What will I do in this role with Garden Spot?
As an LPN, supporting the Garden Spot Communities' mission will include the following responsibilities:
Assist residents with resilient living. This may include personal laundry, normalcy of living, and reminders of various activities within the households and on Town Square.
Perform daily rounds on Meadow View units to ensure that policies are being followed and residents are being cared for properly.
Assist with resident admissions, transfers, discharges and deaths.
Answer all incoming calls in a timely and courteous fashion.
Ensure residents are receiving medication as prescribed by a physician.
Ensure accurate documentation of all medical records and reporting forms.
Prepare all paperwork necessary for all appointments, including the completion of any changes that may have occurred as a result of the visit.
Ensure staff is compliant with documentation requirements including but not limited to MAR's, ADL flow sheets and clinical record documentation; receives and processes physician orders including faxed and written orders.
Ensure that the facility operates in compliance with Department of Health Services (DHS) Regulations.
Oversee all medication changes and medication passes to ensure staff compliance in addition to compliance with State regulations.
Perform medication passes on the occasion or as needed and covers shifts when needed.
What do I need for this role with Garden Spot?
Licensed Practical Nurse with current Pennsylvania State License.
Minimum of two (preferably five) years of nursing experience, supervision, as well as clinical experience or education in areas such as restorative nursing, dementia care, personal care or long-term care is preferred but not required.
Current CPR certification (or willingness to obtain after hire).
Basic computer skills.

Essential Duties and Responsibilities:
Accounts for narcotics for the assigned neighborhoods
Receives report from previous shift's charge nurse for assigned neighborhoods and gives same to subsequent shift's Charge Nurse.
Notifies Resident Assistants of any unusual or additional care needed for residents assigned to them.
Administers medications and completes treatments per physician's orders following the Pharmacy Policy and Procedure Manual.
Makes rounds, checking all residents.
. Completes nursing documentation on resident's records (i.e. progress notes, flow sheets, monthly summaries, etc.).
Assigns additional duties to Resident Assistants (i.e. weekly supply orders and PRN, cleaning chores, etc.).
Assists with resident assessments, admissions, discharges, transfers and deaths.
Receives written, faxed and verbal orders per facility policy.
Maintains safety and infection control standards following the regulations of the State and Federal regulatory agencies.
Initiates Incident and Accident reports per policy.
Keeps Director of Personal Care informed of status of residents including all incidents and accidents.
Supervises Resident Assistants and requires accountability to comply with the facilities policies and procedures.
Responsible for attending mandatory meetings.
Adheres to Corporate Compliance Plan.
Complies with HIPAA Privacy standards.
Completes additional tasks as required to ensure that the highest quality care is maintained.

• VISION: Requires constant overall vision including peripheral vision, color and depth perception.
