Essential Job Responsibilities:
* Provides nursing care to residents/patients within the scope of practice of a Licensed Practical Nurse.
* Responsible for coordination and direction of resident/patient care. Coordinates care and services with other departments/providers from the time of admission to discharge.
* Assists Registered Nurse with completion of assessments, documentation, and data collection within scope of practice and acting timely on findings.
* Administers medications and completes treatments as ordered. Observes and monitors resident's condition and reports changes as appropriate. Ensures that all physicians' orders are signed and implemented within specified time frames.
* Maintains safety of all residents/patients and team members. Responds appropriately to safety hazards and reports any issues to the appropriate person. Ensures that all staff follows the infection control policy and procedure.
* Maintains a clean, sanitary, safe and efficiently organized work area.
* Performs other duties as assigned.
Minimum Required Qualifications:
* Degree from an accredited school of nursing for the Licensed Practical Nurse program
* Current LPN licensure in the state in which they are working

Job Description
The Licensed Practical Nurse (LPN) is responsible for a variety of professional nursing and resident assistant duties in the care of skilled nursing residents, complies with state and federal rules and regulations, and follows established policies and procedures of the facility. Specific responsibilities include:
Assist with new admissions, hospital transfers and discharges as requested by Clinical Director
Train, plan, assign and direct work of resident assistants within established guidelines and assist Resident Assistants as necessary. Assist in the monitoring and evaluation of job performance for resident assistants.
Order and administer medications.

* Completes initial assessment of potential residents. Implements service agreements with all residents upon move-in.
* Monitors and maintains each resident's medical record, ensures appropriate assessments are completed timely and documentation is appropriate, and confirms residents have current care plans in place.
* Ensures all physician orders are signed and implemented within 24 hours.
* Maintains and monitors medication administration program. Supervises medication practices and medical equipment being used in the building. Follows up with physicians regarding medications and/or medical conditions.
* Assists supervisor in providing health and medication training to all new team members as well as continued education for all staff and documents such training to assure compliance with state, federal and local regulations.
* Performs other duties as assigned.
Minimum Required Qualifications:
* Associate's Degree in Nursing
* Must be licensed as a Registered Nurse within the state in which working and must maintain licensure

Job Description
The home health Registered Nurse is responsible for providing direct home health nursing care for a team of clients in accordance withnursing practice, Medicare regulations, agency standards and policies.
Specific responsibilities include:
Completes and documents skilled nursing assessments for patients following physician orders.
Collaborates with physicians, other health care professionals, clients and families in developing a comprehensive coordinated plan of care.
Develops an individualized plan of care which includes objectives goals for client improvement in health and functional status. Evaluates plan of care on a continuous basis, updates physician(s) and other therapies of changes in client status and need for revision of plan of care.
Delivers skilled nursing services efficiently and effectively to meet client care needs.
Ensures that physician orders are communicated to the agency's staff with accuracy, thoroughness and understanding.
Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, plans for continued care, new goals, etc.
Plans for discharge from the agency with the client and caregivers. Makes referrals for alternative level of care if necessary.
Follows schedule frequency as ordered by physician.
Proactively participates in care conferences with other health team members as needed.
Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines.
Promotes personal safety and a safe environment for clients and co-workers.
Other duties as assigned.
Qualifications
The following are the minimum required qualifications for the home health RN position:
Graduate from an accredited school of nursing as a Registered Nurse (RN).
Maintains a current, unencumbered Registered Nurse license in the State of Minnesota.
One (1) year experience as a Registered Nurse in home health.
Knowledge of Medicare home health regulations and OASIS data-sets.
Demonstrated leadership and judgement with strong knowledge in nursing practices.
Must understand and communicate with specialized medical terminology and explain procedures to clients.
Valid driver's license, current auto insurance and reliable vehicle.
Ability to communicate effectively, including written and verbal skills.
Ability to work work well both individually and as part of a team, through respect, collaboration and dedication; successfully interact with a diverse culture base.
Ability to prioritize and organize work effectively and efficiently.
