About the Role: As a Physical Therapist (PT) in Home Health, you will play a critical role in delivering personalized rehabilitation services to patients within the comfort of their own homes. Your primary objective will be to assess, plan, and implement therapeutic interventions that improve patients' mobility, reduce pain, and enhance their overall quality of life. You will collaborate closely with patients, families, and interdisciplinary healthcare teams to develop individualized care plans that address specific functional goals. This role requires adaptability to diverse home environments and the ability to educate patients and caregivers on exercises and safety measures. Ultimately, your work will empower patients to regain independence and prevent hospital readmissions through effective physical therapy treatments in a home setting. Minimum Qualifications: Licensed Physical Therapist (PT) in the state(s) of practice within the United States. Graduation from an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or equivalent degree. Current CPR/BLS certification. Proven experience or clinical rotations in physical therapy, preferably including home health or outpatient settings. Strong knowledge of anatomy, physiology, kinesiology, and therapeutic modalities. Preferred Qualifications: Experience working in home health care or community-based rehabilitation. Familiarity with electronic medical records (EMR) systems and documentation standards. Additional certifications such as Certified Lymphedema Therapist (CLT) or Geriatric Physical Therapy specialization. Excellent interpersonal and communication skills tailored to diverse patient populations. Ability to work independently with strong organizational and time management skills. Responsibilities: Conduct comprehensive patient evaluations to assess physical abilities, limitations, and rehabilitation needs in a home environment. Develop and implement customized treatment plans based on patient assessments, medical history, and functional goals. Provide hands-on therapy, including exercises, manual techniques, and use of therapeutic equipment to improve patient mobility and function. Educate patients and caregivers on therapeutic exercises, safety protocols, and strategies to prevent injury or deterioration. Document patient progress, update care plans accordingly, and communicate effectively with physicians, nurses, and other healthcare professionals. Manage scheduling and travel efficiently to accommodate patient visits across various home locations. Ensure compliance with regulatory standards, ethical guidelines, and organizational policies in all aspects of care delivery. Skills: In this role, your clinical expertise in physical therapy will be applied daily to evaluate patient conditions and design effective treatment plans that address individual needs.
