This position is responsible for the implementation of new or updating of existing data systems, databases, data models and/or data visualization tools in order to create easily accessible, accurate and secure data that can be utilized for one or more of the business teams within Revenue Operations. In order to achieve this, the Data Management Analysts will coordinate multiple stakeholders including IT, external vendors and business teammates in combination with needed technical skills & knowledge of DaVita's data & data systems.
Key Responsibilities
User and Group Management: Adding, removing, and updating user accounts and groups.
Project Creation and Management: Organizing content within the site using projects.
Permissions Management: Assigning access rights to users and groups to control what they can see and do within the site.
Data Source Management: Ensuring data sources are configured correctly and accessible to users.
Content Management: Promoting, certifying, and managing content within the site.
Monitoring and Troubleshooting: Monitoring site usage metrics and troubleshooting user issues.
Software Upgrades: Coordinating with our server admins to test and deploy new versions Tableau.
Data Source Refresh: Ensuring data sources are refreshed regularly to keep data up-to-date.
Essential Function:
Analysis & Technical
Builds data systems, data models or assists in the building of databases through proficiency in technical skills needed (ex. Data modeling in Tableau, SQL, Oracle, Netezza)
Independently exports, transforms and loads data tables into data repositories in order to support maintenance of multiple databases
Demonstrates thorough understanding for a specific business lane or specific scope of data, the source systems where that data exists, the data repositories that it can be accessed from, and for how data gets from its source system to repository

Solves problems by identifying key drivers & developing hypothesis based approach

What Skills Do You Bring to the Role?
Bachelor's degree in related field of study (STEM, Mathematics, Statistics, Economics, Business)
2 to 4 years' experience OR applicable work experience; Master's degree preferred
Strong Excel and PowerPoint skills required; experience using other analytical tools such as SQL, Tableau, VBA, Python, Access, etc. preferred
Strong quantitative & problem solving skills including analytical modeling and insight generation
Ability to communicate complex ideas in a simple, easily digestible manner to a range of audiences including senior leaders

This position is responsible for improving the patient education process by analyzing and sharing best practices with other members of the department.
Patient Education
Communicate with and treat patients with empathy at all times. Meet individually with patients requiring outpatient dialysis placement, depending on program scope, within the assigned region to provide educational materials on: Outpatient dialysis, Modality options, Vascular Access options, insurance which includes an assessment of patients insurance, out-of-pocket costs and grant program options. Provide education on options for patient to select an out-patient dialysis center of their choosing.
Identify, resolve, or escalate patient issues and/or barriers to placement to appropriate teammates for prompt and effective resolution.
Outpatient Dialysis Placement Referral
Work with hospital Case Managers, Social Workers, and Physicians to place patients in outpatient dialysis clinics. Arrange out-patient kidney dialysis placement for patients by contacting local clinics within an appropriate proximity as determined by patient preference and providing all required medical documentation to the outpatient dialysis center. Assist with patient transitions from hospitals to dialysis facilities, including transfer of paperwork and, as much as possible, ensure patient satisfaction. Place patients and document referral in accordance with Patient Pathways Policy and Procedures.
Relationship Building
Work closely with both internal and external kidney care professionals to enhance current relationships or develop new relationships. Partner with Regional Operations Directors and Facility Administrators to identify and implement best practices for accepting patients from inpatient acute care settings into an outpatient dialysis provider. Attend operational team meetings with multi-affiliate providers in assigned region.
Reporting
Conduct operating reviews with management to review reports at a minimum of one time per month. Record patient data in electronic tracking system according to Patient Pathways Policy and Procedures. Immediately inform Regional Operations Manager/Supervisor of any reporting requests from the hospital. Assist Regional Operations Manager/Supervisor in gathering patient details as needed for reporting purposes. Attend hospital meetings to review reporting as needed. Adhere to all audit, compliance, and reporting requirements according to Patient Pathways Policy and Procedures.
Innovation
Learn, develop, and implement new ways for Patient Pathways to differentiate itself in the marketplace. Ensure Patient Pathways leadership is apprised of customer needs and any deviations from current practices. Participate in special projects as needed or requested. Act as a liaison or subject-matter expert for the Business Development team on special initiatives. Educate team local markets on Patient Pathways program and initiatives. Mentor team members, as deemed appropriate by Patient Pathways leadership.
Qualifications:
* Associate degree is required, Bachelor's Degree in social work, nursing, or related feed strongly preferred
* Knowledge of health systems and community resources required.
* Strong communication skills with the ability to work collaboratively with healthcare providers and community resources required.
* Strong organizational and time management skills with the ability to prioritize tasks and manage multiple projects simultaneously, required.
* Knowledge of Medical terminology and the ability to read and interpret medical records is required.
* Proficiency in the use of electronic medical records and other technology preferred.
* At least one year of discharge planning and/or case management support role/experience, or experience working with dialysis patients preferred.
* At least one year of working with physicians preferred.
* Current certification in case management/social work, or willingness to obtain certification is preferred.

**JOB SUMMARY**
The Patient Service Technician is responsible for delivering and picking up equipment from homecare patients.
**ESSENTIAL DUTIES AND RESPONSIBILITIES**
+ Delivers and picks up equipment from homecare patients. Sets up equipment and instructs patients on the basic use of equipment. Assists patients in solving problems regarding equipment use.
+ Schedules and organizes daily deliveries and pickups according to geographic territory. Loads and unloads durable medical or infusion equipment, oxygen and medical supplies onto delivery vehicle.
+ Complies with all applicable regulatory requirements including providing documents needed for the driver qualification file and abiding by hours of service, daily log, hazardous materials (placarding, training and shipping papers) and vehicle inspection and maintenance regulations.
+ Completes necessary paperwork including daily delivery logs, pick-up and delivery forms, reimbursement documents and vehicle maintenance logs. Distributes paperwork to appropriate personnel.
+ Notify supervisor if any one of the following items is discovered in the patient's home:Physical abuse of any family member Home environment is unsafe for the care ordered, such as unsanitary conditions or insufficient food for children Animals in the home that cause unsanitary conditions
+ Maintains daily vehicle maintenance logs and incident reports. Ensures vehicles are in proper working condition. Ensures preventive maintenance on vehicles is performed on a timely basis according to D.O.T., F.D.A. and Joint Commission.
+ Responds to emergency delivery calls as needed and on an on-call basis.
+ May clean and assist with the repair of equipment.
+ Performs routine vehicle maintenance including the checking of gasoline, oil fluids, tire pressure and brakes. Reports malfunction to supervisor.

Clinical Outlier Support:

* Lead efforts with clinic teams to identify and address root causes of PRD and hospitalization outliers by removing barriers and providing targeted coaching.

GENERAL PURPOSE OF THE JOB
This position is responsible for supporting process improvements, metric/report design and development, special projects, and providing detailed revenue valuation analysis to ensure departmental goals and objectives are achieved. The analyst will utilize one or more of the following - MS Applications, Power BI, SQL Server, SSIS, Oracle, VBA, Tableau, Alteryx, and other approved technologies as needed to support department goals. This includes supporting processes for accounting for center and divisional revenue review, accounts receivable, cash and related transaction analysis, account reconciliation and preparing financial reports for corporate and field operations management, and joint venture partners, as well as internal and external audit agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Technical / Reporting / Analytics:
Builds data systems, data models or assists in the building of database through proficiency in technical skills needed (ex. Data modeling in Tableau, Alteryx, Power BI, SQL, Oracle, Netezza)
Independently exports, transforms and loads data tables into data repositories to support maintenance of multiple databases
Using SQL/SSIS/MS Excel/MS Access/Alteryx prepare, design, modify, and develop selected recurring and ad-hoc financial schedules and related revenue trends and fluctuations for management reporting
Manage the team's data sources by identifying system and process opportunities and investigating data inconsistencies
Become proficient in all areas of the revenue cycle and able to support and provide recommendations, via business acumen and technical knowledge
Design and create tools and processes using Tableau/VBA/SQL/MS Access/Power BI/Alteryx used to assist accounting team
Problem solving and critical thinking:
Exercise sound judgement to determine appropriate actions that will assist in breaking down complex problems to develop and implement solutions.
Demonstrated ability to function in a team environment as well as independently and have the ability to build strong relationships across departments/teams.
Inquisitive and self-started with a high degree of initiative, urgency, attention to detail, and follow through.
Project & Stakeholder management:
Effectively coordinates working sessions & facilitates interactions between business team and/or Accounting/IT teams (ex. Requirement review sessions, project status meetings & post-deployment feedback session); synthesizes and sends meeting recaps induing next steps.
Directs & executes on next steps to progress own work stream; identifies & recommends new opportunities to improve existing processes.
Maintains up to date documentation of data/process dependencies for a lane of business and needed requirements/updates for data systems.
Proactively communicates around deadlines & roadblocks.
Qualifications
Bachelor's degree preferably in STEM, Economics, Statistics, Computer Science, Business.
At least 3 years' experience as a data or reporting analyst.
Experience with data modeling in various data systems like Tableau, Alteryx, Power BI, and/or with data relationships and data migration from data repositories to source systems (ex. SQL, Netezza, Oracle)
Strong written, verbal and interpersonal communication skills with ability to probe, question, and listen attentively.
Database and analysis experience with ability to manipulate large volumes of data and to work effectively at both detail and big-picture levels.
Advanced background experience in using Tableau, SQL, SSIS, MS Access and VBA (or other coding, ex. Python).
Expert in data analytics and reporting. Proficiency in Alteryx and Tableau preferred.
Organizational and time management skills with the ability to manage multiple projects simultaneously with tight deadlines.
Preferred relevant experience in healthcare revenue cycle.
Preferred working knowledge of accounting functions.

**JOB SUMMARY**
The Delivery Technician is responsible for delivering durable medical equipment to Apria patients in the home.

+ Pulls, packs, delivers and picks up medications, supplies and basic equipment.
+ May instruct patients on basic use of respiratory therapy equipment, durable medical equipment, oxygen equipment, and other equipment/services approved by state licensure limitations.
+ Loads and unloads durable medical and infusion equipment onto delivery vehicle.
+ Responds to emergency delivery calls as needed during regular business hours and on an on-call basis.
+ Completes necessary paperwork including daily delivery logs, pick-up and delivery forms and vehicle maintenance logs. Distributes paperwork to appropriate personnel.
+ Maintains supply component of patient profiles.
