    *Full Time Surgical Physician Assistant Opening in Danbury, Connecticut (60 miles from New York City)* Danbury Hospital, now part of Nuvance Health, is seeking a Full Time Surgical Physician Assistant to join our established and growing Surgical Advanced Practitioner Services program in Danbury, Connecticut, located about 60 miles northeast of New York City and near the border of Westchester and Putnam Counties in New York State. Our busy service of 10 full-time Advanced Practitioners currently provides services to all surgical patients admitted to the hospital, as well as in Observation, Extended stay, Emergency room, and Operating room, including consults as well as pre and post-operative care. Specialties include, but are not limited to, Urology, Orthopedics, Neurosurgical/Spine, Plastics, and ENT. As we grow the program, we will be evolving to a 24/7 service that includes some OR assist The ideal candidate will be a board certified physician assistant with previous surgical and/or patient management experience and/or surgical residency training and/or Da Vinci robotics training, though new graduates will be considered. This is an employed position with Nuvance Health Medical Practice. We offer a competitive compensation package, including allowances for CMEs and Dues expenses. Danbury Hospital has 371 beds and is a Level II Trauma Center with approximately 70,000 ED visits per year. Summary This is a full time counselor position that reports to the Director of Behavioral Health. The Mental Health Counselor is responsible for providing quality and appropriate clinical services to patients with mental health disorders. The Mental Health Counselor is also responsible for coordinating mental health services in collaboration with the School Based Program of Franklin Primary Health. Responsibilities * Responsible for completing Mental Health a comprehensive psychosocial evaluation including an assessments and screenings. Performs other duties as assigned by the Director of Behavioral Health. Provides individual, group, family and play therapy mode, utilizing basic therapeutic models and theories according to consumer needs as stated in the treatment plan * Utilizes assessment data to support a DSM V diagnosis * Formulates appropriate treatment plan along with consumer and family/guardian, as appropriate, ensures that treatment goals are specific and measurable within the treatment plan * Formulates and oversees implementation of appropriate behavior management plans * Evaluates the client for discharge planning and arranges such services, as needed * Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation * Effectively collaborates with the treatment team members and outside sources in the delivery of clinical services * Participates in treatment team meetings and case staffings * Assures that certification/program standards, policies and objectives are met * Refers patients to other social services for financial assistance, legal services, health care services and other required services. * Establishes and maintains clinical charts, referrals, and other related documents for the treatment of assigned patients; prepares related documentation; keeps abreast of patient data to ensure appropriate treatment and care is delivered. * Works in collaboration with other program staff, and local community based organizations, to coordinate services in order to avoid duplication of services. * Assists in preparing monthly, quarterly and final reports for funding sources and administration. * Maintains current license and requirements for renewals and Attends professional meetings, educational conferences, and in-service trainings in order to maintain and improve professional competence. * Documents in a clear, concise and timely manner, types of client problems, treatment goals addressed, services, and techniques used * Effectively manages caseload based on client needs * Seeks clinical supervision and consultation as needed * Accepts and employs suggestions for improvement * Actively works to enhance clinical skills * Participates in community/public events sponsored by FPHC. * Administers and observes the organizations office operations, policies and procedures. Bilingual in English and Spanish is preferred, but not required. Ability to effectively present information in one-on-one and small group situations to patients, internal providers and staff and other agency staff working in cooperation with the organization. Physical Requirements: Ability to occasionally exert enough force to move objects weighing up to 10 pounds. Ability to continuously remain in a stationary position.
