The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery.
We are seeking a qualified healthcare professional to be our Clinical Counselor Outpatient Services who will counsel and aid individuals who have substance use disorders and/or their families/loved ones in a New York State OASAS-certified outpatient treatment program. Utilizing a person-centered and trauma-informed lens, individualized assessment and counseling (telehealth or virtual) services are delivered both in-person and virtually at Project Contact Outpatient at the Center for Recovery and Wellness.
Schedule: (3) Weekdays - 10am to 6pm, (2) Weekdays - 12pm to 8pm, and a rotating weekend once a month.
Responsibilities
Provide caseloads with weekly individual case management and counseling sessions consistent with the individualized treatment plan for individuals in the Substance Use Treatment and/or Significant Other Services treatment tracks.
Continuously assess the appropriateness of the current level of treatment, corresponding with LOCADTR and utilization review instruments (Substance Use treatment track).
Coordinate integrated services with medical, mental health, vocational, recovery services, and other social service providers.
Complete detailed progress notes, comprehensive treatment plans, intake/admission assessments, psychosocial assessments, discharge plans, and other required documentation in accordance with OASAS and CARF standards.
Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS, managed care, and CARF standards.
Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
Specimen collection for clients and monitors for treatment-related issues in order to update the treatment plan (Substance Use treatment track)
Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the client (Substance Use treatment track)
Coordinate integrated services with medical, mental health, housing, vocational, recovery services, and other social service providers.
Facilitation of individual and group-level interventions during other clinician vacations or leave
Ensure that clients' annual medical examinations are completed, with associated referrals as needed.
Contact referent and/or family contacts upon admission of new clients in a timely manner, and provide written or oral clinical updates as needed.
Effective advocacy and collateral contacts to empower clients
Outreach to referral sources to expand the network of clients.
Attend staff meetings and trainings as required, as well as individual and group supervision.
Perform special projects and other duties as assigned.
Qualifications
Master's Degree in a mental health discipline (MSW, MHC, MFT, etc), and either licensed in the state of New York, or working under a limited permit and eligible for licensure. OR CASAC credentialing (Credentialed Alcoholism and Substance Abuse Counselor) OR CASAC-T with a bachelor's degree in a human services-related field and a minimum of 2 years' experience working in a substance use program.
Possess a demonstrated ability to deliver patient treatment, including communication skills, successful engagement in specialized human services training, understanding of human interaction and personality development, and the ability to write and record case documentation.
Familiar with Regulatory Agency/OASAS Regulations regarding the operation and practices of patient care.
An understanding of group dynamics is essential.
Must be computer-literate with knowledge of Microsoft Office applications.
Competent in electronic health records that support treatment plans and case notes such as AURA Relias, Netsmart, and AWARDS (willing to learn new systems).
Excellent verbal and written communication skills.
Self-motivated with the ability to multitask and handle challenging situations in a professional and courteous manner
Ability to work professionally and empathetically with a diverse group of clients and staff.
Bilingual in any language preferred

JOB SUMMARY:
The Family Support Specialist II is responsible for providing comprehensive case management, referrals, and seamless coordination with community and supportive service activities that promote economic self-sufficiency for families, enhance the quality of life for seniors and people with disabilities, and increase opportunities for residents. This role involves collaborating with a team of professionals in various fields to determine suitable approaches and support families in different neighborhoods under the U.S. Department of Housing and Urban Development.
JOB RESPONSIBLTIES:
Conduct intake, assessments, enroll residents in family support program, and visit various locations for resident information gathering.
Collaborate with families on Development Plans, assist in accessing social services, advocate for service delivery for up to 100 individuals.
Ensure privacy, track resident data in USI's "LEARN" system, attend community and program-related meetings.
Available for occasional nights/weekends, travel up to 25%, perform administrative tasks.
Providing resources and knowledge for clients to access capital, including loans for financial goals like housing and consumer products, to build credit, reduce debt, and support wealth growth through entrepreneurship.
Offering consultation services in financial management and goal-setting.
Hosting in-person and virtual seminars for clients seeking financial literacy management resources and capital access.
Supporting and promoting the development of the USI CDFI client base.
QUALIFICATIONS:
Bachelor's degree in Social Work, Sociology, Psychology, or related field.
3+ years of experience as a case manager or in direct practice with low-income households; health services experience a plus.
Valid driver's license and automobile availability for work use.
Commitment to strengths-based, family-focused service planning and understanding of program philosophies.
Ability to handle confidential information professionally.
Independent and proactive work ethic, with strong team collaboration skills.
Availability for evening and weekend functions as required.
Strong verbal and written communication skills for diverse communities.

POSITION OVERVIEW Performs all activities in the implementation of ERSEA (Enrollment, Recruitment, Selection, Eligibility, and Attendance) requirements ensuring responsiveness to the results of the Community Assessment and all applicable Head Start Performance Standards. Collects, inputs, and analyses data into the child enrollment, health, and attendance systems. Assists in the development of policies and procedures for ERSEA and for prohibition suspension and expulsion. This position will also assist parents through a referral system to community agencies in cooperation with all other disciplines (Child Development/Disabilities, Health, Mental Health, Nutrition, and Administration). Assumes a key role in compliance with Head Start Performance Standards. ESSENTIAL FUNCTIONS · Engage in comprehensive, integrated planning with the management team: o to create ERSEA goals/outcomes and a system of services that support the eligibility, recruitment, selection, enrollment, and attendance of infants, toddlers and that is responsive to the results of the community assessment. o to create a family and community engagement goals/outcomes and a system of services that aligns with the Head Start Parent Family Community Engagement Framework. o to create a system of services that limits child suspension from the program and prohibits child expulsions from the program. o to ensure the implementation of ERSEA and Family/Community Engagement throughout the program. o to support ERSEA/Family Engagement Manager develop policies and procedures for the ERSEA and Family/Community Engagement system of services. · Assist in the assessment of community, program, child and family needs, strengths, and resources in quality-of-life issues; compile and analyze data; produce reports; assure the integration of dual language learners, homeless children, children in foster care, and children with disabilities. · Under the direction of the ERSEA/Family Engagement Manager, assist in tracking, reporting, each child's attendance and follows through with individual intervention in situations of chronic absenteeism. Assist with the development of the Community Assessment. Assist with the annual program self-assessment and federal onsite review process, focused on compliance with ERSEA and progress on family/community engagement; report on strengths, recommendations, required improvements; implement improvements and recommendations; incorporate into the program planning process. POSITION SPECIFIC DUTIES AND RESPONSIBILITIES: ERSEA • Implement the system to track the eligibility, recruitment, selection, enrollment, and attendance of children to comply with federal and state program regulations. • Assist in creating the criteria for the selection and enrollment of children assuring responsiveness to community assessment and reflecting data on dual language learners, homeless children, children in foster care, and children with disabilities. • Perform intake/registration procedures for all families and ensures income verification and eligibility qualifications of children and families are met, and ensure applications are complete and data is accurately entered in the Child Plus database. • Assure and maintain record keeping and reporting policies for waitlists, enrollment, attendance, timelines, schedules, and procedures in accordance with following designated state and federal program requirements. • Conduct ongoing monitoring, tracking, follow-up, and analysis of enrollment and attendance data, and produce regular reports for management meetings. • Assist in addressing any possible child suspensions and in the prohibition of expulsion and collaboration with Mental Health Services. • Maintain enrollment forms, ensuring information is current, correct, and disseminated to all necessary staff to meet program requirements. • Perform recruitment activities in response to enrollment data and waitlist data. • Ensure full program enrollment and maintenance of a sufficient waitlist. • Compile and submit Program Information Report (PIR) data periodically. Family Service/Community Partnerships • Implement the system for Family and Community Engagement Services. • Implement a system of services, record keeping, and reporting of the Family Partnership Agreement and assure its alignment to the Parent Family Community Engagement Framework. • Assist in being the program resource for consultation, information, and referral regarding community resources to staff and client families • Completes family service home visits and Needs Assessments • Completes other related activities and duties as assigned. Health and Disabilities • Support families obtain their infant/toddler's health requirements, such as vaccines, annual health checkups, dental/oral health exams, etc. • Follow up with parents and doctors on infants' and toddlers' health requirements, and ongoing health care, including access to a medical and dental home. • Responsible for tracking and documenting health events in the program's data management system. • Conducts Health home visits, screenings, and assessments with families. • Utilize the Arizona State Immunization System (ASIIS) database to capture children's immunization data. • Monitor children's health reports and works collaboratively with the Health Services Team. • Support the referral process for children with possible special needs and works collaboratively with the Education/Disabilities Team, teachers, and parents. PHYSICAL RESPONSIBILITIES: The ability to see at a normal distance, hear normal conversations and sounds, and use hands and fingers to fill out the required forms. Occasional lifting of fifty (50) pounds is required and must be able to push/pull up to fifty pounds horizontally. Ability to sit on the floor, bend at the waist, kneel and/or stoop 75% of the time. PREFERRED QUALIFICATIONS • Education: Associate or bachelor's degree in social work, family services, human services, or -in a related field. • Experience: Work in a Head Start of Early Head Start program and knowledge of the Office of Head Start Performance Standards. • Skills: Bilingual (English /Spanish). JOB PURPOSE
Counsel and aid individuals who have substance use disorders in Pride Site, a New York State OASAS-certified residential treatment program-one of four programs in the newly constructed Center for Recovery and Wellness (CRW).
SCOPE OF INFLUENCE
Provide individual case management and group counseling to adults with substance use diagnoses (and their families/significant others).
Partner with referral sources, providing regular updates and progress on clients.
Partner with a multi-disciplinary team in assessing diagnoses, planning treatment, providing coverage of clients' individual and group sessions, and offering clinical insight and expertise.
KEY RESPONSIBILITIES AND ACCOUNTABILITIES
Clinical and Documentation (80%)
Provide up to 20 clients with biweekly individual case management sessions to continuously assess appropriateness of current level of treatment.
Coordinate services with medical, mental health, and other social service providers; communicate with the on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility.
Arrange and participate in multidisciplinary interventions for clients on caseload who may be struggling at the current level of care and follow up by assessing progress incrementally.
Provide evidence-based group counseling to clients in the residential program.
Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards.
Maintain accurately and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards.
Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the resident.
Ensure that clients' annual medical examinations and psychiatric evaluations are completed, completing referrals as needed.
Contact the mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed.
Make collateral contacts for clients.
Other (20%)
Outreach to referral sources to expand the network of clients.
Attend staff meetings and training as required, as well as individual and group supervision.
Perform special projects and other duties as assigned.
JOB REQUIREMENTS

LP-MHC or LMSW.
