About the Job Working Title: Integrated BehavioralHealth Specialist Classification & Title: Mental Health Care Professional 2, 8302CC Employee Class: Civil Service Reports to: Integrated BehavioralHealth Program Manager FTE: 1.0 Objective/Overview CUHCC's mission is transforming care and education to advance the health of our patients and the communities we serve. The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients. This position contributes to the efficiency and effectiveness of the clinic's services through integrated behavioralhealth interventions with patients. Staff in this position are a mental health resource embedded in medical/primary care, and provide assessment and brief interventions for patients with mental and chemical health needs. Staff will utilize an approach that supports patients' holistic health goals, recovery, autonomy, and balances their care team's recommendations. Staff will provide care management services for high risk patients that promote improved functioning, and the development of skills and management of chronic conditions of debilitating socioeconomic life circumstances. This position facilitates integrated care and acts as a resource for mental health and chemical health needs.
This position is primarily in person at our clinic location, with options for some remote work. Scheduled hours are between 7:30am to 5:30pm, Monday through Friday, with some flexibility in scheduling within those hours..
Essential Functions:
Conduct mental and chemical health screening and assessments in medical visits (25%)
Participate in workgroups related to patients' physical, mental, and chemical health screenings
Review data on screening incidence, outcome, and follow-up
Provide panel management for patients in crisis situations and/or with care plans for complex needs to ensure follow-up plan is under way
Participate in training for MA/LPN, front desk staff, and/or providers to ensure universal implementation of mental health, chemical health, and domestic violence screenings
Respond to provider's requests for additional support for patients with identified psychosocial needs or crisis situations as a result of positive screenings
Participate in workgroups, projects, and/or clinic initiatives focused on improving client whole person care outcomes
Review medical providers' schedules for patients' mental health, social, or substance use concerns, and facilitate brief pre-visit planning sessions
Provide brief interventions for patients' psychosocial and holistic health needs (25%)
Assess social, mental health, and substance use needs while building rapport with patients
Conduct screenings for chemical dependency, mental health symptoms, and social needs
Document all services in electronic medical record (EMR)
Promote harm reduction strategies and the recovery model tailored to patient's stage of behavior change
Provide psychosocial education, skill building, behavior change, crisis intervention, suicidal ideation assessment, de-escalation, and/or goal planning
Follow-up on emergency room visits (review discharge summary, planning, and follow-up)
Schedule appointments, coordinate transportation, and/or provide no-show follow-up calls
Obtain Releases of Information to communicate with patient's external care team and/or identified supports
Participate in quality improvement efforts, care team consults, and interdisciplinary groups as assigned
Create care plans with patients and their care teams that include patient goals and provider recommendations
Provide referrals to treatment for patients with mental and chemical health needs (25%)
Complete referrals to internal (e.g. case management, ARMHS, care coordination, DV/SA advocates, pro bono legal clinic, pharmacy) and external supports (e.g. NA/AA, support groups, community centers, SUD treatment, government benefits, disability resources, etc.)
Provide transitions of care (e.g. follow-up on a referral, communicate changes to care team, introduce patient to relevant staff members)
Other administrative duties (25%)
Attend all staff meeting, care coordination team meeting, and workgroups related to psychosocial care coordination in the clinic
Collect and maintain resources necessary to implement responsibilities of position
Build community partnerships with corresponding agencies
Engage in practices consistent with Health Care Home and BehavioralHealth Home
Participate in speaking engagements as assigned

Qualifications
All required qualifications must be included in the application materials
Must meet qualifications in one of the following ways:
1) BA/BS in one of the behavioral sciences or related fields including but not limited to social work, psychology, or nursing from an accredited college or university AND
At least 2,000 hours of supervised experience in the delivery of services to persons with mental illness OR is proficient in the non-English language group of the ethnic groups CUHCC serves.
2) OR one or more of the following:
* has at least 6,000 hours of supervised experience in the delivery of services to persons with mental illness;
* is a graduate student in one of the behavioral sciences or related fields and is formally assigned by an accredited college or university to an agency or facility for clinical training or
* holds a master's or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university and has less than 4,000 hours post-master's experience in the treatment of mental illness.
This position requires a negative TB test, and a Hepatitis B vaccination.
Preferred Qualifications:
Bilingual and/or bicultural in one of CUHCC's dominant languages: Spanish, Somali, Vietnamese, Hmong, or Lao
Masters degree in a clinical mental health field, including Master of Social Work (MSW), MA in Counseling and Psychological Services, or MA in Family Therapy
Provisional licensure in Social Work (LSW or LGSW), clinical counseling (LPC), or marriage and family therapy (LAMFT), requiring supervision
Experience in managing clinical care using an EMR
Experience working in a Federally Qualified Health Center (FQHC) or community health
Knowledge of community resources, social service agencies and/or health care reform
Experience in cross-cultural behavioral/mental health service delivery.

Characteristics of the Position
General Overview
The BehavioralHealth Specialist, under the direction of the Clinical Program Manager, will serve as a clinician for Cook County Sheriff's Office (CCSO), Community Resource Center (CRC). The BehavioralHealth Specialist works with a multidisciplinary team of professionals to engage participants in services and assist them with care coordination. He/she will provide individual and group interventions, case management, and supportive services to clients of the Community Resource Center. He/she will administer assessment measures, develop and review treatment and care plans, and coordinate community linkages for participants. He/she will work with community agencies to coordinate service delivery for community corrections, reentry initiatives, and community members. He/she will provide services to persons returning to the community from the Cook County Department of Corrections (CCDOC), those residing in the community under CCSO supervision, survivors of domestic violence, and persons at risk of eviction.
Key Responsibilities and Duties
Assesses participants for determination of clinical, socio-emotional and reentry service needs; focuses on diagnostic and functional evaluations and makes recommendations on treatment goals and plans; collects information through interviews and observations.
Provides focused intervention of participants; provides consultation and training to CRC staff to enhance their skill in providing effective management of impacted participants.
Develops relapse prevention plans and helps participants maintain stability.
Establishes a supportive, motivational, and therapeutic relationship with participants; plans, organizes and leads structured interventions; works to effect behavioral changes and helps individuals make healthier life choices.
Guides individuals in custody in the development of skills to assist in dealing with specific problems; assists with the understanding and management of mental health and chemical dependency issues; serves as a role model and coach regarding development of problem solving, decision-making and conflict resolution skills.
Gathers information about participant's needs and provides resources; collaborates with other service organizations or entities, within and outside the Cook County Sheriff's Office.
Provides preliminary diagnostic information to related agencies in an effort to impact diversion strategies; acts as an advocate in coordinating required services.
Gathers information about community substance abuse and mental health needs and resources that can be utilized; collaborates with other service organizations or entities, within and outside the Cook County Department of Corrections.
Completes and maintains service-related paperwork, in accordance with state and federal requirements; writes reports, treatment plans and recommendations, progress reports and other reports, as required.
Refers individuals in custody to community resources, as necessary; works with Cook County Department of Corrections staff to facilitate coordinated releases to assist behavioralhealth service providers.
Evaluates the effectiveness of the treatment/care plan in correlation to the participant's progress in resolving identified problems and meeting identified goals.
Remains informed on new developments in counseling by reading professional literature and attending workshops or seminars.
Demonstrates use of ethics and complies with the standards of practice.
Knowledge, Skills and Abilities
Possess a thorough knowledge of the principals, practices and techniques of solution focused and/or substance abuse treatment.
Possess a working knowledge of community resources available to mental health and/or substance abuse programs.
Proficiency in evidence-based practices, assessment, treatment, and care planning and the ability to incorporate these practices in service delivery to persons facing eviction, mental health, substance abuse, homelessness, cooccurring disorders, or domestic violence.
Possess a nonjudgmental approach to working with the justice involved individuals and their families, persons facing evictions, and survivors of domestic violence.
Possess a thorough working knowledge of or the ability to grasp and utilize the policies, protocols, systems, procedures and technical vernacular of the department, agency, and County, as applicable to assigned duties and responsibilities quickly and effectively.
Ability to meet prescribed deadlines.
Possess advanced proficiency in maintaining highly detailed, efficient, and accurate work-related documentation and record keeping.
Ability to communicate effectively (orally and written) with participants, families, partner agencies, and CCSO staff in a professional and compassionate manner.

Minimum Qualifications

Two (2) or more years of full time, professional work experience in the mental health field.
One (1) or more years of full time, professional work experience in conducting mental health assessments.
One (1) or more years of full time, professional work experience providing case management in a racially and ethnically diverse environment.
Proficiency in the use of Microsoft Word, Excel, and Outlook.
Ability to work within the Cook County Department of Corrections, including the ability to work directly with the individuals in custody population.
Ability to walk long distances or stand for extended periods of time throughout the course of the assigned shift.
Ability to work any shift assigned, including days, afternoons, or midnights; must be capable of working holidays and weekends; must be capable of working mandatory overtime, as required.
Preferred Qualifications
Possession of a master's or higher degree, from an accredited college or university, in Counseling, Psychology, Social Work, Social Sciences or a closely related field.
Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW) and/or a Licensed Clinical Professional Counselor (LCPC).
Possession of certification as a Certified Alcohol and Other Drug Abuse Counselor (CADC).
Three (3) or more years of full time, professional work experience in a community mental health and/or hospital setting, performing mental health counseling and assessments.
One (1) or more years of full time, professional work experience in a correctional facility.
Possession of bilingual language skills.
Acquaintance with the laws governing the establishment and operation of the Cook County Department of Corrections, familiarity with municipal and state criminal codes and procedures and knowledge of basic penal management security.

The Licensed BehavioralHealth Staff is an active member of the Mobile Response stabilization Services (MRSS) program and provides support services to consumers and families who have experienced a behavioralhealth crisis. Under supervision of the Clinical Supervisor, the Licensed BehavioralHealth Staff provides immediate crisis assessment using appropriate screening tools and evaluations to determine safety risks and provide follow-up intervention and stabilization services to youth and families in the least restrictive setting that is clinically appropriate. They will assist in the identification of issues that contribute to a crisis and may create future crises if left untreated. Evaluators will develop and revise individualized crisis safety plans. As needed, Evaluators will facilitate hospital admission and arrangement of transportation. They also will be responsible for assisting in follow-up during the 7-day post-crisis period.
The MRSS Team is responsible for 24/7 mobile emergency responses (8 a.m. to 8 p.m. face to face and 8 p.m. to 8 a.m. telephonic) for youth and adults experiencing a behavioralhealth crisis in Montgomery or Preble counties in Ohio. The MRSS team operates from a strengths-based perspective, collaborating with first responders, community stakeholders, clinical leadership, and fellow mobile crisis responders. The MRSS team will work within a culture of crisis prevention, making every effort to ensure individuals are effectively stabilized within their home and community whenever possible. The MRSS team will ensure appropriate follow-up for individuals who have experienced a crisis event and ensure linkage to ongoing supports and services, operating in an enhanced localized crisis response system contributing to the goal of increased community stabilization.
Essential Duties and Responsibilities
Provides initial and ongoing mobile responses, inclusive of risk assessments for a young person in crisis.
Attends to safety and clinical concerns that require additional consultation, clinical intervention or assessment.
De-escalates the presenting crisis during the initial mobile response and subsequent crises during MRSS care.
Co-create and update safety and MRSS Plans with the young person and family.
Provide telephonic consultation to MRSS staff when the initial mobile response team is made up of non-licensed staff.
Conduct necessary assessments to determine young person and family needs, strengths, and protective factors including the use of the Ohio Children's Initiative Brief CANS when appropriate as identified in the Ohio Administrative Code.
For those instances when the initial mobile response team consists of unlicensed staff, the licensed staff coordinates with the unlicensed team to conduct the remaining elements of the initial mobile response phase when warranted (i.e. crisis assessment, MSE, lethality assessment etc.).
Works with the youth and family to establish and achieve family defined goals; develop skills to prevent future crises; and determine what other types of services and supports are needed.

The Choices Mobile Crisis Response Team is responsible for 24/7 mobile emergency responses for individuals experiencing an emotional, behavioral, and/or mental health crisis in Preble and Montgomery Ohio Counties. The Qualified BehavioralHealth Specialist (QBHS) is a full-time position (40 hours/week) operating during scheduled shifts. The QBHS provides crisis response as part of a team and has understanding of either mental illness or substance use disorder treatment and recovery as well as crisis response procedures, de-escalation techniques and how the individual's own behavior can impact the behavior of others. The incumbent is also aware of the functioning of relevant healthcare systems and how to engage therapeutically with a person with mental illness or a person in substance use disorder treatment and recovery. The QBHS operates from a strengths-based perspective, collaborating with Choices' Crisis Team, clinical leadership, and fellow responders. The QBHS performs a wide range of tasks to assist individuals in regaining optimal functioning within the community and mastery over their own recovery process.
Essential Duties and Responsibilities
Can articulate the mission, vision and services provided by Choices and the Mobile Response Team and partners.
Communicates the practice expectations of the Mobile Response Team to consumers, community partners, and funders.
Can participate in shifts based on a fire house model.
Responds to crisis referrals in person within 60 minutes.
Work with individuals for 42 days post crisis to provide stabilizing connections to the community and peer support.
Adheres to the policies and protocols established by Choices for the Mobile Response Program.
Incorporates the guiding principles and values of Systems of Care into their interactions with Individuals experiencing a crisis, fellow team members, Choices staff and system partners.
Communicates effectively with the Mobile Response Team Manager to ensure collaborative efforts and proactive response to issues and concerns.
Participates in at least one hour of supervision weekly with direct supervisor.
Follows established strategic plans and quality improvement procedures to achieve stated goals and enhance the experience of referred individuals.
Ensures collection of all necessary service data.
Completes required documentation on a timely basis.
Participates in scheduled meetings to review performance and quality indicators and makes recommended adjustments to improve the response and stabilization service.
Willingly completes other duties as assigned to advance the mission of Choices and the Mobile Response Team.
Qualifications
High School Diploma
Completed training for or education in either mental health or substance use disorder competencies; and who has demonstrated, prior to or within ninety days of hire the minimum competencies in basic mental health or substance use disorder and recovery skills listed in “Position Specific Competency” section of this job description.
Geographically located to assure a 60-minute response to an immediate need.
Prior experience with mental health crisis response preferred.
Strong communication and writing skills. Bi-lingual skills (Spanish) a plus.

Job Description:
Patient Care Technician facilitates and assists in meeting the needs of the patient. At the heart of Safe At Home is Margarets Place, named in honor of Joes mother. Margarets Place creates safe spaces inside schools offering healing, understanding, and empowerment. Through trauma-informed intervention and prevention services, every child learns what healthy relationships look like, how to care for themselves and each other, and how to support friends in needso that they feel safer and braver at school, at home, and wherever they go.
Today, there are 30 Margarets Place sites serving K12 students across New York, California, Ohio, and two international locations. Since its inception, Safe At Home has reached more than 275,000 young people, their families, schools, and communities.
This is a full-time, school-based position located at Riverview East Academy in Cincinnati, OH.

The Position:
The Safe At Home Foundation is seeking a number of masters level clinicians Social Workers, Creative Arts Therapists, or Marriage and Family Therapists to continue their groundbreaking work of teaching youth what healthy relationships look like and how to care for themselves and each other. The school-based counselors/trauma-informed therapists are at the heart of the organization and provide a safer place to help children strengthen their resilience, ground them in the support of their communities, and underscore that the violence is not their fault.
Counselors will provide trauma-informed care, consultations, and training to school staff, families, and communities. Working under the supervision of a clinical supervisor, this position will provide an excellent opportunity for new social workers to obtain their clinical hours as well as valuable experience working with experts in the field. The position also offers the ability to work in both intervention and prevention, allowing counselors to develop a range of approaches and a more comprehensive understanding of the multifaceted impact of trauma.
The counselor must be a team player but also able to work independently. Approaching the job with an anti-oppressive lens, they must have multi-cultural humility and the ability to speak to a wide range of people with empathy and respect. The ideal candidate will demonstrate a passion for strengths-based youth empowerment work, exhibit creativity in their approach to challenges, and be self-motivated. Additionally, counselors will maintain the highest level of professionalism and ethical behavior.
Specific Responsibilities:
Be physically present on the school campus when school is in session.
Provide individual and group counseling to youth exposed to violence or abuse.
Teach trauma education/violence prevention curriculum to youth.
Develop, implement, and facilitate a weekly peer leadership group, peer leader-run events, and field trips, as needed.
Develop and implement anti-violence campaigns and activities with peer leadership groups promoting healthy relationships and raising awareness about violence related issues.
Provide outreach, referrals, and workshops for parents/legal guardians.
Provide outreach, referrals, and training for staff and administration.
Attend committee and staff meetings and develop and maintain relationships and collaborations with staff and administration.
Participate in weekly individual and group supervision.
Comply with the best practice standards for practice and documentation as per JTSAH and the state.
Participate in and/or accommodate on-site program research, evaluation, and site visits.
Participate in professional development related to job responsibilities and professional growth (e.g., clinical practice, adolescent development, navigating systems, etc.)
Perform other duties as assigned.
Qualifications:
Masters degree in Social Work from an accredited institution, or a related field.
Active, unrestricted license conferred by the Ohio Counselor and Social Work Board.
Experience working with children and adolescents.
Solid understanding of interpersonal violence (e.g., intimate partner violence, teen relationship abuse, child abuse, etc.) and trauma-informed principles and practice.

The Role Itself
Kids Thrive is currently seeking experienced Therapists to work in a Pediatric Mental Health Program providing care and treatment to clients ages 3 to 10 in a school-based setting. Previous experience working with children is preferred. In this role you will be working inside of a school setting, many times inside of different classrooms.
Our compassionate therapists at Kids Thrive specialize in supporting a high-acuity early childhood population. With a focus on both group and individual services, our dedicated professionals provide a safe and nurturing environment where young minds can flourish.
Key Responsibilities:
Co-lead Day Treatment groups with Group Facilitator.
Provide individual and family therapy.
Conduct family interviews and offer referrals to appropriate resources.
Provide crisis intervention to clients as needed.
Complete Intakes, Diagnostic Assessments, Treatment Plans, and Discharge Summaries.
Contribute to treatment team meetings and collaborate with other providers on the team.
Job Requirements:
Education:
Masters Degree or Bachelors Degree in Social Work or Mental Health Counseling
Licensure in the State of Ohio (LSW, LISW, LPC, LPCC)
