* Write referrals for all who do not pass minimum requirements and follow-up on action taken
* Participate in meetings with parents to create a care plan for children with special needs
* Work with school nurse on special activities in classrooms
* Participate in safety/health/wellness school functions
* Participate in special training
* Contribute to the creation and implementation of a school disaster plan

Screening Services CertifiedNurse Practitioner (CNP)
REPORTS TO: Clinic Director
The CNP provides clinical, consultive and administrative support to improve patient outcomes. Work performed in this position is complex and requires a significant amount of independence, flexibility, attention to details and the ability to handle multiple tasks simultaneously.
Clinical services are guided by a Standard Care Arrangement (SCA) between the CNP and a designated collaborating physician. The SCA shall be reviewed and signed by the CNP and collaborating physician every two years (see attachment).
Clinical services are provided in conjunction with other health team members including mammography technicians, volunteers, interpreters, and other support staff. Clinical services focus on cancer risk assessments, cancer screening exams, health teaching, care coordination and referrals to other health care providers as indicated. Additional services may include diagnosis and treatment of selected minor acute and chronic stable health conditions. Services may be provided offsite in a variety of public and private settings including worksite and other community settings.
Consultant services may include providing medical guidance for managing selected problems for other LifeCare Alliance programs. This may include pre-employment physicals, immunization protocols and reviewing camp physical exams.
Administrative services include program planning to improve or expand clinical services, developing or updating medical forms and assisting in establishing Columbus Cancer Clinic goals and objectives.
PRINCIPAL RESPONSIBILITIES:
Performing physical exams and cancer screening tests following established cancer screening guidelines
Diagnosing and treating selected common health problems within the CNP scope of practice
Consulting with the collaborating physician for management of problems beyond the CNP scope of practice
Patient referrals and care coordination
Documenting patient encounters and phone calls in the clinic's electronic medical record
Assessing and evaluating patient outcomes
Other duties as assigned. This includes serving on agency committees, maintaining local and national affiliations and memberships, helping cover agency phone during staff shortages, attending clinic staff meetings and mandatory LifeCare meetings, assisting with conducting clinic tours and assisting with clinic events as requested.
WORKING CONDITIONS:
Work is performed at both the clinic location and in various locations throughout Franklin and the contiguous counties. Working hours are generally between 8am and 4:30pm Monday through Friday but may include evening hours and weekend days and or nights.
QUALIFICATIONS:
Current Ohio APRN license
2 years of APRN experience in area of certificationCertified Family Nurse Practitioner or Adult Nurse Practitioner
