    The Complex Case Manager (CCM) provides people experiencing homelessness with complex high acuity needs or chronic homeless designation with nuanced case management engagement for solutions to housing placement. RESPONSIBILITIES General Initiates and maintains good relations with all Denver Rescue Mission (DRM) departments. Maintains the relationships necessary to ensure DRM's case management efforts reflect a real-time understanding of broader re-housing efforts, availability, requirements, and protocols. Core Provides housing focused case management to the most complex cases in our city with the goal of connecting clients to long term solutions, paying particular attention to coordination of services and care as well as acquiring vital documents, benefits, and providing housing navigation. Completes documentation such as: case notes, service recording, entries into HMIS/Scribe within 48 hours of client contact. Responds to community collaboration such as emails, phone calls and appointments thoroughly, professionally, and within three business days. Transports individuals to appointments that will aid in stability and housing acquisition using company vehicle. Services clients both inside DRM properties and out in the community at appointments, outdoor areas, and public places. Meets a minimum number of client contacts per day as well as housing placements per quarter. Demonstrates respect, cultural competence, and knowledge of trauma-informed care in interactions with participants, staff, and other community providers. Develops a case plan with individualized goals based on individual acute needs. Identifies action steps necessary to achieve goals and monitors progress through scheduled follow up meetings. Performs comprehensive assessment of participant needs. Recommends the necessary level of intervention. Develops and regularly modifies treatment service plan as appropriate. Encourages and holds participants accountable to program guidelines. Monitors and assesses participant progress towards identified goals. Works collaboratively to identify and assess escalating crisis situations and responds with appropriate interventions. Maintains professional best practices in areas of document storage, confidentiality and professional ethics when dealing with clients and client information. Oversees participant case files to ensure progress. Regularly updates all case planning records, services, and case notes in SCRIBE including uploading vital documents and ROIs. Documents all meetings, referrals, and services in SCRIBE and utilizes the appropriate templates. Attends staff meetings and is prepared for case manager and team meetings to address critical needs and provide creative solutions for engagement and resolution. Provides SCRIBE reports, spreadsheets and statistics as requested. Facilitates community provider appointments with clients including case note documentation in SCRIBE (Denver Human Services, Medication Assisted Treatment, Veterans Administration, Social Security Income/Social Security Disability Income Attorney, Rocky Mountain Human Services, Colorado Access). Collaborates, communicates, and advocates with service providers in the government, medical, mental health, housing, benefits acquisition, employment, legal, and veterans, etc. fields to aid clients toward a successful transition into long-term programs or sustainable housing. Collaborates with other DRM personnel regarding participant evaluation, goals, and continuity of care. Engages in ongoing training as assigned by direct supervisor and leadership team. Seeks to develop a continuing knowledge of the field, what services are provided in the community, and establishes communication with those entities. Provides high level of attention to detail in schedule, community connection, documentation while maintaining creativity and flexibility required to establish progress with a high need caseload. Other duties as assigned. Requirements MINIMUM QUALIFICATIONS Bachelor's degree in human services or related field. 1 year of experience in working in underserved populations. A combination of education and experience can be substituted. Colorado Certified Addiction Specialist (CAS) certification or equivalent preferred. Ability to obtain and maintain CPR certification within 90 days of hire date. Must have a valid driver's license and willingness to drive or meet with clients in the community. Proficient in computer skills as well as verbal and written communication. Time management and organizational skills, being able to independently prioritize the order of tasks and their completion. High level of accuracy and attention to detail. Ability to professionally engage with clients that are experiencing mental, physical, behavioral and hygienic issues. Schedule will include some early mornings and/or late evenings. High level of “team integrated work” requiring strong partnership mentality. Active driver's license as a limited amount of work-related travel may be required. Victim Advocate Specialists deliver statutorily mandated and supportive services to victims and witnesses of crime throughout the prosecution of criminal cases. JOB SUMMARY: Victim Advocate Specialists provide direct services to victims and witnesses of crime that include offering information about victim's rights, critical event notification, emotional support, information and referral assistance to community-based victim service agencies, and other criminal justice related services that support witnesses and aid victims of crime in their efforts towards recovery. Victim Advocacy * Inform victims of their rights as per Colorado statutes and the role of the Victim Witness Services Unit. * Offer practical explanations of court processes, criminal justice procedures, and legal terminology specific to district, county, and juvenile courts. * Educate victims on potential emotional, mental, and physical reactions to their involvement in the criminal justice system, providing necessary support for participation. * Communicate consistently with victims on all cases and maintain appropriate documentation. * Identify and address barriers such as childcare, transportation, or language needs, providing solutions or referrals. Court and Legal Process Assistance * Conduct courtroom orientations and accompany victims to court hearings as requested or deemed necessary. * Provide information on procedures, victim impact statements, and sentencing processes. Assist victims in completing forms and understanding their rights. Case Management and Referrals * Screen victim impact statements for restitution requests, compensation referrals, and other concerns, conducting necessary follow-ups. * Provide referrals to resources including victim compensation funds, mental health services, social services, medical resources, and financial assistance programs. Administrative and Front Desk Duties * Assist with answering calls, updating staff on victim and witness check-ins, and supporting the front desk as needed. * Maintain accurate records and documentation of interactions with victims. Determine clinical appropriateness for withdrawal management services using ASAM 4th Edition multidimensional assessment.
