GENERAL JOB FUNCTION The Family Resource Coordinator works with donor hospital personnel, physicians, and the Donor Network West team to facilitate a donation discussion and the organ and tissue authorization process. The primary responsibility is to initiate the donation discussion and request for organ and tissue donation or notify the Notifying Party of donor designation. The Family Resource Coordinate also immediately responds to donor referrals to assess family needs as requested by the hospital, family and or resource/transplant coordinator. He/she must utilize consistent communication and empathy for both the donor family and potential transplant recipients. The individual must be compassionate, conscientious, and ethical, and must possess strong interpersonal skills which will allow her/him to deal effectively with conflict resolution, donor families in grief/crisis, individuals or groups and a variety of medical professionals. This position requires strong conflict management skills as well as written and verbal communication skill; and an ability to work productively both independently, and as a team participant with accountability to all other team members for meeting agreed upon objectives. JOB DUTIES AND RESPONSIBILITIES Works with Donor Network West staff and hospital staff to ensure that the Authorizing Party is identified per Uniform Anatomical Gift Act guidelines. Determine family dynamics and assess the family's understanding of the patient's prognosis, when appropriate initiate the donation discussion. Initiates the donation discussion and authorization process for potential organ and tissue donor families prior to, during and after death declaration. Provides families with the detailed information required to give legal informed authorization for anatomical donation. Responds on site independently and/or in conjunction with assigned staff to all appropriate hospital referrals within designated time outlined per policy and procedure. Initiates frequent huddles with hospital and Donor Network West staff to ensure that the Authorizing Party/Notifying Party is presented the opportunity for donation or donor designation notification in the most appropriate time frame and manner. Access donor registries to determine prior authorization for donation. Communicates with the attending physician and other members of the healthcare team to establish rapport and ensure a collaborative planned approach for the donation discussion and authorization process. Will communicate directly with donor families, hospital personnel, physicians and DNW staff. Obtains authorization for donation per UAGA and verifies appropriate medical and legal documentation necessary for organ recovery consistent with DNW policy and procedure. Completes the Uniform Donor Risk Assessment Interview (UDRAI) with appropriate historian per DNW policy and procedure and obtains follow-up information if required. Visually assesses donors, interpret charts, document information and communicate findings. Collaborates with hospital and medical staff to provide potential donor families with accurate and timely information regarding the patient's current clinical course. Maintains communication with hospital staff and attending physician regarding the potential donor family's understanding of the prognosis and acts as a family advocate to the health care team as necessary. Provides education and relevant materials to hospital staff on the authorization and family process as well as the benefits of organ and tissue donation and services provided by Donor Network West to grieving families. Provides excellent customer service to internal and external partners. Collaborates with hospital staff to provide bereavement support as applicable. Collaborates with hospital staff to obtain administrative authorization when appropriate. Provides education to hospital staff regarding authorization, family care process and donation process. Responsibilities may be affected by increased donor activity. Performs other duties as assigned. QUALIFICATIONS Possess knowledge of medical and legal principles of authorization, donor evaluation, and management. Has ability to communicate and present information effectively and concisely within a team environment. Has ability to work productively both independently, and as a team participant with accountability to all other team members. Proactive team player who must uphold the core values of the organization which include passion, integrity, and team work. Has strong conflict management skills as well as excellent written and verbal communication skills. Multi-lingual skills are beneficial. The most common languages spoken are Spanish, Mandarin, Cantonese, and Vietnamese. Compassionate, conscientious, and ethical, and must possess strong interpersonal skills which will allow her/him to deal effectively with conflict resolution, donor families in crisis, individuals or groups and a variety of medical professionals. Maintain strong organizational skills that will allow him/her to assess changing environments establish appropriate priorities, handle highly stressful situations in a calm, mature manner, using sufficient judgment and discretion at all times. This individual must also provide accurate assessments of job performance and recommend changes in policies, procedures, strategies and action plans. Thrives in a fast-paced dynamic environment and adjusts to new priorities as required. EDUCATION AND EXPERIENCE A degree in biologic sciences or social sciences or an equivalent degree/diploma/certification as a Counselor or allied health field or one year equivalent OPO experience. At least two years' of experience in working with families, counseling, bereavement and/or crisis intervention preferred. Donor Network West takes a market-based approach to pay. JOB SUMMARY The Resource Triage Coordinator (RTC) is integral to providing leadership and continuity of service to all donor families and hospital partners by collaborating with the operations center that receives referrals, screens potential organ donors with referral facilities, approaches families to offer the option of donation, and coordinates all logistics involved in the donation process. This individual is responsible for enhancing the mission of DNWest by responding to, and triaging organ referrals so the appropriate DNWest staff can be dispatched to facilitate the donation process. Typical duties include immediate response to organ donor referrals, evaluation by phone of the referral, engagement of the attending MD and hospital team for the implementation of guidelines for care/catastrophic head injury orders, support and evaluation of family needs, identification of resources needed for the referral and triaging the referral to the appropriate DNWest staff member. In conjunction with the DNWest Medical Director, determines medical suitability of potential organ donors and utilizes appropriate resources to achieve this. The RTC may also be assigned duties as determined by management, developing, implementing and monitoring policies, procedures strategies, and action plans related to the procurement and utilization of organs for transplantation, medical research, and education.Call Day Clinical Activities/Organ Donor Evaluation and Management Provides consistent and timely service to hospitals, donor families, and other transplant personnel in all potential donor situations. The RTC must also possess knowledge of medical and legal principles of organ donor evaluation, authorization, and donor management. This is necessary in order to effectively communicate with physicians and hospital staff involved with a potential organ donor. Assists staff in identifying issues related to the donation process and in collaboration creates a plan of action. Collaborates with staff onsite to determine resources needed for every ongoing organ referral. Provides initial risk management assessment. Activates the occurrence reporting system and follows up as necessary. The RTC may implement the occurrence alone or in conjunction with managers/supervisors. Responsible for triage of new organ referrals, obtains clinical information to determine medical suitability, provides assessment of hemodynamic stability and initiates communication with attending physician and or hospital staff to maintain stability and potential organ viability. Identifies need for additional information to assist with determining suitability. Assists with determining DCD suitability. Will provide accurate communication of referral status to Clinical Procurement Coordinator (CPC) and other transplant professionals. Determines acuity level and ensures DNWest response accordingly. Works with Clinical Operations Manager, Regional management / family management to ensure resources to respond are identified. Creates case plan for organ referrals including further needs once onsite staff involved. Is responsible for the effective management of the referral until authorized or referral is closed. Escalates challenging referrals with case dynamics as appropriate. Ability to respond to a referral and or donor with an onsite presence in times of high activity Moderates AM/PM huddle and reports on all high acuity referrals with accurate information and case plan. Ensures huddles are called on referral processes as determined necessary, reviews to ensure documentation of case planning occurs. Provides Donation Program Consultant (DPC) information for debriefing on-site experiences. Is able to direct the drawing of blood and labeling of specimens for infectious disease testing to facilitate organ placement, when required by phone. Maintains concise, accurate records of data regarding donor referrals and recoveries. Maintains accurate information on all laboratory values, medications and diagnostic tests required to evaluate and manage all potential and/or actual donors. This also includes recording problems/incidents and reporting them in the established manner. Consults with other Transplant Network staff and/or DNWest Medical Director, as needed, on issues pertaining to the clinical and technical aspects of quality organ donor evaluation. Participates in post-donor and or post referral review with team members and appropriate management staff. Participates in appropriate follow-up with nurses, physicians, and other donor hospital staff, transplant center staff, and other organ procurement organizations (OPOs) per policy, as requested. May precept new DNWest staff, in conjunction with primary preceptor, in orientation to the process of response to referral. Manages "Backboard” cases: makes calls to determine case plan and moves referrals between I-transplant and Backboard as needed. As needed, helps with transportation arrangements for staff responding to referral and donor activity. Approves and assures judicious use of overtime. Takes sick calls and records them in the reporting system. Authorization Situations/Family Care Will access donor registries on all referrals 80 years old and younger. If First Person Authorization is identified, the RTC will attach the Document of Gift to the chart and ensure the CPC and hospital is aware of it. Evaluates potential donor family dynamics, in phone discussion with bedside staff and discloses information to DNWest's clinical leadership and/or FRC to assist with the distribution of resources to a case. May interact with families by phone when resources are limited, or the situation dictates. Contacts appropriate DNWest's resources in the event of potential media attention. Other Responsibilities Assist the DPC in the development and implementation of response to referral protocols specific for donor hospitals. This may include presentations and or in-services at critical care courses, orientation etc. Completes categorization of outcome of cases in ITX for eligibility criteria as outlined by Centers for Medicare and Medicaid Services (CMS). Categorizes referral outcomes for potential criteria as outlined in protocol. Reviews feedback on donors/referrals and the donor/referral management process to improve quality, as warranted. Participates in conference calls to review donation/referral activity and attends staff meetings and workshops as mandated. Participates in chart reviews and shares findings with management. Performs spot audits to asses for compliance after implementation of change or education. Works with Donation Services team (s) and Medical Director to review cases and activity to identify opportunities to improve outcomes. Professional/Personal Competency Maintains state of the art knowledge of end stage organ disease, organ recovery, donor maintenance, transplantation and organ preservation through appropriate professional organizations. Maintains critical care knowledge to ensure ability to function independently in donor hospitals, either internally or through external avenues. QUALIFICATIONS Compassionate, conscientious, ethical, and must possess strong interpersonal skills that will allow her/him to deal effectively with individuals or group of individuals, particularly a variety of medical professionals as well as donor families. Strong written and verbal communication skills and an ability to work productively both independently, and as a team participant with accountability to all other team members for meeting agreed upon objectives. Maintain strong organizational skills that will allow him/her to assess changing environments; be able to establish appropriate priorities; handle highly stressful situations in a calm, mature manner, using sufficient judgment and discretion at all times. Qualified to perform donor evaluations and donor management without supervision. EDUCATION AND EXPERIENCE Bachelor's degree in biologic sciences or an equivalent degree/diploma/certification in an allied health field, i.e., Emergency Medical Technician/ Paramedics (EMT/P) or Respiratory Therapist. CA registered nurse license or certification as a Physician Assistant, or allied health field is preferred. DNWest requires the RTC to maintain a valid CA driver's license and current vehicle insurance based on CA minimum insurance coverage standards. Functions as an expert clinical resource for the hospital regarding organ and tissue donation. Participates in decision-making programs and committees in the hospital that have an influence on organ donation, promoting donation and seeking opportunities for further collaboration. Facilitates the donation process. Maintains high visibility by conducting rounds to all critical care units with medical teams and functioning as the designated resource within the hospital for all issues related to organ donation. Provides donor and referral follow-up to appropriate hospital staff one-on-one or by post-recovery conferences. In partnership with the In-House Donation Specialist (when applicable), develops, provides and evaluates in-house training and in-service educational programs on all aspects of the donation process for hospital staff. Also, collects and reports donation outcome data to various levels of clinical and administrative hospital staff. Documents hospital activities, updates hospital plans, goals, and critical issues in a timely manner on a regular basis. Performs other duties as assigned. Must have reliable personal automobile transportation to be used with company reimbursement using IRS guidelines. Employee may be required to drive personal vehicle while functioning in the role. Must be able to travel within the service area by ground or air. Ability to work minimum of a 12-hour shift, with the requirement of staying past shift when required by donor volume and staffing, with few breaks. The department schedule may change over time to meet staffing needs for tissue recovery. The schedule includes 7, 12-hr shifts per pay period. The employee is required to work weekends (defined as Friday, Saturday, and Sunday), nights and holidays. Holiday requirement includes work on 5 holiday shifts per calendar year. Ability to handle confidentiality of all donor/recipient information. Maintains recovery performance metrics that are consistent with department goals. EDUCATION AND EXPERIENCE Prove performance efficiency and excellence as a DNWest Tissue Recovery Coordinator I - Trainee Associate degree in biological sciences/allied healthcare/ mortuary science, or completion of a surgical technology program, or an emergency medical services program 2 years of work experience in a related field may be substituted for educational requirement. Must maintain a valid California or Nevada driver's license and current vehicle insurance based on applicable state minimum insurance coverage standards. GENERAL JOB FUNCTIONThe Tissue Recovery Coordinator responds to tissue recovery cases and provides technical support for all aspects of tissue recovery for transplant and research. The Tissue Recovery Coordinator is responsible for the intraoperative leadership of the recovery team and communication to a team of other medical professional, including hospital personnel, funeral home staff and coroner or medical examiner professionals.
