Experience Experience working with the elderly population preferred. Must be listed in good standing on the New York State Nurse Aide Registry as a Certified Nursing Assistant with no findings of abuse, mistreatment, neglect or misappropriation of property/funds. Education High School graduate or GED preferred Essential Functions 1. Performs routine patient/resident care duties under the direction of the Registered nurse or LPN. Assists with direct care of the patient/resident (dressing, grooming, ambulating, toileting, transferring). Prepares and assists with meals and distributes water and other nourishments according to the needs of the patient/resident. Measures vital signs: temperature, pulse, respiration, and blood pressure and reports them to the nurse. Provides routine am/pm care and assists with routine skin care. Assists with I & O's, empties and measures patient's drainage containers and documents I & O appropriately. Collects specimens, labels properly, transports to proper area in a timely manner following established procedures. Participates in shift report communicating information to oncoming staff. Makes round willingly and promptly responds to call lights. Participates in clinical learning experiences for orientees and students. 2. Assists with transportation activities related to patient/resident care. Escorts patients/residents to ancillary departments. Escorts discharged patients/residents in a safe and efficient manner. Demonstrates knowledge and ability to safely and effectively use equipment. 3. Emergency Preparedness-Demonstrates working knowledge and understanding of emergency preparedness plans and operations. Aware of and actively participates in emergency preparedness procedures and operations. Aware of and actively participates in Code Red, Blue, Yellow, Pink, Orange, Green, and Silver drills and actual events. 4. Documentation-Completely and accurately documents patient/resident care activities and response in a timely manner in accordance with facility policy. Education, License & Cert: High School Diploma or equivalent required. Preferred for NY. Requires valid driver's license, reliable vehicle, and active vehicle insurance. Requires Basic Life Support certification. For NY: Home Health Aide Certification Required. If a caregiver has a Certification as a Nursing Assistant, then enrollment into Home Health Aide class will be arranged and covered by Guthrie Lourdes Hospice. Continued employment is dependent on successful completion and maintenance of the Home Health Aide certification. Until certified, the caregiver may shadow a preceptor per Hospice COP (Condition of Participation) guidelines. Prepares basic meals as necessary following patient prescribed diet. Assist patient with eating as needed. 3. Turns and repositions patients in bed for comfort. 4. Assists patients with ambulation, transfers, and exercises, utilizing appropriate body mechanics and assuring patient and caregiver safety. 5. Performs simple procedures, i.e.: changing urinary drainage bag, changing simple dressings, following appropriate infection control procedures. 6. Provides personal care and help with ADL's under written plan, 7. Documents care provided in accordance with the Hospice Home Health Aide plan of care in a timely manner. 8. Records vital signs as directed. 9. Performs light household tasks to ensure maintenance of clean, safe, and healthy environment, including changing bed linens. 10. Promptly reports any change in the patient's condition to RN / designee. Notifies RN of any patient / family concerns or questions. 11. May direct and remind patients to take medication or treatments only as directed by RN. 12.Follows universal infection control precautions with all patients and additional patient specific precautions (ie.
