Job Summary:
The Scribe will accompany clinicians during patient visits and document dictated history of present illness, review of symptoms, examinations, and patient history in an electronic medical record system. Medical scribes work closely with clinicians as a part of the team-based patient centered care that Finger Lakes Community Health provides, but are not patient care providers.

Duties/Responsibilities:
Annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system.
Clearly and accurately recording each patient's symptom's, history, physical exam and documenting the diagnosis
Able to manage laboratory and imaging orders as dictated by the provider
Maintains appropriate behavior in the examination room and does not improperly interact with patients.
Completes chart documentation in a timely manner.
Able to find and summarize past medical visits from external progress notes.
Uses correct medical terminology and makes minimal grammatical charting errors.

Responsibility Summary

Essential Functions
* Performs vital signs and activities of daily living as outlined on plan of care inclusive of ADL's. Assists on hourly rounding and documents care rendered per guidelines.
* Meets nutrition needs of patients as outlined in the plan of care, and in accordance with policies and procedures. Performs meal delivery, sets up trays and adaptive equipment; assists with feeding, and distributes nourishments while honoring choices.
* Properly documents per policy, protocol, and procedure.
* Maintains patient comfort and safety as outlined in the plan of care and in accordance with policies and procedures. Turns and positions patients, provides skin care, immediately answers call lights/alarms, places mats, positions low beds, etc.
* Assures rehabilitation needs are met as outlined in the care plan and in accordance with policy and procedures, including ROM, ambulation, transfers, proper body alignment, and use of assistive devices. Documents rehab per guidelines.
* Appropriately participates in shift changeover communication providing complete and relevant information about patients for which they provided cared.
Experience
Minimum 1-2 years customer service and/or health related experience.

Under the direction of supervisory nursing staff, provides daily care for residents to whom assigned. Assists in development of nursing care plans by participating in care conferences. The individual may be a trainee for no more than a maximum of 120 days from their date of hire. During classroom training time, the trainee is directly supervised by the Program Coordinator, Program Primary Instructor, and possibly additional trainers when utilized. During supervised clinical training time on resident unites, the trainee is directly supervised by the Program Coordinator, Program Primary Instructor, and possibly additional Nurse Aide Trainee Clinical Supervisors when utilized.
