This assessment should be linked with an ongoing assessment of the strengths and weaknesses of the program's services in relation to those of other providers and to the needs of the community. The business plan is supported by an effective public relations program, developed by the Home Health Administrator, that builds community understanding and support of the program and services and those of the national organization. Organization The Home Health Administrator should create and monitor the organizational structure of the program to ensure its effectiveness and responsiveness to those being served. The nature of the structure is dynamic and flexible, responsive to needs of the clients. The Home Health Administrator should develop an organizational plan that clearly assigns responsibilities for the program's services to functional departments and to individuals. The Home Health Administrator should develop, recommend and implement a plan for continuity that ensures ongoing stability of the program. Such a plan should include performance standards that are stated in terms of continuous improvement targets, opportunities for internal and external development, and a system for evaluating the performance levels of all employees, consultants and other service providers. Operations (Management) The amount of time that can be allocated to essential activities that call upon the visionary and strategic skills of the Home Health Administrator is dependent upon the effectiveness of the day-to-day operations of the program. The Home Health Administrator must create, sustain and evaluate on an ongoing basis an infrastructure of people who take overall responsibility for implementing the program's service strategy and for initiating corrections that keep the strategy on course. The infrastructure should be supported by the organization's general policies and guidelines, and by program-specific systems and processes that are customer-responsive, efficient and continually improved by collecting and analyzing relevant data. These systems and processes must include mechanisms that ensure compliance with the regulations, standards and criteria of various health, governmental, professional and accrediting agencies. The Home Health Administrators responsible for the developments of personnel qualifications, policies and procedures. The Home Health Administrator may delegate these tasks to other appropriate personnel such as the clinical manger while retaining the responsibility for assuring the tasks and related duties are completed properly. The Home Health Administrator serves as permanent chair and/or leadership responsibilities of the program's Safety Committee or programing, marketing activities, Wound Committee, if applicable, Quality Improvement programming and related activities, allocation of resources, initiation of project team solutions and monitoring results. Assures that a clinical manger is available during all operating hours. When the Home Health Administrator is not available, a qualified, pre-designated person, who is authorized in writing by the Home Health Administrator and the governing body assumes the same responsibilities and obligations as the Director of Support Services. The pre-designated person may be the clinical manager, (484.105) (b) (1) (2) (3). An individual is pre-designated to fill the Home Health Administrators role in the absence of the Home Health Administrator and will be qualified to do so. Pre-designated means that the individual who is responsible for fulfilling the role of the Home Health Administrator in his/her absence is established in advance and pre-approved by the governing body. Responsible for coordinating with local pharmacies to obtain urgent medications that are not available through on-site dispensing. Responsible for obtaining signed prescriptions for required controlled substances and delivery of those signed prescriptions to the dispensing pharmacy. Responsible for entering all medication orders for new enrollments, setting up pharmacy cycle, delivery system, assistive systems if needed, linking diagnosis information to medications (through H&P), and verifying all information in writing with participant's primary care provider. Also responsible for coordinating medications for dis-enrolling participants to provide needed medications for a period of twenty-eight (28) days after leaving program. Alternative Medication Delivery Systems: Responsible for filling alternative medication delivery systems (MRB, Med-E-Lert, etc.) in accordance with medication orders and organizational policies/ procedures. Responsible for in-house monitoring of participant INR values, dose adjustment, and dispensing of Coumadin for participants, under the direction and supervision of Medication Manager. Responsible for making medication changes to currently filled delivery systems in accordance with medication orders and organizational policies/ procedures. Responsible for coordinating delivery of medications from pharmacy to participant either through direct carrier shipping or delivery through or from PACE center. Ministry Program The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant's condition to primary caregivers, physician and care team. Required Qualifications: Graduate of a school of professional nursing. Current Colorado Registered Nurses (R.N.) License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred. Must have a valid driver's license, proof of insurance and have means of transportation. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Preferred Skills: Knowledge about the PACE model and philosophy of care Essentials: Functions as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with teammates during meetings or one-on-one conversations. In coordination with the Care Manager and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts nursing re-assessment semi-annually and as needed. Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants. Receives participant complaints, triages, and responds to participant needs. Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy. VOANS is seeking a Social Worker Intern who is about to graduate with a Master's Degree in Social Work (MSW) from an accredited institution. This role is ideal for individuals who plan to become a Licensed Clinical Social Worker (LCSW) and are eager to gain practical hands-on experience in a supportive and professional environment. Candidates must be willing to relocate as part of this opportunity. This position extends the opportunity for full-time hours (30 - 40 hours per week) This position will be in the Eckert or Montrose, CO area OR must be willing to relocate to the area. The internship is unpaid, however, a stipend is provided Primary Responsibilities: Provide direct support and case management services to clients under supervision.
