POSITION OVERVIEW: The YouthAdvocate is responsible for case management, psycho-social assessment, development and implementation of intervention strategies. The YouthAdvocate provides clinical leadership to a multi-disciplinary team and provide coaching to support each youth to achieve goals of housing stability, sustainable and sustaining employment, and educational achievement. The YouthAdvocate is active in developing, supporting and implementing an individualized, strength ‘based, culturally competent, youth-centered approach. A Successful YouthAdvocate understands how to motivate, influence, and mentor young adults to achieve goals while launching adult lives from foster care. This position is a combination of onsite and community-based work Monday through Friday.
DUTIES + RESPONSIBILITIES
Provide youth with individual counseling, crisis intervention, referrals, advocacy and life skills training. (70%)
Direct Care and Interventions:
In-residence visits are mandatory as outlined in the program guidelines.
Develops partnership strategy with each youth
Gathers and analyzes on-going information to ensure youth safety, well-being and progress
Provides clinical leadership to multi-disciplinary teams consisting of youth, family and other natural supporters, county case managers, Employment and Education Specialists, Housing Specialists, and Intake Specialists to ensure seamless and effective service delivery
Organizes, schedules, and facilitates youth and team meetings
Provides on-going assessment of youth well-being and develops plans to ensure safety and Deescalate crisis as needed
Provides direct support, coaching, mentoring and intervention to increase youth self-sufficiency and goal attainment.
Participates on a rotating basis to provide on-call coverage
Provides crisis counseling as needed. Case Management/Coordination:
Collaborates with community providers including county workers, courts, professionals, vocational and independent living skills programs, schools, family members and significant others
Keeps key team members informed and Works with team members to identify and engage appropriate natural supporters. Documentation/Administrative Duties (30%)
Completes documentation including but not limited to Care and Services plans, needs assessments, and progress notes.
Ensures all needed paperwork and consents are completed for each client at time of entry.
Participates in agency committee as assigned.
Provides on-going orientation and training to staff as assigned.

QUALIFICATIONS:
BA in social work, psychology, counselling or equivalent required with Minimum four years of experience working in a child welfare social service setting or with transition age youth with complex presentations including chronic trauma.
Master's Degree in a social work, psychology, counselling or equivalent preferred.
A strong dedication to cultural competency that creates conditions that remove barriers and increase a sense of belonging.
Professional License is a plus.
Must be comfortable working in community settings.
Willingness to work collaboratively, but with the capacity to work independently
Must be able to pass a Community Care Licensing background check
Must have a car, a valid CA driver's license and auto insurance.
Computer skills -- Proficiency with Microsoft Office applications including MS Word, Excel, Access, and PowerPoint, Project and Outlook. Experience with database systems
Availability - for occasional evening and weekend work and some travel as needed.

POSITION SUMMARY: The TAY Case Manager provides intensive onsite and community-based case management for individuals whom faced homelessness as Transition Age Youth (TAY) and now live at our affordable housing locations. Through comprehensive case management, the TAY Case Manager helps clients to identify and attain goals in the areas of self-sufficiency, independent living, health and wellness, relationships, employment and education, and personal advocacy. The TAY Case Manager also provides on-site, drop in resource and referral support for residents as needed and coordinates community building activities and workshops that promote independent living skills. This position works closely with Affordable Housing partners to ensure housing stability. This is fully on-site position requiring onsite work in San Francisco, This is fully on-site position requiring onsite work in San Francisco, five days per week-Monday through Friday from 9:00 a.m-5:30 p.m.
Essential Duties and Responsibilities:
Responsible for providing Case-management and skills development to participants in program
Conduct monthly meetings with participants assigned to caseload to help participants identify and accomplish short and long-term goals
Create and deliver impactful service plans for participants that focus on achieving outcomes in the areas of wellness, budgeting, long-term housing, education, employment, parenting, relationships and overall health
Coordinate with referral agencies, identify community resources, track participants' progress, and develop creative ways for participants to practice and strengthen their independent living skills
Prepare clients to maintain housing after program exit or help them to identify alternative housing options if necessary
Support participants to develop and practice social emotional learning skills that promote conflict resolution and decision making
Assess educational status and interest of participants and support their educational goals
Work collaboratively with Education and Employment Specialist and participants to improve resumes and cover letters, submit job applications, prepare for interviews and secure full and part-time employment
Coach participants on handling work-related issues, promote self-advocacy and increase success in job retention
Form partnerships with community agencies that can help link participants with community connections, building networks, wellness and employment opportunities
Create new linkages with community-based organizations, on-site resources and housing stability support through the case management process to help build participant transition to independent living.
Help participants move into supported housing. This includes helping participants secure and transport furniture, shop for home necessities, and locate resources in their new community
Ensure that apartments meet cleanliness standards by conducting monthly apartment inspections
Display and disseminate community resources relevant to residents and provide resource, referral and eviction prevention support to participants as needed
Client meetings scheduled as needed to ensure successful transition to independent living
Support property management to address serious housing concerns related to participants
Guide tours for funders and community providers to demonstrate how First Place for Youth partnerships with Affordable Housing agencies provide TAY with a stable and affordable housing solution
Coordinate community events and workshops to assist participants with independent living skills
Actively participate in weekly coordination meetings with property management staff and agency partners
Documentation/Administrative Duties
Attend weekly program meetings, monthly agency staff meetings, and individual weekly supervision with TAY Program Manager
Maintain files for caseload and update monthly paperwork
Ensure proper and timely documentation of services including written case notes
Work collaboratively with colleagues across the organization and within respective department
Other duties as assigned
Qualification and Requirements:
BA Social Welfare, Psychology, or equivalent required
At least three years' experience working with homeless, at-risk, transition age (18-24-year-old) or foster youth populations with complex presentations including presentations of chronic trauma.
A strong dedication to cultural competency that creates conditions that remove barriers and increase a sense of belonging.
Must be comfortable working in community settings.
Experience working with participants who have mental health and substance abuse issues
Experience in creating and delivering impactful service plans for participants
Experience in youth development model, asset-based relationship building, and experience in motivating youth,
Strong clinical skills, and/or a desire to learn more about the effects of the child welfare system on the lives of youth and young adults
Must be able to pass a Community Care Licensing background check

The Adult Services Division has a full-time opportunity for the position of Social Worker I/II. Role description

The Metro Mental Health Specialist works as a team member of a multi-disciplinary homeless street outreach team (MDT), in collaboration with social service professionals, ensuring the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Health Services (DHS).
The Metro Mental Health Specialist as part of the DHS ICMS Street Based Engagement Outreach (SBE) Team, will work in a variety of settings that will involve traveling on public transportation and on HYC designated vehicles. The Metro Mental Health Specialist will travel with the MDT Outreach team to a variety of sites including but not limited to transit stops on bus and rail line stops/stations, homeless encampments, shelters, drop-in centers, mobile outreach teams, and other areas as designated by DHS.
The Mental Health Specialist will use the whatever it takes approach to help people experiencing homelessness with mental health support and interventions on the field.
Essential Duties and Responsibilities:
Facilitate connections with local mental health providers to establish a network of resources.
Provides mental health assessments, counseling, and interventions when necessary.
Provide crisis intervention implementation, especially for clients experiencing the effects of mental health crisis.
Facilitate training among the MDT team regarding helpful practices, methods, and interventions for addressing mental health among people experiencing homelessness.
Collaborate with the MDT outreach team to engage with people experiencing homelessness on the Metro transit system and help connect them to local resources to help them achieve their goals and improve their quality of life.
Conduct triage and assessment tools to understand the needs of individuals seeking services.
Develop care plans and treatment plans and be able to help clients maintain and implement goals towards addressing housing insecurity, mental health, and substance use disorders.
Meet with established clients on a regular basis to review, evaluate, and support goal completion.
Help connect clients to mainstream benefits.
Maintain documentation standards as set forth by the program contract and HYC polices.
Complete all necessary paperwork to track client progress (daily logs, ISP, etc.) as well as enter information into the appropriate data management system (HMIS).
Compete paperwork on each face-to-face and telephone contact with the client or any collateral contact by close of business on the next working day.

Qualifications

Demonstrate knowledge of and experience with advanced and evidence-based interventions including, but not limited to: harm reduction, motivational interviewing, and housing first.
Ability to meet the geographic range of the position. Position requires frequent travel throughout Los Angeles County.
Physical demands include but are not limited to frequently required to stand, sit, walk, stoop, talk, hear, reach above and below shoulders; use hand and finger dexterity, keyboarding and making and receiving telephone calls.
Ability to successfully pass a TB screening test.

Wayfinder's Kinship Support Services Program is a kinship navigation service for grandparents, extended family members or close family friends who step up to raise children so they avoid the trauma of separation from their birth family. To accomplish this, the Social Worker II will provide engagement, assessment, goal planning, skill building, referrals, advocacy, and support during crisis. Assist families in locating funding for services where possible. Accurately document all services provided, assessments, meetings, decisions, and reports. The social worker II will work cooperatively and collaboratively with team members and community partners.
Primary Responsibilities:
Promotes the Continuous Quality Improvement (CQI) process by identifying quality indicators and opportunities for departmental and organizational process improvement.

The SBCC Academic Counseling Center Counseling Assistant supports academic counselors with a variety of projects and is the first point of contact for students who come to the Academic Counseling Center Front Desk. Primary duties include: * Helping students to schedule academic counseling appointments and prepare appropriately for their session(s)
* Supporting students in navigating the community college system, providing referrals as needed
* Assisting counselors with the facilitation of college support services including various workshops and programs, e.g. Class Planning for new students
Work Schedule:
* This is an hourly position to work no more than 19.5 hours per week and no more than 175 days per fiscal year.
* The position is on campus in person during hours the Student Services building is open.
* Mondays, Wednesdays, Thursdays 8:30 am - 4:30 pm
* Tuesdays 8:30 am - 6:00 pm
* Possibility of working some Fridays 9 am - 1 pm
* Opened until Filled
Minimum Qualifications:
* Bachelor's degree and/or equivalent combination of education and experience
* Strong interpersonal, organizational, and written communication skills
* Able to work well independently and collaboratively as part of a team
* Able to work confidently with diverse student populations and is committed to practicing and promoting inclusivity
* Experience with or demonstrated ability to learn new software and technology
Desired Qualifications:
* Ideal applicants are knowledgeable about SBCC Student Services
* Two years of previous customer service experience preferred
* Bilingual skills, Spanish preferred

Job Summary: The Social Worker-MSW HIV Grant provides assistance to HIV patients and their families in the form of diagnostic evaluation; individual, family & group therapeutic services; crisis intervention, and resource referral. Identifies patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge. Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive, integrated discharge plan. Makes appropriate referrals for community services; may provide inpatient emotional support and counseling services, crisis intervention, and bereavement services. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care and services to all patient groups: Neonatal, infant, pediatric, adolescent, adult and elderly. Performs other duties as needed.
Education and Experience: Master's degree in social work required. One year of experience in clinical setting required. Previous counseling experience preferred.
Knowledge and Skills: Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position; operate/troubleshoot basic office equipment; type 30 words per minute without errors. others; provide leadership and supervision; follow policies consistently; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to positively and effectively manage complex interpersonal relations with patients and staff. Works independently without direct supervision.
