Position Summary: A Clinic Assistant Offsite is placed inside a physician practice to collect and register lab specimens, schedule and pre-authorize diagnostic testing, and act as a general liaison to MMC. Their daily routine varies depending on the needs of the practice. They collect lab specimens, register them, and prepare them for transport to MMC for processing to the specifications outlined in the laboratory policies and procedures. They ensue that diagnostic testing is ordered to the provider and patient's preference and according to the guidelines set by the department and Central Scheduling. They also ensure that results reach the provider whether it be via EMR or fax. Clinic Assistant Offsite function as a representative of MMC and are often the main channel of communication between the practice and MMC. Clinic Assistant Offsite may also be trained to assist in multiple locations reporting to Offsite Services. Clinic Assistant Offsite function under the authority of the Offsite Services Manager; however, they are also responsible for maintaining the standards outlined by the Laboratory Director for all laboratory services they provide. Primary Responsibilities: * Schedules patients at physician request for diagnostic services * Verifies insurance coverage for services as well as whether a pre-authorization is required - obtaining the pre-authorization as needed * Communicates appointment information and preparation instructions to patients * Communicates with referring providers to ensure all clinical documentation needed is available at time of appointment * Ensures that provider receives results in a timely manner * Registers lab patients and ensures accuracy of information by utilizing AccuReg on a daily basis * Prints medical records for practice as needed to improve continuity of care * Assists with ease of referral to various other MMC services such as PT, CCC, etc. * Collects blood specimens via venipuncture or finger stick following MMC collection and infection guidelines * Prepares other body fluid specimens (urine, fecal matter, sputum, semen, cultures, etc.) for transportation to the lab * Maintains requested statistics, clinical documentation, and reports Qualifications This position requires, at minimum, a high school diploma or GED. Prior experience in the healthcare environment and/or customer service is preferred. Prior experience with or certification in phlebotomy is preferred. A working knowledge of computers and systems to include Microsoft Word, Excel, and Outlook are necessary. This program provides the entry-level training that will prepare individuals for work as emergency medical technicians (EMTs) at HEMSI. Individuals that successfully complete this program will be eligible to sit for the National Registry of EMTs (NREMT) examination at the emergency medical technician (EMT) level and will have an understanding of HEMSI-specific clinical policies and procedures. National Registry certification is required for State of Alabama EMT licensure. EMT course completion is also required to continue to Advanced EMT or paramedic-level education. Upon completion of the EMS Academy and licensing requirement, employee may be able to transition to full time employment as an EMT, Student agreement to include but not limited to the following items: * Student must obtain National Registry certification on the first attempt, after completion of the academy. (Pending testing availability, no more than 14 days post academy completion) * Students who do not pass on their first attempt will be transitioned from full-time employment to PRN employment. Use ACO tools for office/clinic staff review of patients coming due for their Annual Wellness Visits (AWV). Ensure review of patient record to identify gaps in problem list prior to provider visit, post reminders to secure that all co-morbidities are discussed and documented during AWV. Identify scheduling opportunities, determine special needs for patients arriving that office/clinic day, identify patients who need care outside of their scheduled visit, patients overdue for AWV and those with missed appointments needing rescheduling Qualifications Minimum Knowledge, Skills and Experience required: Education: High School diploma or GED required; post-secondary education preferred, medical assistant certification preferred. Experience: Previous 3 years medical experience, preferably in an office/clinic setting caring for chronic disease patients.

Checks out patients, collects co-pays and payments on account balances and completes end of day deposit according to policy. 10. Manages and maintains physician and clinic schedules appropriately. Updates office schedule regularly for patient additions/cancellations. 11. Completes assigned responsibilities in an appropriate time frame. 12. Demonstrates flexibility when involved in changing situations. 13. Demonstrates professionalism in appearance and actions. 14. Attends required meetings. Participates in staff development and education activities. 15. Maintains work area and lobby in neat and orderly manner. 16. Maintains patient confidentiality. 17. Centered in a clinician provider practice, this individual acts as a collaborative member of the clinical team in providing continuity of and quality patient care. Initiates patient care assessments, gathers vital signs, prepares patient and exam room for procedures, and collects specimens. Coordinates care for patients with chronic disease and manages effective care transitions for them within the continuum. Partners with the provider care team for successful preventative care visits to reduce the severity of chronic disease and avoidable acute illness. Provides effective clinical health coaching to assist patients with self-management of their chronic disease and life-style changes to mitigate health risk. Responsibilities Assess problems/conditions during workup and accurately documents in medical records. Assists provider in examination, treatment and referral of patient, preparing necessary paperwork, equipment, etc. This includes, but not limited to: giving injections, CLIA waived point of care testing, providing education, scheduling referrals and tests. Responsible for ordering and maintaining adequate supply levels. Responsible for training new clinical staff members within clinic. Maintains and assists in coordinating clinical coverage within the clinic in conjunction with network leadership team. Stratify patient population according to risk to effectively and efficiently manage patients, determine frequency of need for provider appointment. Maximize use of qualified clinical staff within the care management team for ensuring needed non-face-to-face patient contact. Ensure effective office/clinic staff internal tracking of test/procedure results, medication acquisition, missed appointments and adherence to follow-up appointments. Use ACO tools for office/clinic staff review of patients coming due for their Annual Wellness Visits (AWV). Ensure review of patient record to identify gaps in problem list prior to provider visit, post reminders to secure that all co-morbidities are discussed and documented during AWV. Identify scheduling opportunities, determine special needs for patients arriving that office/clinic day, identify patients who need care outside of their scheduled visit, patients overdue for AWV and those with missed appointments needing rescheduling Qualifications Minimum Knowledge, Skills and Experience required: Education: High School diploma or GED required; post-secondary education preferred, medical assistant certification preferred. Experience: Previous 3 years medical experience, preferably in an office/clinic setting caring for chronic disease patients.
