Essential Job Functions: Facilitate communication and patient empowerment in interactions with healthcare/social service systems. Troubleshoots any barriers the patient might have in adhering to care. Engage in the community through outreach to recruit and engage high-risk patients. Serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Knowledge and Skills: * Proficient in the Microsoft Office suite * Excellent oral, written, and interpersonal communication skills * Willingness to learn new skills Knowledge, Skills & Abilities * Assisting Office of Cultural Excellence and Health Engagement Zones teams with drafting grant applications, letters of intent, proposals, and other relevant duties related to proposal development. * Assisting Office of Cultural Excellence and Health Engagement Zones teams with work related to deployment of department materials for programming * Collecting relevant data for projects, database/spreadsheet development and maintenance for community partnership tracking, etc. * Assisting with the development or updating of relevant presentations * Supporting team members in staff meetings with various activities related to the outcomes of those meetings, etc. * Attending staff meetings and other meetings related to work duties. * Attending relevant meetings in the community and interacting with a diverse group of individuals and organizations. * Learning about the operations of Health & Hospital Corporation and its related divisions, to provide context for the basis of the work deliverables. Working Environment * Hasbrook Building in Office of Cultural Excellence for some projects, will be required to attend meetings or perform work remotely from the health center or hospital campus as well.
