Responsibilities
Area of Responsibilities:
Develops the overall Residential Service Plan with the resident and his or her support network by identifying the individual's needs and desires in an effort to maintain placement and / or progress to more independent housing.
Provide teaching, monitoring and recovery oriented supports in accordance with OMH 595 and 593 including the following rehabilitative services: Assertiveness / Self-Advocacy Training, Community Integration / Resource Development, Daily Living Skills, Health Services, Medication Management / Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, Symptom Management.
Ensure that each resident receives comprehensive planning in a person centered approach for; admission (including the completion of admission notes), on-going recovery oriented services, and discharge. Ensure that each plan is complete and up-to-date in accordance with standards of practice and the New York State Office of Mental Health.
Participates in monthly meeting with Primary Counselor and Resident to monitor recovery efforts, progress and changes needed in plans and ensure delivery of services in accordance with each resident's service plan.
Conducts at minimum quarterly reviews of the Residential Service Plan with the individual in an effort to ascertain the degree of progress and to make revisions when necessary.
Provide guidance and oversight to Counselors to ensure Counselors observe, monitor, document, and report resident behaviors as well as their progress in accordance with agency standards.
Provide guidance and oversight to Counselors to schedule or ensure that annual appointments for physicals, eye exams, and dental appointments are scheduled. Ensures that transportation is arranged for these appointments (when appropriate).
Works collaboratively with the resident's community service providers to ensure continuity of care (care managers, clinical, medical, educational, vocational, and entitlement specialists, etc.). Makes appropriate referrals to these services when needed.
Serves as the key liaison between the resident and his / her clinical provider with regard to the monitoring of the individual's mental health and well-being. Works with the Primary Counselor to keep the resident engaged in clinical services through regular attendance.
Help residents to plan for the purchasing of clothes and other necessities.
Works with residents to encourage a healthy connection with family and community ties and to develop new supports and interests.
Respects and maintains resident confidentiality and demonstrates a caring, positive attitude toward all residents, staff, guests, and service providers.
Provide safe transporting of residents when needed.
Follow all safety rules and regulations for self, residents, and staff.
Performs any other duties necessary for the effective operation of the program.
Qualifications
Education & Experience: QMHS Standard Bachelor's degree in Human Service field
or
an Associate's degree in Human Service field plus three years of FT related experience
or
a High School Diploma plus five years of FT Human Service experience. This position also requires a clean NYS driving record as outlined in DePaul's personnel policy. Must be at least Eighteen (18) years of age.
Work Environment
This position is Sunday-Thursday with the following schedule:
Sunday: 10am-6:30pm
Monday: 10am-6:30pm
Tuesday: 9am-5:30pm

Plan, prepare and implement a monthly activities calendar to be posted in a prominent spot no later than the first day of each month. The first week of every month needs to be posted 7 days prior to the start of the new month.
Review, upon move-in, personal information about each resident's interests and capabilities. Include this information and other data obtained from the resident / family members, on an individualized index card system or the equivalent. This information should be used when developing activities and be updated as needed.
Schedule a minimum of fourteen (14) hours of planned activities a week.
Schedule outings so that each resident has the opportunity to participate in at least one outing every other month.
Plan and implement one-on-one activities with residents.
Enable residents to enjoy solitary activities.
Notify residents of activities and assist and encourage them to attend.
Decorate the facility for holidays and make sure decorations are removed within five (5) days following a holiday.
Order supplies for activities according to budget. Ensure that all expenditures are accounted for from the activity expenses and that all receipts are properly recorded and submitted.
Plan facility programs, parties, potluck and other celebrations with approval of the Administrator.
If desired, develop a volunteer program from within the community. Train and coordinate all volunteer activities.
Attend staff in-service meetings.
Schedule and facilitate a monthly Resident Council meeting, document using meeting minutes, and document a resolution to any questions and/or concerns raised. Review minutes with Administrator and provide copies to residents.
Assist with new staff orientation r/t/the role of activities.
Possess a working knowledge of Resident's Rights.
Evaluate the overall effectiveness of the Activity Program, including input from the residents at least every six (6) months and document finding. Review results with the Administrator and make changes as appropriate.
Ensure that MSDS are available for all substances used in the Activity Department. Keep such materials secure when not in use and supervised.

Heritage Manor of Lockport is hiring a part-time Resident Care Assistant to work the evening shift.
Under the general direction of the Supervisor-In-Charge, the Resident Care Supervisr provides direct personal care and assistance to residents as needed.
The pay range for this opportunity is $16.25 - $16.55 per hour.

Responsibilities
Respects and maintains resident confidentiality and demonstrates a positive, caring attitude toward all residents, staff and guests
Assists residents with bathing, personal care and oral hygiene.
Assists residents with choice of attire and care of clothing.
Reminds residents of meal times and monitors meal attendance..
Makes beds daily and changes bedding and linens weekly or as needed.
Performs rounds on each resident at the beginning and end of each shift.
Documents and reports all concerns to the supervisor; (i.e. changes in behavior, evidence of illness etc.)
Assists relatives, guests and visitors as needed or requested. Will also assist with room preparation and welcome of new residents.
Reminds residents of medication times.
Attends staff meetings and in-services when requested.
Keeps scheduled shifts and plans time off in advance with the supervisor.
Performs housekeeping duties and laundry as scheduled needed or requested.
Inspects building areas and furnishings for wear and defects, completes maintenance requests.
Assists with activity programs as requested.
Performs other duties as requested or assigned.
Qualifications
High School diploma or GED preferred.
CNA, HHA or PCA certification preferred.
One year work experience in a care-giving field desired.
Must be dependable, hardworking and willing to work as part of a team.
Must have the required references and criminal record check.
Must have annual TB screening and Health Assessment completed.
Must be at least 18 years of age.

Responsibilities
Provide teaching, monitoring and recovery oriented supports in accordance with OMH 595 including the following rehabilitative services: Assertiveness / Self-Advocacy Training, Community Integration / Resource Development, Daily Living Skills, Health Services, Medication Management / Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, Symptom Management.
Assists the Medication Coordinator with supervising resident medications in accordance with physician's orders, agency policy and NYS Office of Mental Health regulations.
Maintains accurate and timely documentation in resident medication records.
Communicates relevant information regarding a resident's mental and physical status to the Residential Services Coordinator, the Supervisor on site or directly to the provider when needed.
Completes basic documentation in the resident file with regard to any psychiatric concerns, medical concerns, progress, or anything else deemed as significant.
Ensures that daily tasks and assignments are completed as directed by the Residential Services Coordinator or Supervisor on site. This includes assisting with the preparation of resident's rooms prior to admission to the program, helping with the orientation of new residents moving into the program and assisting with the discharge and transfer of residents when needed.
Completes rounds at least 4 times per shift in an effort to monitor the program and intervene when necessary to settle any potential problems or disputes between residents; this includes updates on the daily census check if needed. Works with Overnight security to ensure the safety and well-being of the resident (i.e., observation of the building and grounds, enforcing visitor's hours, ensuring the doors are locked at 9:00pm each night, restricting unwanted visitors and monitoring of the security camera's etc.).
Shares information with supervisor or other administrative staff in regard to problems or concerns that occur during their shift.
Works with and helps residents maintain the upkeep of their personal rooms.
Coordinates and participates in resident's recreational activities.
Respects and maintains resident confidentiality and demonstrates a caring, positive attitude toward all residents (as per respect policy), staff, family members, guests and service providers.
Attend staff meetings and trainings as required by the agency.
Work as a member of the residential team for each resident and give relevant input for treatment team meetings and Continued Stayed Reviews.
Provides safe transporting of residents when needed.
Performs any other duties necessary for the effective operation of the program.
Qualifications
CLS 1: Minimum of High School diploma or GED and ability to demonstrate verbal and written skills necessary to carry out services for the success of the program.
At least one-year of experience in a residential setting working with people in various stages of recovery from mental illness preferred.
Valid New York State driver's license that meets qualifications set by agency's clean driver record policy is preferred.
Must be at least Eighteen (18) years of age.
CLS 2: The same requirements as above, however additionally, valid New York State driver's license that meets qualifications set by agency's clean driver record policy is required.
CLS 1 pay rate: $20-20.30/hr
CLS 2 pay rate: $20.25-20.55/hr
