Job Summary:
Forestdale Inc. Preventive Service Program is a family support program for families facing or experiencing challenges which can impact their ability to parent their children. The program is designed to assist families in meeting the individual and family challenges in their lives. The program Case Aides assist the Case Planners to locate and enroll the family in concrete services and connect them to resources that will help to address the individual and family needs. Case Aides also escort families to appointments for support and guidance in navigating systems.
Major Responsibilities/Activities
Initiate and maintain regular contacts with families in their homes
Establish a trusting relationship with families
Provide assistance to clients with applications to concrete services like fair hearings and public assistance and establish contact with community resources
Assist in making and attending health and human service appointments, including activities related to employment and educational goals.
Assist parents to improve their skills to optimize the home environment for the child's well-being and safety
Responsible for weekly paperwork, including descriptive narrative of each home and office visit.
Entry of documentation required for all program systems (CARA, Connections, PROMIS)
Assist case planners to enroll clients in services.
Assist in monitoring clients use of services for mental health and/or substance abuse
Meet with the Case Planner and Supervisor to collaborate and make sure that treatment plan is being addressed.
Minimum Requirements
Bachelor's Degree in Social Work or related human service field
Self-motivated, organized, compassionate and creative
Have a working knowledge of the child welfare and family court systems.
Bilingual (Spanish) preferred

About the Role The Mental Health Therapist working in the Community and Residential CFTSS program is responsible for providing an array of direct clinical services for youth across both community-based and residential programs and settings. In the Community CFTSS program, settings include home, school, and community/office settings. In the Residential CFTSS programs, the setting is program-based office space. Overall responsibilities include screening, diagnosis, treatment planning, and therapy services through the Children and Family Treatment and Supports Services (CFTSS) program. The community-based program is provided under OMH auspices, while the residential programs are under OCFS. The Mental Health Therapist is expected to provide both in-person and virtual services.
What You'll Do
* Provide trauma informed, evidence-based individual, family, and group therapy for cases as indicated.
* Complete Licensed Evaluations (Assessments) to identify the child/youth's behavioral strengths and weaknesses, determine medical necessity to continue services and identify other potential services needed, provide a diagnosis, and
* Develop Treatment Plans describing the child/youth's conditions, detailing the scope of services to be provided, expected outcomes, frequency, and duration of treatment
* Provide Crisis Intervention Activities for child/youth experiencing psychiatric, behavioral, or situational distress
* Provide consultation with non-licensed professionals who also work with persons supported.
* Prepare appropriate and timely written documentation of all clinical services delivered in the Electronic Health Record.
* Participate in Family Treatment Conferences and monthly Treatment Team Meetings
Your Qualifications
* Master's degree from accredited program in social work, mental health counseling or creative arts therapy or doctoral degree in psychology.
* Must have one of the following New York State professional licenses OR have a limited permit with one of the following degrees:
o Licensed Mental Health Counselor (LMHC)
o Licensed Marriage & Family Therapist (LMFT)
o Licensed Psychoanalyst
o Licensed Creative Arts Therapist (LCAT)
o Licensed Master Social Worker (LMSW)
o Licensed Psychologist (Ph.D. or Psy.D.)
* Training and at least 2 years of experience in providing therapy/treatment services for at risk youth and families.
* Two years' experience in Cognitive behavioral Therapy or other evidence-based psychotherapy interventions preferred.
* Valid New York State Driver's License Preferred
* Bilingual/Spanish a plus.

Position Type/Expected Hours of Work: This is a full-time position. Program Description: in this new Clinical Support Program, Rising Ground will embed licensed professionals specializing in mental health, substance misuse, and intimate partner violence directly into child protective work in Brooklyn. These clinicians will collaborate closely with ACS staff and families to assess needs, provide tailored service recommendations, and ensure families are connected to appropriate supports. Responsibilities of the Position: The full-time Clinician provides direct clinical services and clinical guidance, training, and support to an ACS Child Protective Services team. The clinician works collaboratively with the CPS team, providing feedback on a family's mental health needs, assisting with referrals for subsequent mental health treatment and providing warm hand offs to community based providers.
This position requires daily in-person presence at an ACS field office in Brooklyn or in the community as needed. Occasional travel to other agency sites or meetings may also be required.

* Actively participate in planning conferences, acting as mental health expert

BASIC FUNCTION

The program operates under the overall supervision of the Program Director.

The youth care worker works as scheduled in the shelter, providing supervision, support and direction to children fostering their positive growth and development; maintains a hazard free , clean and orderly household; prepares meals to conform to menus; completes all required logs, forms and reports; participates in meetings as scheduled; accompanies groups or individual children for outside activities or appointments.
DUTIES AND RESPONSABILITIES
Provides a warm, homelike atmosphere by establishing an individual relationship with each child focusing on that child's positive growth and development.
Under the supervision of the Lead Shift Youth Care Worker, ensures an orderly schedule for the children by supervising their daily activities: wake-up, school or other program attendance, mealtime, study time, assigned age appropriate chores and bedtime, monitors individual appointments for each child to ensure punctual attendance.
Monitors each child's physical well being, ensuring suitable clothing, adequate food and all required medical treatments, including the dispensing of medication.

Accompanies and transports children to outside activities (including supervised recreation) and appointments as required.

The Youth and Family Specialist will implement multi-dimensional educational, vocational, economic and recreational opportunities for youth in New York Foundling child welfare programs on multiple health and developmental related topics that introduce them to new situations, ideas, people and challenge them to learn new skills.
Responsibilities
The Youth and Family Specialist will implement multi-dimensional educational, vocational, economic and recreational opportunities for youth in New York Foundling child welfare programs on multiple health and developmental related topics that introduce them to new situations, ideas, people and challenge them to learn new skills.
Provide service to all eligible youth, in collaboration with the youth's case planner, child welfare staff and/or families, individual and group formalized instruction in Independent Living Skills with a strong focus on health and wellness topics, interpersonal skills, vocational training and money management.
In preparation for discharge from care, provide youth with resource information and assistance in locating appropriate and stable housing.
Provide to all eligible youth vocational training either directly or through referral to a structured program
Provide assistance in selection and admission into the most appropriate educational setting.
Assist eligible youth in locating, maintaining, and preparing for a job
Develop contacts with outside consultant/trainers providing job readiness services, career counseling, academic tutoring, and IL related workshops. Research, develop and organize information on resource programs and services related to Independent Living.
Document all required service related contacts with youth. Maintain attendance records and participation reports on youth involvement in IL services
Participate in dispositional and FTC conferences, including the six month discharge meetings, treatment team rounds, and clinical meetings
Assist with the coordination of the Summer Youth Employment Program (SYEP)
Participate in IL projects, conferences, events, meetings, and supervision
Attend trainings and conferences for professional growth.
Assist youth in scheduling and maintaining appointments to plan for their future in an environment that is safe and supported.
Coordinate and arrange recreational activities for program youth
Complete job related tasks as needed
Requirements:
Bachelor's in Social Work, Sociology or Psychology
Must be able to do site visits and home visits.
Ability to commute to and around the Mott Haven Section of the Bronx.
Knowledge and fluent in Microsoft Office Suite (Word, Excel, Access, PowerPoint, Project Manager).
Written communication and writing skills.
Ability to work evenings and Saturdays as assigned.

Responsibilities
Interventionist will provide direct home-based interventions to address risk factors, increase protective factors, and ensure the safety of families.
Reports to assigned program supervisor or covering supervisor.
Provides home-based clinical treatment utilizing the Functional Family Therapy - Therapeutic Case Management (FFT-TCM) model to assigned caseload of up to 10 cases and provides coverage for peer's caseload as needed
Manages the extensive travel requirements involved in serving families in their homes.
Conducts assessments including intakes, review of referral information, identifying and engaging key participants, identifying relational patterns, and individualized treatment planning.
Engages families to sign for services and completes intakes with families within the required ACS timeframes.
Participates in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings.
Adheres to model expectations and implement with fidelity including the timely completion of progress notes, session planning guides, session impact scales and appointment notes in the Care4 documentation system.
Actively participate in weekly consultation and supervision and site visits when they occur.
Consistently meets all program and model Key Performance Indicators (including session pacing, required casework contacts, successful client outcomes, timely completion of Family Assessment Service Plans, data entry in all systems, etc.).
Identify and address family risk factors that give rise to possible child removal, ACS involvement, and out of home placement due to continued referral behavior or new risk/safety factors. This includes the completion of safety plans and incident reports if necessary.
Maintains clear, concise and timely documentation of treatment efforts in progress notes and Family Assessment and Service Planning (FASP) that demonstrate compliance with the FFT-TCM Model and supervisory review feedback.
Manages the extensive documentation requirements in all databases to successfully comply with mandates from agency, ACS and model.
Identify community-based resources for families, collaborate and build relationships with stakeholders/providers (i.e., ACS, schools, doctors, mental health providers, courts, etc.) to monitor participation in services and accompany families to appointments if needed.
Provides 24/7 coverage during the week and participates on a weekend on-call rotational schedule as applicable.

Job Summary
Parent Advocates play an important role in helping parents who have been struggling to maintain or improve their relationships with their children in foster care and help them along a path to stability so they can be reunited with their children safely and promptly. Forestdale is seeking candidates with “lived experience” to serve as Parent Advocates for families currently involved with the foster care system. The Parent Advocate is responsible for identifying and implementing strategies to effectively reach out to parents of children in foster care and/or at risk of placement. The Parent Advocate works with staff to meet parents, identifies and surveys the needs of families, provides support during family visits including visit coaching, assists in developing and facilitating parent support groups, and serves as an advocate for both specific and general parent needs.
