Position Detail:
The Community Liaison serves as a vital connector between our healthcare services and the community. This role is responsible for driving business development and managing both inbound and outbound communications. The Community Liaison acts as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) engagement, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
Business Development & Outreach:
Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons
Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
Identify and pursue new business opportunities to grow patient census across service lines.
Represent the organization at community events, health fairs, and professional networking functions.
Patient & Family Engagement:
Educate patients, families, caregivers, and legal representatives (POAs, guardians) on the benefits and services offered through Hospice.
Serve as a compassionate and knowledgeable resource to guide families through the enrollment process.
Communication & Coordination:
Develop relationships with medical staff offices, hospitals and nursing homes; act as a consultant to medical staff and hospital committees, as required to provide hospice and end-of-life information
Facilitate seamless communication between referral sources and internal clinical teams.
Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
Market Leadership:
Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families
Act as the face of the organization in the local market, promoting brand awareness and trust.
Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field.
Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
Referral volume targets
Patient admission and conversion rates
Community engagement metrics
Timely follow-up and documentation standards
Relationship development with referral sources
Strong interpersonal and communication skills, with the ability to engage diverse audiences.
Knowledge of Medicare/Medicaid and other payer sources is a plus.
Knowledge of Hospice eligibility and levels of care is preferred
Knowledge of customer referral source CRM management systems for activity tracking
Ability to work independently and collaboratively in a fast-paced environment.
Valid driver's license and reliable transportation required.

As part of Brewers Community Foundation, the Intern will provide administrative and operational support to Brewers Community Foundation. Brewers Community Foundation (BCF) is the official charitable arm of the Milwaukee Brewers. This paid internship will run from February until the beginning of October.
Core duties for this role include, but are not limited to:
Assist in the day-to-day operations of Brewers Community Foundation including, providing administrative assistance to all members of the department, answering phone calls, data entry, and additional miscellaneous office duties
Assist with the preparation and execution of special events (e.g. 5K/10K Famous Racing Sausages Run/Walk, BCF Weekends, Drive For Charity, Ultimate Auction / Fan Packs)
Oversee the fundraising and logistical components behind the Brewers Community Foundation Scoreboard Message program by fielding calls from interested parties, helping to reserve their messages, coordinating with Entertainment Department to make sure request is fulfilled, etc.
Assist with BCF Grant program and fulfill Donation requests throughout the season
Interact with fans and BCF supporters at Brewers Community Foundation kiosk during Brewers home games at American Family Field
Assist with operation of Brewers Community Foundation's 50/50 Raffle during Brewers home games
Interface with other departments during shared events for Brewers Community Foundation and Milwaukee Brewers Baseball Club
The ideal candidate will have one or more of experience in customer service, as well as the ability to interact professionally with fans, and respond quickly and thoughtfully to any situation. Candidates with strong communication, writing, analytical and organizational skills are preferred.

Job Description
This PAID internship offers students and early-career organizers the opportunity to develop real-world skills in canvassing, fundraising, and community mobilization. You'll work alongside experienced staff to:
Engage Minnesotans in campaigns protecting clean water and community health
Recruit supporters and raise funds for grassroots advocacy
Collect postcards, letters, and calls to lawmakers to advance environmental and corporate accountability policies
Responsibilities:
Canvass door-to-door to inform and inspire the public about environmental and social justice campaigns
Recruit and mobilize supporters and new Clean Water Action members
Fundraise to sustain grassroots advocacy work

Intern will learn how to screen individuals for nutrition risk, provide nutrition counseling, document consumer interactions, and have exposure to the operations of a community based senior nutrition program.
* Aging and Disability Resource Center Social Work Internship - Seeking a bachelor degree level social work student. Opportunity for student to work with older adults and adults with disabilities learning how to provide information and assistance, options counseling and connection to resource and benefits. Students will have exposure to other service areas including: elder abuse investigation, senior nutrition programs, health promotion and prevention programs and Veteran Services.
CHILDREN AND FAMILIES
* Child Welfare Bachelor level Internships are designed for Social Work degrees. Focused on learning and participating in child welfare services to children and families within standards, ordinances, and policies set forth by Chapter 48. They may also work at community events, participate in treatment courts, and experience Access, Initial Assessment, Ongoing Services and Foster Care Services. * Child Welfare Master level Internships are designed for master of social work degrees. Focused on learning and participating in child welfare services to children and families within standards, ordinances, and policies set forth by Chapter 48. During the course of their internship, they will practice first in Initial Assessment and then move to Ongoing Services. They also will be able to experience Treatment Courts and Foster Care Services. During the second half of their internship, they will be responsible for a Child Welfare case and implementing services. Research of policies and best practice data, may be assigned. Job shadowing in all areas of Child Welfare is available as a part of this internship.
* Youth Justice Bachelor level internships are designed for social work, criminal justice, sociology, and psychology degrees. Focus is on learning and participating in youth justice services to adolescents and families within standards, ordinances, and policies set forth by Wisconsin Chapter 938. Job shadowing in the area of Youth Justice is part of this internship.
VETERANS
* In collaboration with the ADRC, Children & Families, and Behavioral Health Divisions, the Veterans Service Office offers internships for Bachelor of Arts and Bachelor of Science in Social Work candidates. Interns will learn about Federal and State Veterans Benefits navigation, counseling individuals on eligibility for programs, and how to interact with Veterans and their beneficiaries to determine individual needs and how available programs might address those needs.
BHU
* Behavioral Health Internships are designed for a broad range social service degrees. Focused on providing resources and support for those with a variety of mental health needs. These interns typically work with consumers by assisting mental health professionals with teaching coping skills, symptom management and risk assessments.
* Behavioral Health Case Management Internships are designed for students pursuing a Master's degree in Social Services (e.g. Social Work, Counseling). There are licensed clinicians (LCSW and LPC) on site that are able to provide clinical supervision and meet educational program requirements of practicum and clinical hours.
