    RESPONSIBLE FOR: The discharge coordinator is responsible for supporting the transplant team by ensuring a safe and smooth transition for kidney and liver transplant recipients from hospital discharge to home. This role involves direct patient education, medication review, and instruction, along with coordination across multidisciplinary teams to optimize education for post-transplant success. Responsibilities: Schedule and hours: Tuesday- Friday 10 hours KEY RESPONSIBILITIES: Educate kidney and liver transplant recipients on post-transplant care, including lifestyle changes, follow-up requirements, and warning signs. Provide comprehensive pillbox education for kidney and liver recipients to ensure medication adherence, compliance, and understanding. Update and maintain accurate medication plans in collaboration with pharmacists and providers. Coordinate discharge planning for kidney and liver transplant patients, ensuring all medical, logistical, pharmaceutical, and educational needs are met. Keep Epic-based transplant checklists up to date for new kidney transplants ensuring compliance with institutional protocols and standards. Communicate effectively with patients, families, and the care team to facilitate a successful discharge. Complete liver pathology documentation Complete and maintain epic documentation for delayed graft function and dialysis Qualifications: MINIMUM EDUCATION REQUIRED: Graduate from an accredited school of nursing MINIMUM EXPERIENCE REQUIRED: Two years of nursing experience required. MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Registered Nurse licensure in the State of Georgia. ADDITIONAL QUALIFICATIONS: Two years of critical care or procurement experience preferred.
