Position Summary:

Compensation:
Local Candidates: $70 - $72 per hour
Travelers (50+ miles radius): $77 - $78 per hour

Key Responsibilities
1. Profiling & Transcribing
Review patient profiles for appropriateness based on age, diagnosis, comorbidities, labs, allergies, height/weight/BSA, pregnancy/lactation status.
Transcribe and verify all medication orders-including specialized and protocol-driven orders-for accuracy in drug, dose, route, and frequency.
Recommend therapeutic alternatives when necessary.
2. Medication Dispensing
Dispense medications, prescriptions, and miscellaneous pharmaceutical items accurately and promptly.
Process phone, window, fax, and electronic requests professionally and efficiently.
Source medications economically while considering inpatient vs. outpatient status.
Sign all associated paperwork and labels legibly.
3. Medication Checking
Perform accurate unit-dose checks and update patient profiles as needed.
Verify medications stocked in automated dispensing machines and floor stock areas.
Check IVs, batch preparations, patient-owned meds, repackaged items, and extemporaneous compounds.
Inspect crash cart trays, rapid response kits, and emergency supplies.
4. Sterile Compounding
Prepare and supervise aseptic compounding of IVs, including chemotherapy agents.
Manage batched IV medication levels to prevent waste and support efficiency.
Maintain strong knowledge of sterile technique, TPN compounding, and hazardous drug handling.
Complete all required documentation and labeling.
5. Clinical Functions
Participate in clinical initiatives and therapeutic monitoring.
Document clinical interventions and clarification activities.
Support Pharmacy & Therapeutics Committee initiatives.
Identify and eliminate duplicate or inappropriate therapies.
Convert non-formulary requests to formulary medications when appropriate.
6. Drug Information
Provide accurate drug information to physicians, nurses, allied health professionals, and patients/families.
Utilize and interpret drug information resources effectively.
7. Performance Improvement & Customer Service
Answer phones and service windows promptly and courteously.
Independently troubleshoot issues as they arise.
Participate in performance improvement and cost-saving initiatives.
Support departmental and system-wide projects, teams, and committees.
Professionally supervise and collaborate with pharmacy technicians.
8. Education & Training
Complete all required competencies.
Participate in training programs for new and current staff, pharmacy students, and residents.
Stay current with literature, new medications, and evolving therapies.
Encouraged to maintain active membership in relevant professional organizations.
Skills & Experience
Preferred: 2+ years of hospital pharmacy experience.

Position Type: Full-Time Position Schedule: Friday - Sunday, 6:00pm - 6:00am Position Location: Gilbert, Arizona Position Overview:
Ensure appropriate dosing and verify compounded medications while upholding quality control and compliance according to Strive Pharmacy SOP's.
Key Responsibilities

Checking entered prescriptions for accuracy and appropriateness
Performing calculations to ensure ingredients within a formula are measured and utilized properly
Visually checking lots compounded in sterile and non-sterile labs
Visually checking individual prescriptions to ensure color, consistency, and general appearance
Answering and making clarification phone calls dedicated for pharmacists
Counseling patients
Offering guidance to providers regarding appropriate therapy and dosing for optimal individualized care
Required Qualifications
Minimum of Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) Degree
Active AZ Pharmacist license
Desired Skills
Experience in compounding preferred
Understanding of aseptic technique

ESSENTIAL JOB DUTIES AND FUNCTIONS

* Lead the development, implementation and sustainability of pharmacy services at TWCCH's health centers
* Supervise pharmacy staff including pharmacists and pharmacy technicians
* Oversee, direct, coordinate and supervise all pharmacy activities
* Work to facilitate and coordinate pharmacy operations
* Participate/assist in procurement of drugs, supplies and equipment used in preparing and dispensing drugs
* Perform duties of the clinical pharmacist
* Maintain clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in services, seminars, and conferences
* Ensure adherence to all applicable laws and regulations
* Maintain accurate records and documentation
* Develop and participate in pharmacy-related quality improvement and medication management initiatives.
* Provide clinical oversight and leadership for the 340B program to optimize its importance
* Coordinate and manage medication and sample inventory for clinics
* Work with the Primary Care team in meeting a patient's transitional care goals (may include home visits, practice encounters, etc.).
* Explore academic partnership integrations for workforce development with agencies such as (AHEC, Wilkes University, LCCC etc.)
