b. Documents UR findings in appropriate computer system and screen.
c. Utilizes reports and other mechanisms to identify cases for UR screening and follows procedures for follow up as necessary.

Position Summary:
Under the direction of the interdisciplinary team, the Hospice Nurse works under the general direction of the Hospice Medical Director and the attending Licensed Independent Provider (LIP) in the provision of skilled nursing assessment, planning and care to maximize the patient's comfort at end of life and support the patient's families/ primary caregivers. The Hospice RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending LIP's orders, the interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible for providing direct supervision of Home Health Aides and or LPN's providing care for Guthrie Hospice.
Education, License & Cert:
Registered Professional Nurse currently licensed in New York. Knowledge of nursing theory and practice at a level normally associated with an associate degree. Baccalaureate Degree and/or Hospice and Palliative Care Nursing Certification preferred. Education and experience requirements may vary according to the combined background of the individual.
Requires valid driver's license and reliable vehicle.
Experience:
One (1) year nursing experience in an acute care setting but a minimum of 3 years Med /Surgical Nursing or Similar Experience is desired.
Previous experience with end of life and /or chronic disease management is preferred.
Experience and skill in supervision desired.
Essential Functions:
Assessing the patient's and the family/caregiver's physical, psychosocial, bereavement, environmental, safety and developmental needs;
Developing an individualized plan of care, in conjunction with the hospice interdisciplinary team based on assessment, identification of needs, and patient and family/caregiver goals and preferences;
Providing care to patients and families/caregiver through the utilization of interventions and evaluation of outcomes of care;
Performing ongoing assessments and revision of the plan of care, with interdisciplinary collaboration, in response to the changing needs of the patient and family/caregiver;
Performing comprehensive assessment of pain/symptoms and developing individualized symptom management plan;
Anticipating, preventing, and treating undesirable symptoms;
Providing support, instruction, and education to the patient/family/caregivers;
Documenting nursing assessments, identified problems, measurable goals of care, limitations to provision of care, care interventions and response to care;
Coordinating all patient and family/ caregiver services with the hospice interdisciplinary team and other health care providers;
Consulting and collaborating with the hospice interdisciplinary team and others involved with the patient's care;
Developing the hospice aide plan of care and providing direct and indirect supervision of aide at least every 14 days;
Maintaining the dignity of the patient;
Recognizing and supporting the patient's and family/caregiver's spiritual and cultural beliefs;
Providing holistic, family - centered care across treatment settings to ensure continuity of care;
Participating in the hospice program's quality assessment performance improvement program;
Assessing the ability of patient and family/caregiver to safely administer medications and perform treatments;
Reporting abuse and neglect in accordance with state laws and regulations and Guthrie policy.

Position Summary:
The Registered Nurse (RN) provides professional, comprehensive nursing care for patients in an acute care environment. Guthrie Cortland Medical Center (GCMC) is searching for a compassionate & skilled Registered Nurse (RN) to provide exceptional care in our Acute Care Unit at our beautiful hospital in Cortland, NY. The position is full time on Night Shift working three 12 hour shifts 7:00 PM - 7:30 AM with an every other weekend rotation.
Department description: 2 Central GCMC - 28 bed acute care unit at Guthrie Cortland cares for patients with respiratory illnesses, diabetes, wounds, as well as other acute care illnesses, and end of life care. Post operative unit for ortho, abdominal and urology patients.
Our Acute Care department has been described by our staff as having a caring & comforting environment with an incredible team that feels like family. The department is fast-paced but has a strong focus on teaching and learning. Our RNs work on a Care Team with a LPN and a PCP sharing a patient load.

Job Summary:
Responsible for delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. Follows medication administration policy using e-Mar, bar coding and scanning of medications and has at least an 85% scan rate.
Insertion of IVs under aseptic technique and follows IV policy by changing site, tubing and dressing and documents accordingly.
Always follows hospital policy when administering blood products.
Maintains security of electronic signature with Omnicell and accounts for all narcotics removed from automated dispensing system through proper waste/return administration documentation.
Patient and Family Education
Assesses and addresses educational needs of patients and families to include disease state, care and treatment.
Documents educational needs.
Distributes appropriate teaching sheets.
Includes evaluation of learner's understanding post-teaching.
Safe Discharge/Discharge Planning
Contributes to or initiates discharge plan upon day of admission and participates in interdisciplinary planning.
Completes all pre-discharge checklist items.
Always gives patient correct forms and discharge instructions by using 2 patient identifiers.
Provides patient a safe discharge and appropriate follow-up information and community resources are in place.
Assist Providers-Assist providers in performing procedures and treatments per unit, department specific and hospital policy.
Assist physicians/providers with beside procedures and remains competent with unit-specific procedures.
Performs read back of critical values and telephone orders when interacting with physicians as warranted.
Uses SBAR when giving physicians report on change in patient condition.

Position Summary:
