Role Description:
The purpose of a CommunityHealthWorker (CHW) at Oak Street Health is to act as the bridge between our patients, community, and medical systems in order to remove barriers and increase wellness across all life domains. A CHW is a patient's advocate or liaison, accompanying patients through proactive in-person and phone outreach based on their care needs to promote health literacy and increase access to resources needed to live healthier lives. High levels of flexibility, problem solving, strong communication, and an intimate knowledge of the community served are required to be successful.
CHWs work closely with Medical Social Workers to manage patient care plans, support care team decision making, and coordinate clinical and complementary services needed to provide high quality health care and improve the quality and cultural competence of service delivery. CHWs are expected to work within their scope of practice. There is no expected clinical license for this position.
Core Responsibilities:
Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to coordinate patient needs
Manage patient referrals defined by the care team & collaborate with the Medical Social Worker on action plan
Facilitate communication between all identified parties involved in patients' care as needed (e.g., family members, caregivers, medical providers, community-based organizations)
Form relationships with and build an inventory of local community organizations that may benefit our patients
Connect patients to state and local community resources related to housing, transportation, food, and activities of daily living among other social and physical barriers to health.
Assist patients with completion of applications for accessing eligible benefits and resources
Promote goal setting and achievement to improve patients' quality of life and self efficacy with patients. Goal definitions are agreed upon by the care team
Meet with patients in patient-centered and patient-preferred locations (e.g., Oak Street Health center, patient's home, external medical provider facility, community setting)
CommunityHealthWorkers should plan to spend about half of their time outside of the center in patient-centered locations; this means having access to a reliable means of transportation to do so is required
Drive engagement with high risk individuals (e.g., completed specialty appointments, adherence to Post Discharge Visits) may include accompaniment to appointments
Complete referrals to organizations and agencies as needed
Deliver culturally appropriate health education in the areas where OSH has provided competency training to the CHW
Support care team decision making through participation in interdisciplinary team meetings
Document interactions with patients in electronic medical record in a timely manner while maintaining HIPAA standards and confidentiality of protected health information
Manage time, set priorities, work independently, and collaborate effectively with an interdisciplinary medical team

What we're looking for

The Pima County Health Department is seeking a CommunityHealthWorker (CHW) II position in the Office of Non-Communicable Disease Prevention Program. In this role, the CHW II will serve as a trusted liaison between Pima County, the community, health, and social service systems. This grant-funded position will be responsible for collaborating with internal and external partners to increase community member awareness, self-sufficiency, and access to services and other resources among diverse populations. The CHW II improves the quality and cultural competence of service delivery and is a key program component in the dissemination of education and resources to the community being served. This position reports directly to the manager, and duties may change as community funder, and/or department needs evolve.
Essential Functions:

* Design, deliver, and continuously refine a multi-component Train-the-Trainer program that builds long-term capacity within a minimum of four partner senior-housing organizations for senior physical activity opportunities.

POSITION SUMMARY
This position provides clinical assessment and psychotherapy services for defined patient population. Position provides treatment in one or more of the following areas: substance abuse, emotional and behavioral issues, eating disorders and other mental disorders as outlined in the "Diagnostic and Statistical Manual of Mental Disorders" (DSM).
CORE FUNCTIONS
1. Provides assessment and direct psychotherapy to individuals, families, and groups. 7. Provides critical assessments and appropriate treatment plan; Manages and provides direct care of patients in behavioral health treatment program (may be specialty area or across disciplines). Monitors, reassesses and documents treatment programs. Interacts with patients, families, groups, all levels of clinical staff in assigned program, physicians. External contacts include managed care providers, community resources, and referral agencies.
8. May provide indirect supervision to associate level therapists to support the process of becoming independently licensed. This may include weekly supervision and appropriate documentation.
9. May be required to review and sign off on clinical documentation, such as treatment plans and assessments from non-licensed staff within 72 business hours of completion.
MINIMUM QUALIFICATIONS
Requires a Master's degree in counseling, behavioral health, or a related human services field, or educational requirement as mandated by state board in order to maintain licensure in field of specialty.

Community Outreach:
* Create and conduct wildfire fuel reduction education focusing on rural and unincorporated areas and towns that have high fire risks in Pima County.
* Collaborate with Invasive Species personnel to address the role invasive species have in fuels management.
* Partner with CLR to develop outreach materials with consistent messaging on topics that include mitigation that coincides with the seasonal wildfire cycle, and creating defensible natural spaces around homes and businesses.
* Create Extension publications to educate residents, businesses, and municipalities about landscaping and property management to reduce wildfire fuels and increase resilience in Pima County.
Community Relationship Building:
* Build and coordinate relationships with agencies and organizations that are engaged in fire preparedness and wildfire-risk reduction. These fire-risk management entities include the Arizona Department of Forestry and Fire Management (DFFM), federal agencies, local fire departments and fire marshals, Pima County Office of Emergency Management, and Conservation Lands and Resources (CLR).
