Responsibilities
Provide in-home, para-professional services necessary for caring for patients' personal needs under the direction of the RN.
Perform and/or assist patient with personal care activities, such as, baths, showers, skin and hair care, and oral hygiene. Assure universal safety precautions and direction on the care plan are followed when providing care. Provide care that meets the patient's needs.
Notify RN of any changes in patient condition observed in a timely manner and document appropriately.
Record all required information on patient chart completely and accurately. Assure documentation is completed per organization protocol.
Assist patient with transfer and ambulating. Perform range of motion exercises. Assure all activities are performed safely and follow the principles of good body mechanics.
Perform general housekeeping of patient's area and change patient's bed linens. Assist family with light housekeeping activities, such as dusting, vacuuming, damp mopping, etc.
Assist client with self-administration of oral medication/s. Remind patient when to take medication/s.
Prepare simple meals in accordance with dietary instructions and assist patient with eating if necessary.
Assist with of foley catheter and colostomy care to ensure proper hygiene. Assist patient with changing colostomy bags. Perform peri care. Measure urine. Assist with ostomy care and assure skin is clean around stoma.
Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
Adheres to HMH's Organizational competencies and standards of behavior.
Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs. and stands a minimum of 8 hours a day.

Short Description
Cooper University Hospital Interventional Radiology department is looking for an Advanced Practice Provider to join our collaborative team of physicians and practitioners who use advanced imaging and other innovative methods to assess and treat complex conditions less invasively and with unprecedented precision.
The role of the Interventional Radiology APP includes procedural and clinical care services to adult patients in the inpatient and outpatient setting at Cooper University Hospital and MD Anderson at Cooper University. Duties include independent and collaborative management of new and established patients, reviewing pertinent imaging, providing consult services and performing image guided procedures.
Advanced Practice Provider (APP) Functions
The Advanced Practice Nurse will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses' Association Code of Ethics. The advanced practice nurse utilizes the scientific process and national standards of care as a framework for managing patient care.
The Physician Assistant will function within the scope of practice as defined by the supervising physician.
APPs practice autonomously and/or in consultation with a collaborating/supervising physician and other health care professionals to assess, diagnose, treat and manage patients' health problems/needs.
The APP will perform a patient history and physical examination to develop an individualized plan of care.
Services include, but are not limited to ordering, performing, supervising, and interpreting diagnostic and laboratory tests, procedures and prescription of pharmacologic agents and non-pharmacologic therapies.
The APP may provide therapeutic measures as appropriate and provide teaching, education and counseling to individuals, families and groups.
In addition to diagnosing and managing acute and chronic illnesses, APPs will emphasize health promotion and disease prevention.
APPs will serve as health care researchers, professional educators, interdisciplinary consultants and patient advocates.
Prescribing Medications
Prescriptions may be provided by the APP, where clinically indicated, and shall be in accordance with the current APP prescribing guidelines and in accordance with state and federal law.
Continuing Education
The APP will stay current with medical trends and provide evidence-based practice medicine by completing the required continuing education as mandated by certifying organizations.
The autonomous nature of the APP requires accountability for healthcare outcomes. Insuring the highest quality of care requires national certification, periodic peer review, clinical outcome evaluations, adherence to the code for ethical practice, evidence of continuing professional development and maintenance of clinical skills.
Training
1:1 orientation with experienced APP/Physician covering all roles and expectations, duration based on prior experience.
Experience Required
Interventional Radiology as an Advanced Practice Nurse or Physician Assistant is required
Prior experience in Interventional Radiology as a Registered Nurse will be considered.

The Advanced Practice Nurse will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses' Association Code of Ethics. The advanced practice nurse utilizes the scientific process and national standards of care as a framework for managing patient care.
The Physician Assistant will function within the scope of practice as defined by the supervising physician.
Advanced Practice Provider (APP) Functions:• As licensed independent providers, APPs practice autonomously and/or in consultation with a collaborating/supervising physician and other health care professionals to assess, diagnose, treat and manage patients' health problems/needs.• The APP will perform a patient history and physical examination to determine a working differential diagnosis and develop an individualized plan of care.• Services include, but are not limited to ordering, performing, supervising, and interpreting diagnostic and laboratory tests, and prescription of pharmacologic agents and non-pharmacologic therapies.• The APP may provide therapeutic measures as appropriate and provide teaching, education and counseling to individuals, families and groups.• For uncommon or unstable conditions, the APP will formulate diagnoses in consultation with his/her collaborating/supervising physician and either refer to a specialist or participate in the dual management and treatment with his/her collaborating/supervising physician.• In addition to diagnosing and managing acute and chronic illnesses, APPs will emphasize health promotion and disease prevention• APPs will serve as health care researchers, professional educators, interdisciplinary consultants and patient advocates.• Prescribing Medications• Prescriptions may be provided by the APP, where clinically indicated, and shall be in accordance with the current APP prescribing guidelines and in accordance with state and federal law. Rutgers, The State University of New Jersey, is seeking a Health Care Child and Family Nurse for the Child Health Program/Child and Family Nurse Program within the School of Nursing. The Child and Family Nurse (CFN) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care, protection and well -being needs of children residing with their biologic parents.

Experience Required
Looking for APP (Acute care certified APN only or PA) to work in collaboration with Cooper intensivists to manage intensive care patients. Ideal candidate will have experience in critical care and/or hospital medicine and ideally have experience with POCUS and performing procedures such as intubation, central lines, arterial lines etc. New graduates will be considered but would need to be highly motivated to learn and seek out independent opportunities in addition to work related orientation to become autonomous in role.

Overview The Children's Regional Hospital at Cooper University Hospital is a pediatric facility that consists of several units that meet the diverse needs of pediatric patients: Pediatric unit, Pediatric Intermediate unit (PIMU), Pediatric Kostic Infusion Room (PKIR), and Pediatric Intensive Care unit (PICU). We provide care for pediatric patients aged 0 to 18 years. We offer subspecialty services through consultation and referral in areas such as Endocrinology, Genetics, Infectious Diseases, Neurology, Neurosurgery, Gastroenterology, Otolaryngology, Pediatric Surgery, Trauma, and Hematology. The Family-centered rounds are conducted daily with an interdisciplinary care team including physicians, professional nursing services, child life specialists, social services, and nutrition support. All our pediatric units are secured with restricted access for staff, with visitors gaining entry via monitored access. All pediatric patients wear security tags to assure their safety. Our facilities include a teen room, a spacious playroom for younger patients, and a family waiting room. Our Pediatric unit and PIMU have 38-bed capacity. The Pediatric unit is in Kelemen building on South 7th and offers 26 beds. The PKIR is also located in Kellemen building on S7th floor. The area consists of large room with two bays and available for inpatient and outpatient procedures, infusions, and various tests. Experience Required 0-2 years' experience required.
