Qualifications
Requirements:
Graduate of an accredited or NLN-approved RN program.
New graduates are encouraged to apply in their final semester of nursing school before graduation.
Current license to practice nursing in South Carolina.
BLS and ACLS certifications required. Successful completion of CPI training during orientation.

Prisma Health Heart and Vascular Institute Cardiovascular ICU
Prisma Health Heart and Vascular Institute is South Carolina's first comprehensive, freestanding hospital dedicated solely to the prevention, diagnosis, and treatment of cardiovascular disease. Open to all doctors and patients, our continuum of care begins with disease prevention and management and extends to emergency services and procedures, with diagnosis, surgery, and rehabilitation, all performed in beautiful surroundings of our hospital.
The Cardiovascular ICU is a 12-bed acute care unit that recovers patients immediately post open-heart, thoracic, and vascular surgery as well as a robust TAVR and LVAD program. This unit performs various bedside procedures and therapies.
Essential Functions

Collects data and completes nursing assessment utilizing appropriate area and/or age specific standards/policies and tools. Completes appropriate forms. Assesses and recognizes subtle or actual changes in patients' status and patient response to intervention. Identifies nursing priorities for patient plan of care based on nursing assessment and other data as appropriate. Validates these priorities with healthcare team members and documents appropriately.
Develops an individual plan of care based on the age and assessed needs of the patient. Develops the plan in collaboration with the patient and/or family, the provider, and other members of the interdisciplinary health care team.
Updates the plan based on changes in the patient's condition and communicates changes in the plan to other members of the team.
Identifies realistic and measurable goals and outcomes to be attained prior to discharge. Validates these with the patient and/or family.
Demonstrates respect for the patient's/family cultural, spiritual, and belief system in planning for care.

Prisma Health Richland Hospital Neurosurgical Intensive Care Unit
The Neurosurgical ICU is a 16-bed unit caring for critically ill patients with injury or disease to the nervous system. The nurse to patient ratio is 1:2. Common diagnoses of patients include post-op craniotomies or tumor resections, interventional radiology procedures, ischemic or hemorrhagic stroke including SAH, post TPA administration, brain tumor and AVM. Care of this population incorporates a multi-disciplinary team of physicians with specialized training in neuro critical care, acute care nurse practitioners, critical care nurses with neuroscience training and critical care trained pharmacists. There is also a close involvement with physical therapists, occupational therapists, speech therapists, respiratory therapists and case managers. Nurses hired to NSICU will continue education specifically around the neuroscience patient population and will directly assist practitioners, while monitoring patients, during bedside procedures such as IVD placement, CVC insertion and intubations.

POSITION SUMMARY: Care Manager is responsible for the development, maintenance and implementation of the standards of nursing practice and objectives as it relates to the Minimum Data Set requirements and with the established policies and procedures of the governing body in compliance with federal, state and local regulations. The Care Manager is responsible for optimal reimbursement, proper documentation/diagnoses to justify the reason for a skilled service and reimbursement request.
ESSENTIAL FUNCTIONS:
• Coordinates/manages and works with clinical staff/therapy for proper documentation/care, programs and optimal reimbursement throughout the stay of a skilled resident
• Researches all pertinent medical information/diagnoses for optimal reimbursement
• Meet regulations for timely certifications/re-certifications, and signing of all orders
• Meet all requirements in working with Managed Care organizations
• Daily meetings with therapy and line staff on progress of skilled residents
• Monitors Nursing skilled documentation and provides continued education as needed
• Participates in the development and implementation of the resident assessment process, following the Minimum Data Set requirements of Federal, State and local regulations in accordance with Senior Living Communities/Wellmore policies and procedures
• Responsible for proper ADL Documentation and continued education/training
• Audits and analyzes the MDS documents for accuracy and appropriate completion dates.
• Timely submissions of all MDS's
• Oversees and schedules resident care planning meetings.
• Assistance on floor and on-call/manager on duty as needed
• Participates in and attend all required in-service training and education sessions as scheduled.

Evaluates educational material for compliance in the latest evidence-based practice, regulatory standard, and policy requirements.
Assesses educational program evaluation feedback to determine if changes in content are necessary. Based on evaluation feedback, revises content to ensure it is relevant, current, and effective. Makes all necessary revisions to the curriculum in a timely manner.
Evaluates transition to practice curriculum to ensure it is meeting the needs of the learner.
Maintains accurate records of educational offerings.
Generates reports necessary for informing leadership team members within Prisma Health on the individual or group status of education and training.
Collaborates with other health care professionals to develop and refine educational materials.
Provides consultation with healthcare leadership in response to new educational requests.
Maintains individual validation requirements needed to provide competency training to other team members.
Preserves clinical expertise and knowledge in the specific area of healthcare in which assigned to provide education and training.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Bachelor's degree. Master of Science in Healthcare field of study preferred.
Experience - Five (5) years of acute care clinical experience
In Lieu Of
In lieu of the degree and certification requirements, a Master's degree in a healthcare related field, five (5) year of acute care clinical experience and RN licensure may be considered.
Required Certifications, Registrations, Licenses
Licensed to practice as a Registered Nurse in South Carolina
Certification by specialty board credentialing organization recognized by Magnet
BLS certified
May require ACLS certification or other specialty education/training depending on clinical area of assignment.
Knowledge, Skills and Abilities
Basic computer skills
Knowledge of office equipment (fax/copier)
Data entry skills
Working knowledge of patient care equipment
Ability to demonstrate and maintain clinical expertise in the specific area of healthcare in which they will provide education and training.
Specialty training such as arrhythmia, learning management systems, and electronic medical record systems - Preferred
