of the STI HIV Prevention and Harm Reduction Program and will work an 8-hour schedule Monday through Friday.
This is a full-time represented position. It is represented by Local 335.
This position has the option of a remote/hybrid work schedule possibility. However, the candidate selected must reside in OR or WA. No Exceptions.
KEY OR TYPICAL TASKS AND RESPONSIBILITIES
Duties may include but are not limited to the following:
* Demonstrates proficiency in using various computer applications (e.g. MS Word, Excel, PowerPoint, Smartsheet) as applicable to design or complete forms, correspondence, spreadsheets, data entry, etc.
* Effectively operates complex equipment and office machinery, which includes computers, printers, scanners, and facsimile machines.
* Provides courteous customer service to providers, clients, and the public. Responds to routine inquiries by phone or face to face; listens to questions and explains procedures, rules or functions according to relevant program guidelines; fulfills requests for information or directs clients to appropriate person or section.

* Supports the HRC STI HIV clinical services by performing tasks including client check-in and registration, specimen ordering and processing, and other duties as needed.
* Package and ship specimens in accordance with requirements for shipping biological substances.
* Support the planning and implementation of programmatic outreach activities and other special projects.
* Participates in program specific or department-wide evaluation activities.

Qualifications
Education and Experience:
High school diploma or GED equivalent and two (2) years of experience, training, or applicable community experience in counseling or health outreach.

About the Role
The CommunityHealthWorker serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.
To be successful in this role, you:
Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness.
Are comfortable working with at-risk populations, communities of color and sourcing local resources for persons with behavioral and/or physical health needs.
Experience using a health plan care management system or electronic medical record system.
Have a high school diploma or equivalent.
Are a certification Peer Specialist if you do not have minimum experience and education requirements.
You may have other special skill sets such as speaking another language or certification as a CommunityHealthWorker. Have a valid state issued drivers' license and maintain an acceptable driving record and transportation.
Essential functions and Roles and Responsibilities:
Responsible for engaging prospective care management program participants who have been identified through data analysis and referral sources to likely benefit from care management program offerings.

Provides follow up services via telephonic or face to face engagement with clients and service planning partners as needed to coordinate reminder calls, medication, and medical appointments, upon request from the care management team.

Job Summary:
The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS). The Health Services Coordinator preserves dignity and promotes independence for each resident while providing care and services according to each individual service plan and in accordance with MBK policy and procedure.
Duties & Responsibilities:
Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies and procedures that govern resident assessment.
Draft initial individualized Service Plans and update as needed.
Review service plans with responsible parties and Executive Director as requested by DHS.
Ensure continuity of the assisted living residents' total care regimen.
Under the direction of the DHS, provide training and education as needed on a range of essential topics including competent medication delivery, acceptable treatments, safety protocols, emergency procedures, accurate record, and state requirements to provide the best possible resident care.
Perform all assigned duties accurately and timely including required documentation.
Serve as a medication technician, if needed.
Provide coverage of job duties within the department during absences, either through assistance in finding coverage, or personal completion of duties.
Check vital signs as directed and look for signs that health is deteriorating or improving.
Perform basic nursing functions such as treatments, medication delivery and managing resident emergencies ensuring residents are comfortable, well-fed, and hydrated.
Maintain adequate inventory of resident care supplies and demonstrate a commitment to minimizing waste of supplies and equipment.
Coordinate prescription orders with doctors' offices and manage pharmacy delivery of medications including oversight of the central storage, tracking and delivery of medications, and ensuring Medication Administration Records are completed according to company policy and State regulations.

* Learns to analyze inventory trends and supervises inventory management, including ordering items, keeping stock, and liquidating stock and leveraging company resources to avoid outs and overstock.
* Assists manager or assistant store manager in evaluating and developing displays, including promotional, seasonal, super structures, and sale merchandise. Completes resets and revisions.
* Engage in weekly meetings with store manager or pharmacy manager
* Responsible for basic department pricing and making daily price changes; ensures proper signage is displayed at the store to support accurate pricing of products. Ensures any additional pricing tasks related to local regulations and/or regulatory compliance programs are completed accurately and within the required time frame.
* Assists with exterior and interior maintenance by ensuring clean, neat, orderly store condition and appearance, including requesting store or system repairs as required in manager absence, or as requested by manager.
* Assists with separation of food items (e.g., raw foods from pre-cooked) and product placement as specified by policies/procedures (e.g., raw and frozen meats on bottom shelves). For consumable items, assists in stock rotation, using the first in, first out method and restock outs.
* Has working knowledge of store systems and store equipment.
* Receives exposure to the analysis of financial & performance data for the store, pharmacy and clinic and to the analysis of asset protection data and action plans to reduce loss.
* Ensures compliance with state and local laws regarding regulated products (e.g., alcoholic beverages and tobacco products).
* Work as a group to complete the Intern Team Challenge and present to area, district and store leaders

The Health Specialist is an employee of Horizons Greater Washington. The Health Specialist at the Maret Site will provide health support for the Horizons students during the five-week program, as well as to the students who attend the Maret summer camp.

Position Description
The Health Specialist will play a crucial role in ensuring the health and safety of approximately 135 Horizons students and will also support, as needed, up to 50-60 students at the host school camp. The Health Specialist will work closely with the program staff and faculty to address any health concerns, implement preventive measures, and provide immediate medical support as needed. The Health Specialist will report directly to the Site Director and will collaborate closely with the administrative team and teaching staff to ensure a safe and enriching experience for all participants in the summer program.
Core Duties and Responsibilities
First Aid Provision: Be readily available to administer first aid to students and staff, handling minor injuries and illnesses during the program hours.
Medical Assessment: Conduct initial assessments of health-related issues, promptly addressing any concerns that arise and taking appropriate action based on the severity of the situation.
Emergency Response: Respond swiftly and effectively in emergency situations, coordinating with local emergency services if necessary and ensuring proper documentation
Record Keeping: Maintain accurate and up-to-date records of all medical incidents, treatments provided, and any pertinent follow-up actions, and complete and submit incident reports as needed, adhering to privacy and confidentiality regulations.
Medication Management: Upon completion of mandatory training on medical administration and management, oversee the administration of prescribed medications, adhering to strict protocols and ensuring compliance with relevant legal and safety guidelines.
Collaboration: Collaborate closely with program staff, faculty, and parents/guardians to ensure a comprehensive understanding of any pre-existing health conditions or concerns related to the participating students.
Health and Safety Policies: Assist in the development and implementation of health and safety policies, actively participating in the creation of a secure and healthy environment for all participants.
Participate in a mandatory Medication Administration and Management training prior to the start of the program.

GENERAL DESCRIPTION AND PURPOSE OF THE POSITION: This is a professional level direct service position with the members of Seattle Clubhouse as the primary recipients of services. The Clubhouse Rehabilitation Associate provides mental health rehabilitation services to adults recovering from serious mental illness in a clubhouse model program. The responsibilities include sharing in the operation of Seattle Clubhouse's programs. These programs include the work ordered day, social/recreational program, employment programs and education programs. The ultimate responsibility for the operation of these programs lies with staff. Central to this responsibility is the engagement of members in all aspects of clubhouse operation. This engagement is performed by helping members to experience the sense of belonging, with a focus on their strengths, talents and interests.
This position exists to assist King County adults with serious mental illness to maximize their optimal level of functioning within the community.
The Clubhouse Rehabilitation Associate is responsible to and is supervised by the Program Director of Seattle Clubhouse.
Primary Job Functions
Offer a positive supportive relationship that helps promote hope, choice, wellness and recovery for participants while working side by side on the work of the clubhouse.
Work side by side with the members of Seattle Clubhouse in program areas of work ordered day (

Provide education and resources that support illness self-management, self-determination, self-advocacy, and shared-health-care decision making

Provide assistance in identifying and linking to community-based services and supports (primary care, mental health and or substance abuse treatment, housing, financial benefits, etc.)
Provide and model advocacy to ensure access to service for the member where/when needed.
Help with identifying and/or re-engaging with family, friends, and other natural supports

The Mobile Integrated Health (MIH) Specialist works as part of a multi-disciplinary team to provide a variety of care in the community and home setting. The MIH Specialist plays a crucial role in improving patient health outcomes, preventing re-admissions to the Emergency Room, Rural Health Clinic, and Hospital level of care. The MIH Specialist serves as a communicator to other departmental team members when referral opportunities and care coordinator needs are recognized.
ESSENTIAL DUTIES AND RESPONSIBILITES
Delivers a patient care plan and orders prescribed by the Provider.
Manages the referral dashboard in the electronic health record.
Maintains knowledge of Ferry County Health nursing district policies.
Communicates closely with external care partners to achieve timely social, medical, and behavioral health referrals as needed.
Communicates clearly and professionally to deliver and document education about various health topics (such as diabetes prevention, hypertension, nutrition, and advanced care planning).
Analyzes lab values and integrates them into patient assessments and clinical decisions.
Provides a compassionate and calming environment for patients so any questions or concerns can be adequately addressed.
Monitors the referral dashboard and accurately updates patient statuses.
Inspects, cleans, and restocks the MIH truck to ensure that all the equipment and supplies are present, in good operating order, and ready for response.
Drives the MIH truck safely as a representative of the district while also following all Washington State statutes and revised codes.
Activates 911 emergency response when a patient at home is triaged and found to need emergency medical treatment.
Administers patient care following established active protocols.
Attends monthly Inter-Agency meetings with the Care Coordinator for collaboration and better understanding of local resources.
Cooperatively works with community-based organizations to provide comprehensive patient centered community nursing care.
Delivers MIH program updates to the Nursing department during staff meetings.
Checks work cell phone voicemails daily and responds to patients timely.
Reviews Meditech Workload messages daily and communicates clearly back to team members with status updates.
Documents care provided, assessments, and phone communication events same day the in electronic medical record.
Assists the Care Coordinator to conduct post-ER follow-up calls to identify patients in need of a potential MIH visit referral.
Delivers care in the home setting to patients identified as homebound with an active Rural Health Clinic care plan.
Manages, coordinates, and participates in task force project assignments.
Perform related duties as assigned by management.
