    The Central Texas VA Health Care System, Temple, Texas is seeking a full-time Podiatrist (Home Base Primary Care Medical Director) to provide clinical oversight of all Non-Institutional Care (NIC programs to include all Home and Community Based Services, Contract Nursing Homes and State Veterans Home Programs) in collaboration with NIC program Manager. The HBPC Medical Director is the senior clinician leader and is responsible for the overall medical care delivered by the HBPC team. He/she works in collaboration with the HBPC Program Director to meet the requirements of the program. The incumbent may have a faculty appointment and be involved in academic activities with affiliated Medical Schools, residency and fellowship programs. The incumbent provides patient care directly and/or through the supervision of licensed independent practitioners (LIPs), residents, fellows and/or medical students in the home setting. In addition to their role as clinician, the HBPC Medical Director will have administrative responsibilities for leadership and direction of HBPC development and programs, as well as effective interface with the HBPC staff. The HBPC Medical Director will participate fully in all accreditation processes, reviews, and site visits. The incumbent will provide direct patient care which includes assessments, diagnosis, treatment, and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations, and scope of practice. Collaborate with in house specialty clinics as well as community providers. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. Administrative Responsibilities: The HBPC Medical Director is responsible for collaborating with the HBPC Program Director to: 1.Provide leadership to the HBPC Program. 2. Determine appropriateness and eligibility for veterans referred to HBPC program. 3. Plan and direct the educational and clinical experience of medical students, residents and fellows assigned to the HBPC Program. 4. Assume a leadership role in the development and implementation of the HBPC performance improvement plans. 5. Advocate for HBPC with VHA leadership and the medical community. 6. Jointly select HBPC Team members. 7. Providing clinical input and oversight for all patient treatment plans. 8. Being readily available to the team members for collaboration when medical or other problems arise. 9. Participating in HBPC interdisciplinary team meeting and other meetings as arranged to ensure clear, effective communication. 10. Keeping the HBPC team apprised of medical care advances and practice standards through regular educational activities. 11. Arranging physician coverage and communicating the plan of coverage to the HBPC team. 12. Collaborating with other HBPC Medical Directors, VISN leadership, and VA Central Office staff on program development issues. Clinical Responsibilities: The HBPC Medical Director, in collaboration with the HBPC interdisciplinary team, is responsible for: 1. Assuming primary medical responsibility for HBPC patients. 2. Identifying the patients' medical problems. 3. Defining the medical management of these problems. 4. Determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics as well as community care. 5. Determining the need for, and facilitating, admission to the hospital. 6. Visiting the HBPC patients at home, when appropriate. 7. Serving as collaborator for advanced practice nurses and/or physician assistants. 8. Providing medical administrative support such as signing forms requiring physician signature for the HBPC patients. 9. Possess DEA licensure to prescribe controlled substances II - V. Other Responsibilities: 1. Collaborating with community home health and hospice agencies as needed for HBPC patients. 2. Following HBPC Staff Safety procedures. 3. Complying with Medical Center and VHA regulations regarding the use of government vehicles. 4. Demonstrating work practices that include adherence to Infection Control standards and the safe use and operation of equipment. The PM&R Musculoskeletal Physiatrist will primarily provide direct patient care in a high volume outpatient consultation model with teaching and supervision of PM&R residents and supervision of PM&R therapy staff; will make contributions in PM&R medical care, education and research to be suitable for an academic appointment at the affiliated medical school and possess leadership skills in coordination of activities of various ancillary staff, house staff, therapists, other rehabilitation professionals, and students. The Supervisory Podiatrist, PM&R should have experience in managing and rehabilitating patients who have disorders of function across the spectrum of rehabilitation-related illnesses; demonstrate knowledge of preventive medicine and primary care as related to PM&R (e.g. complications of disorders & treatments, side effects of medications used), the role of non-physician health care professionals and health care evaluation. It is expected that he/she be a full-time employee, appointed on a career basis, with the expectation that tenure will span several years. Functions or Scope of Assigned Duties: Administrative: * Leadership of interdisciplinary team activities. * Reviews orthopedic surgery and/or PM&R consults for medical appropriateness * Participates in peer reviews * Maintains medical licensing and Board certification requirements * Obtains resident supervisory privileges through medical school affiliate * Supervision of (including but not limited to) IPU-assigned Physical and Occupational Therapy staff in collaboration with the Outpatient Therapy Supervisor, PM&RS * Customer service: includes responding to service concerns, completing Patient Advocate, White House or Congressional complaints Clinical: * Performs a comprehensive, detailed, targeted, initial history and physical examination in compliance with national guidelines. Utilizes diagnostic point-of-care ultrasound as necessary to refine differential diagnosis and treatment plan. * Orders diagnostic studies such as laboratory tests, X-rays, electromyography (EMG), electrocardiograms (EKGs) and other tests as indicated. * Consults other disciplines, as necessary, to complete the patient examination; e.g. Occupational Therapy, Physical Therapy, neuropsychological testing, audiology and speech language pathology testing, vision testing, dental evaluation, psychosocial evaluation, etc. * Carries out health promotion and disease prevention activities including education and shared decision-making. * Writes Medical Orders and prescriptions for non-controlled substances, as necessary, for the care of the patient in accordance with Federal and State law. * Evaluation and treatment of clinical conditions associated with the musculoskeletal system, including specialty care. * Performs corticosteroid, hyaluronic acid, and platelet-rich-plasma injections and other clinic-based procedures as indicated. * Call coverage of inpatient rehabilitation unit. * Participates in physician call pool to support our 12 bed inpatient rehabilitation unit. On call responsibilities include rounding on weekends and holidays that are assigned and supporting and supervising residents assigned first and second call priority. Develops teaching plans to include family if appropriate. * Documents appropriately, on the medical record, all audiology treatments, assessments and evaluations performed for the inpatient and outpatient population. * Ensures compliance with policies and procedures regarding departmental operations and hospital fire, safety and infection control. * Demonstrates applied knowledge when evaluating care for selected patient populations including care appropriate to the age of patients served. * Assists with the development of policies and procedures related to the Audiology Department. Develops policies and procedures for review and approval and implements upon approval. * Prepares reports related to audiology activities, including performance improvement activities. * Oversees state mandated Outpatient Newborn Hearing Screening Program and provides consultation to the coordinator for the Inpatient Newborn Hearing Screening program. * Provides performance information regarding the personnel assigned to Audiology Clinic. * Coordinates programs with physicians, nurses, professional and para-professional staff; May develop new audiological programs; may coordinate research programs. * Conducts and/or attends in-service training programs for staff. * Dispenses hearing aids. MINIMUM QUALIFICATIONS Education: Graduation from an accredited college or university program with a master's degree in audiology. Experience: Two years of progressively responsible experience as a licensed audiologist. Licenses/Certificates: 1) Must possess and maintain valid license as an Audiologist issued by the California Speech-Language Pathology and Audiology and Hearing Aid Dispensers' Board; and 2) paneled as a California Children Services' provider OR at time of application, must provide verification that an application for paneling as a California Children Services' provider has been submitted and is pending. Failure to obtain paneling within the probationary period may result in release from probation. Special Requirement: If required by the nature of the assignment, possession of a current and valid California hearing aid dispensing license. KNOWLEDGE Current principles, methods, techniques, equipment and materials used in audiology; possible reactions of and hazards to patients during treatment, and of precautionary and remedial measures; anatomy and physiology related to treatment, psychological and physiological problems of persons with disabling conditions.
