At Garden Spot Village, our Personal Care LPNs provide direct and indirect resident care, administer medications, perform treatments, and support Certified Nursing Aides (CNAs). This position ensures high-quality, person-centered care while promoting resident safety, comfort, and dignity.
What You'll Do
Comply with established policies, procedures, and standards, including the Compliance Program/Code of Conduct
Follow Residents' Rights policies at all times
Maintain confidentiality of all resident information, including conditions and family matters
Follow established standards of nursingpractice and implement facility policies and procedures
Communicate pertinent information during and between shifts to ensure continuity of resident care
Conduct daily resident rounds and report problems to the nursing supervisor, initiating corrective actions as needed
Identify safety hazards and initiate corrective actions immediately
Participate in facility education programs
Supervise CNAs providing direct resident care and coach them according to established schedules
Ensure compliance with federal, state, and local regulations within the assigned unit
Build relationships with residents and families to obtain daily preferences and communicate them to CNAs in alignment with Person-Centered Care philosophy
Administer and document direct resident care, medications, and treatments per physicians' orders
Notify physicians of changes in resident condition, medication errors, abnormal lab/X-ray results, or any unusual observations
Participate in development and implementation of resident interdisciplinary care plans
Respond promptly to resident calls for assistance
Count narcotics, check emergency kits, and manage exchange keys
Provide daily instruction to CNAs regarding care and treatment procedures
Provide direct resident care as necessary, in addition to supervisory duties
Collaborate with residents, nursing staff, physicians, families, consultants, community agencies, and ancillary service providers
Participate in meal service while monitoring dietary needs of residents
Order, receive, and store medications appropriately
Ensure resident safety under your supervision
Observe all facility safety policies, including universal precautions for infectious waste and disease
Maintain professional appearance and wear a clean, neat uniform
Demonstrate dependability and punctuality by coming to work as scheduled
Participate in quality assurance activities and relevant in-services/training sessions
Engage residents in household activities, documenting participation and responses in PointClickCare
Perform other duties as assigned
What We're Looking For
LicensedPracticalNurse (LPN) in Pennsylvania under the State Board of Nursing
Ability to rotate shifts and work weekends as scheduled
Strong organizational, initiative, judgment, dependability, and leadership skills
Ability to work congenially with others and delegate wisely
Ability to teach necessary skills for proper care and documentation
Excellent customer service skills and ability to interact professionally with residents, families, physicians, and other visitors
May rotate shifts and work Saturdays, Sundays, and holidays as scheduled
May be available for evening/weekend activities or assist during weather emergencies
Additional Information
Our FLEX (per diem) LPNs have a minimum requirement of 16 weekend hours per month.

What You'll Do

Minimum of two (preferably five) years of nursing experience, supervision, as well as clinical experience or education in areas such as restorative nursing, dementia care, personal care or long-term care is preferred but not required.
Current CPR certification (or willingness to obtain after hire).
Basic computer skills.

Registered Nurse - Admissions & Assessment Coordinator (RNAC)

Fairmount Homes , a faith-based Continuing Care Retirement Community founded on Christ's love and Mennonite values, is seeking an experienced Registered Nurse - Admissions & Assessment Coordinator to join our Healthcare team.
Position Summary
This role is responsible for coordinating timely, accurate, and compliant MDS assessments, Care Area Assessments (CAAs), and interdisciplinary care plans. The RN Admissions & Assessment Coordinator oversees healthcare admissions, supports Medicare eligibility and documentation, facilitates IDCP meetings, and serves as a clinical and regulatory resource for the interdisciplinary team.
Key Responsibilities
Coordinate and manage MDS, CAAs, care plans, and RAI submissions per state and federal regulations
Maintain the MDS calendar and ensure timely electronic transmissions
Collaborate with the interdisciplinary team on Medicare eligibility, skilled needs, and care planning
Review and respond to admission referrals and oversee the admissions process
Ensure accurate Medicare documentation, HIPPS codes, ICD-10 coding, and physician certifications
Monitor Quality Measures (QM/QI) and Case Mix Index (CMI) processes
Provide education, mentoring, and ongoing support to nursing and interdisciplinary staff
Supervise assessment coordinators and Admissions Associate
Participate in committees, audits, daily huddles, and quality initiatives
Qualifications
Current RN license in Pennsylvania
Minimum of two (2) years of long-term care nursing experience
Working knowledge of MDS, Medicare PPS, and CMI
Strong organizational, communication, and computer skills

QUALIFICATIONS:
* High School diploma or equivalent.
* Current Pennsylvania license to practice as a Registered Nurse.
* Experience working with rehab residents preferred but not required.
* 2-4 years of related experience and/or training, preferably in a skilled nursing or retirement facility.
* Basic computer proficiency required. Must be comfortable using Microsoft Office, including Word and Outlook.
* Must have and be able to maintain a current CPR certification.
RESPONSIBILITIES INCLUDE:
* Manages medications, evaluates vital signs, completes treatments, and changes dressings.
* Monitors indwelling catheters and peripheral IV's.
* Lifts, turns, and transfers residents, as scheduled or as needed. Properly uses mechanical lifts and require 2 staff for all transfers.
* Monitors, responds to, and reports health changes of residents.
* Monitors the skin condition of residents, implements appropriate procedures for skin tears and pressure ulcers.
* Assists in any emergency situation.
* Documents physician's visits, inputs/outputs, pharmacy requisitions, and daily charting.
* Supports the RN Nurse Manager in overseeing Caregiver routine duties and acts as a resource person for Caregivers.

At Garden Spot Village, our Registered Nurse Supervisors provide leadership and oversight for nursing services, ensuring high-quality resident care, staff development, and regulatory compliance. This role is responsible for supervising certified nursing assistants, coordinating care plans, and supporting a safe and compassionate environment for all residents.
What You'll Do
Comply with established policies, procedures, and standards, including the Compliance Program/Code of Conduct
Follow Residents' Rights policies at all times
Maintain confidentiality regarding all resident information, including condition and family matters
Counsel certified nursing assistants and recommend personnel actions to the Director of Nursing Service as necessary
Observe and enforce infection control procedures related to nursing services
Ensure the safety of residents under your care or supervision
Adhere to all facility safety policies and procedures
Demonstrate dependability and punctuality by coming to work as scheduled
Work cooperatively with administration, nursing personnel, physicians, community agencies, residents, families, and consultants
Maintain a clean, neat uniform and professional appearance
Accept assigned duties cooperatively and assume accountability for responsibilities outlined in the employee handbook
Direct, evaluate, and supervise all resident care and initiate corrective actions as needed
Conduct daily resident rounds and report nursing care status to the Director of Nursing Service and Administrator
Supervise the implementation of nursing rehabilitation and restorative programs
Assist with analysis of incident and accident investigation reports, determining causes and implementing corrective actions as appropriate
Complete residents' MDS assessments and care plans in a timely manner, with follow-up as needed
Review newly admitted residents and compare physician orders with care plans to ensure accurate implementation
Participate in the Quality Assessment and Assurance Program for the nursing department in cooperation with the Quality Assurance Committee
Perform other duties as assigned
What We're Looking For
Graduate of an accredited School of Nursing
Current Registered Nurse (RN) license in Pennsylvania
Experience supervising nursing staff preferred
Knowledge of restorative nursing, long-term care, and infection control procedures
Strong leadership, communication, and critical thinking skills
Commitment to customer service, teamwork, and professional development
Must rotate shifts and work weekends and holidays as scheduled
May be required to work during emergency conditions (short staffing, severe weather, evacuation, post-disaster, etc.)
May be asked to work overtime to ensure adequate staffing
Additional Information
As a FLEX (per diem) employee there is a minimum of 16 weekend hours per month.
