S&S Registered Nurse Job Posting Constellation Health Services is seeking a Registered Nurse to join our start-up and survival team (S&S). As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Team members will manage the initial setup and transition to the permanent clinical team through onboarding and training. They will be or will become MatrixCare power users. Responsibilities: Delivery of all aspects within their scope of practice, RN CM, Oncall, Aides services where needed, and role has not been fulfilled yet by a home health aide in all environments that a patient calls home (skilled nursing facility, home, etc) Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges. Conduct thorough assessments to identify patient needs and develop individualized care plans. Manage medications and treatments as prescribed by the physician. Provide emotional support and guidance to patients and their families. Educate patients and families about hospice care and end-of-life issues. Collaborate effectively with interdisciplinary team members to ensure coordinated care. Maintain accurate and up-to-date patient records. Ability to travel up to 100% of the time within the assigned geography.
