What is Community Comprehensive Services (CCS)?
A state certified mental health program that focuses on psychosocial rehabilitation
Serves both adults and children who have mental health or substance abuse diagnosis and need more than outpatient services, but less intense than that of the Community Support Program and have medicaid
A wraparound model of service provision, meaning all of the systems that a consumer is involved with work together to support the consumer.
Systems are both formal and informal and can include everyone involved in their life such as family, friends, religious organizations, and other natural supports.
CCS is a recovery oriented and strengths based program.
Requires consumer participation in all aspects of treatment including assessment and service planning.
A consumer's goals are their own stated goals.

The Program Supervisor has day-to-day responsibilities for one or more programs within the Business Unit. Responsibilities include but are not limited to, effective coordination of day-to-day program operations, direct supervision of program staff, ensuring the program meets licensing and/or contract requirements, ensuring agency best practice standards are met.

Establish priorities and directs the work of program staff to include delegating and overseeing work assignments.

Select, train/teach, assign, coach and evaluate assigned LSS employees.
Build trusting and open relationships with staff through open dialogue as well as intentional and ongoing employee conversations.
Hold direct reports accountable for the responsibilities associated with their role.
Support day-to-day functioning of assigned employees in support of the agency goals and strategies.
Ensure communication and implementation of agency-wide policies and procedures to program staff.
Schedule and facilitate staff meetings on an ongoing basis.
Work collaboratively with employees and other leaders to problem solve and provide solutions to issues encountered by direct service employees.
Provide ongoing support and guidance to employee which promotes autonomy and critical thinking.

Stakeholder Relations:
Establish and foster healthy and positive relationships with community stakeholders, funders, and donors.
Participate actively and represent LSS with marketing and communications to establish and reinforce a positive image of LSS in the community.
Educate the community (market the program) about the services provided at LSS.
With a focus on program expansion, communicate any need for additional services to leadership.
Demonstrate community commitment with follow through and engagement in the community.
Receive, track, and coordinate referrals effectively and efficiently for your service area.
Work as a collaborative team member with other leaders to support One LSS.
Hold self and team accountable to promote and create a safe environment for employees, clients and stakeholders by identifying and resolving risks as appropriate, and following applicable safety procedures.

Assign/delegate job tasks under the direction of the Store Manager and holds team members accountable for established best practices and standard operating procedures.
Maintains top-flight condition and appearance of store and donation area by focusing on housekeeping and safety at all times. Communicates needs for repair, fixtures, maintenance, etc.
Completes monthly Store Inspections, Health/Safety Checklist and monthly drills.
Works collaboratively within department to maximize service opportunities and maintain a continuous improvement agenda.
Completes staff schedules.
Completes and provides insight to the Store Manager on team member performance reviews.
Recommends corrective actions to the Store Manager by providing supporting documentation while maintaining confidentiality at all times.
Completes the store's opening and closing procedures as needed.
Completes, verifies, and submits daily paperwork, deposits, and reports as directed by the Store Manager.
Completes and submits Truck Orders in a timely manner.
Communicates any new or unusual information promptly to the Store Manager.
Helps prepare outgoing truck loads including baling, sorting, operating forklift, organizing, etc. and assists truck driver with loading/unloading.
Minimizes waste expense and maximize salvage revenue opportunity by recycling as many products as possible in most resourceful way.
Ensures that equipment is in safe operating condition and that all safeguards are in place.
Operates forklift truck.
Completes all duties of the job and all work requirements.

About the Role
Lutheran Social Services (LSS) is seeking a compassionate and detail-oriented Forensic Care Coordinator to provide comprehensive, person-centered case management for individuals involved in Wisconsin's forensic programs. This role requires ongoing professional development in Motivational Interviewing and other evidence-based practices, with monthly coaching and quarterly fidelity reviews.
You will work with clients in one or more of the following programs:
Conditional Release (CR): Supporting individuals found not guilty by reason of mental disease or defect.
Outpatient Competency Restoration Program (OCRP) & Jail-Based Competency Program (JBCR): Assisting adults adjudicated not competent to stand trial but likely to regain competency.
Opening Avenues to Reentry Success (OARS): Voluntary program supporting individuals transitioning from correctional institutions back into the community.
Supervised Release (SR): Coordinating services for individuals committed under WI Statute 980 (sexually violent persons) as they transition from secure treatment centers to community living.
Forensic Care Coordinators collaborate with treatment teams, community providers, and natural supports to promote independence, recovery, and successful reintegration.
Key Responsibilities
Conduct assessments of client needs using required tools (e.g., functional screen).
Develop and implement individualized service plans with clients, families, and providers.
Coordinate and authorize services according to service plans.
Provide trauma-informed, evidence-based direct services.
Maintain accurate documentation and electronic client records.
Testify at legal proceedings and prepare court documents as required.
Facilitate person- and family-centered team meetings.
Collaborate with community resources, treatment teams, and supervisors to determine interventions.
Participate in staff development, supervision, and professional seminars.
Meet productivity expectations and agency standards.
Travel daily to community locations; occasional overnight travel may be required.
Qualifications
Education: Bachelor's degree in a human services field (examples include Social Work, Psychology, Criminal Justice, Counseling, Community Mental Health, Rehabilitation Counseling, Vocational Counseling, etc.).
Experience: Prior experience in human services preferred.
Licenses/Certifications:
Valid driver's license and reliable transportation.
Satisfactory driving record per LSS Driver Safety Procedure.
Ability to meet LSS auto insurance requirements.
