    Job DescriptionDescription: Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Qualifications: Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) , or LPN License. Other Requirements: Proficient computer skills required. Experience working with an electronic health record preferred. Requirements: Essential Functions Of The Position: Treats patients and their families with respect and dignity. Facilitates patient flow throughout the encounter. Completes chart prep, driven by visit type and chief complaint including reviewing the medical chart for current information regarding the patient's medications, test results, hospital records, operative reports, notes from consulting specialists, etc. Any missing results or other information should be requested and obtained. Maintains and updates clinical information including identifying and documenting active problems, completing/resolving historical problems, updating social and family history, reviewing and updating allergy list, reconciling patient's current medication list, and determining if the patient needs refills after documenting any reported side effects of medications. Records chief complaint for visit, as obtained through patient interview. Performs patient intake including obtaining vital signs, contributing to the assessment of the patient's health status, and recording results in the EMR. Maintains and updates preventative health screening documentation. Identifies psychosocial needs of patients utilizing the depression screening tool and communicates concerns to the patient's medical provider. Utilizes nursing standards of care including assessment, plan, implementation and evaluation. Communicates the patient's condition to the medical provider including any significant changes noted during patient assessment. Performs point-of-care testing per established policies and procedures. Assists medical provider with procedures. Maintains aseptic technique during treatments and special procedures. Reports results of laboratory and other tests to the patient when directed by the medical provider. Utilizes available communication methods, which may include EMR and telephone, to convey patient-related communication as necessary. Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations. Stocks patient rooms with necessary supplies. Maintains current knowledge of medications and administration of injections. Understands and applies the Six Rights to Medication Administration. Works in a multi-disciplinary team setting and seeks guidance from providers, Registered Nurses and Licensed Practical Nurses. Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage. Assists in the resolution of complaints, requests and inquiries from patients. Maintains a cooperative professional working relationship with staff including but not limited to medical staff, and other departments, as well as outside agencies, community members and our patients. Completes annual education, certification and/or license requirements. Adheres to regulatory requirements. Attends/completes all mandatory in-services and trainings. Adheres to Grace Cottage policies, procedures, and Standards of Behavior. Strong team player and ability to function autonomously Requirements: Essential Functions Of The Position: Performs triage using the Emergency Severity Index, while considering both physical and psychosocial elements to determine the degree of urgency for treatment. Maintains a strong understanding of the Incident Command Structure and can operate fluidly within it, maintaining patient and facility safety during an internal or external crisis. Collects current symptoms, as well as a detailed patient history and then consults and coordinates with health care team members to assess, plan, implement and evaluate patient plans of care and treatment. Manages basic life support needs, based upon GCFHH nursing standards and protocol. Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays including but not limited to; Cardiac monitoring, EKG monitoring, respiratory ventilators, non-invasive positive pressure machines and high flow nasal cannula. Initiates corrective action whenever information from monitoring equipment shows adverse symptomatology within the GCFHH nursing standing orders in addition to notifying the Provider. Records patients' vital signs and all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms. Works directly under Providers, assisting them during exams, diagnostic testing and treatments Prepares and administers medications and records medication administration appropriately, including scanning patient identification and medications. Memory Care Unit The Memory Care Unit is designed to meet the changing needs of individuals as they experience the progression of symptoms and increases in cognitive loss associated with a diagnosis of dementia. Our early-stage program offers activity-based care in a warm, supportive and safe environment with a primary goal of helping individuals maintain dignity, purpose and functional ability. This highly specialized unit is staffed with very dedicated advocates and team members. Residents in this unit are the most vulnerable, usually unable to advocate for themselves. Post Acute Care The goal of the Post Acute Unit at Porter Medical Center - Helen Porter Rehabilitation & Nursing is to provide supportive, goal-directed care following surgery and/or hospitalization. Specially designed treatment programs provide patients with the preparation, support and education that enables them to return home as quickly as possible. Our goal is to help patients reach their highest possible level of independence. DEPARTMENT SCOPE OF SERVICE: The Progressive Care Unit (PCU) is an acuity adjustable unit providing care to a diverse patient population requiring medical-surgical and telemetry levels of care, as well as patients requiring a higher level of care, including, but not limited to, those on BiPap, Insulin drips, or cardiac drips. The PCU is a 24/7/365 service with 25 beds, which includes 5 monitored beds and 7 portable telemetry units. Medical patients are admitted by the 24/7 Hospitalist service, and surgical patients are admitted by the surgeon. PCU patients are reviewed daily by an interdisciplinary team at MDR rounds. (Multidisciplinary Rounds). Care is reviewed and discharge planning is done. KEY RESPONSIBILITIES: Under the direction of the RN, the SNAP directly assists with providing care to meet patient hygiene needs, takes and records vital signs and weights, assists with transfers, ambulation and transportation, assists with food trays/nourishments, feeds patients and records intake & output, answers call lights and telephone, performs clerical tasks, assists with organizing supplies, applies/uses specified patient care equipment, and maintains a safe environment while performing care. Requirements QUALIFICATIONS: Education: * High School Education or equivalency required. * Actively enrolled in an accredited nursing program. Experience: * Has successfully completed at least one year of clinical rotation. Licensure/Certifications: * Current Healthcare provider CPR certification/BLS required. OSHA CATEGORY: OSHA Category I - Tasks that involve exposure to blood, body fluids, or tissues require use of techniques of Universal Precautions as evidenced with blood borne pathogens standards.
