Job Summary:
Functioning within the context of the framework for professional nursing practice, the Cystic Fibrosis ClinicNurse Navigator is a registerednurse experienced in patient throughput, preventing transitional care gaps, and resolving issues to enhance the quality and continuity of a patient's or populations health care leading to improved health outcomes and equitable care. This role supports the HHC mission to improve the health and healing of the people and communities we serve. Under provider direction, the Cystic Fibrosis ClinicNurse Navigator provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Nurse Manager Manager.
Job Responsibilities:
• Functions as a member of an interprofessional care team in an expanded nurse role to help patients transition from the acute care setting (HH ED or inpatient). The goals include reducing all-cause readmissions, and inappropriate ED utilization, improving care coordination for patients during the transitional care period, and ultimately improving care quality and access for vulnerable populations. This role will be responsible for educating the HH community at large and advocating for resources to enhance patient healthcare engagement and expand the collaboration and communication between (inpatient/ambulatory/outpatient/attending/transitional care/specialty care/primary care) providers and care teams for high risk/complex patients.
• Partners with the inpatient (i.e. acute care, IOL, STR) or ED physician and care team to proactively identify potential transitional care gaps for this patient population, and establish a safe transition plan. Key strategies include ensuring a patient/caregiver agreed upon CF Clinic and urgent specialists scheduled appointment(s) with transportation, verifying patient has necessary DME, finalizing an achievable community medication plan, completing diagnostic workup, educating the patient on disease and symptom management, and incorporating a patient-centered home care plan.
• Performs post-hospitalization/ED transitional care strategies within 24-48h after discharge, including post-discharge phone calls, patient education, symptom management, and medication reconciliation, and collaborates with CF clinic physician and (clinic and community) care team to minimize identified gaps in care.
• Throughout the post-inpatient/ED transitional care period, facilitates the completion of the diagnostic workup, follows up on unresulted diagnostics, collaborates with homecare, pharmacy, and DME to ensure the patient has necessary supplies/medications/resources, obtains necessary authorizations, and schedules additional consultant appointments.
• Collaborates with clinic physicians to resolve issues and to advance the treatment plan until the patient has an established primary care provider.
• In collaboration with the CF Clinic physician, assists the patient in identifying a primary care practice for continued care and facilitates the transfer of care to that practice
• Documents all communication, transition plan, implemented strategies, and patient outcomes in EPIC.
• Establishes a therapeutic rapport with patients and demonstrates a commitment to serve as a patient advocate.
• Demonstrates the ability to work independently as well as collaboratively as a member of the health care team in order to provide safe patient care and prompt and efficient service. The Cystic Fibrosis Nurse Navigator provides transitional care strategies to his/her peers/colleagues and patients based on need/coverage.
• Attends and actively participates in care team meetings to facilitate a safe transition plan or resolve a patient issue.
• Establishes evidence-based standard work and workflows. Develops and implements processes that improve the patient experience. Collects and analyzes patient and program level data identifies areas of opportunity, recommends improvements/revisions or program development, and leads/participates in the idea/plan implementation.
• Applies the nursing process as appropriate within the context of the organization's framework for professional nursing practice and following guidelines established by the team.
• Provides office-based nursing care in collaboration with provider, communicates with provider regarding patient needs, nursing assessments, and recommendations, demonstrates independent nursing actions based on assessment and problem identification.
Qualifications
BSN required, MSN preferred
• Minimum five (5) years of nursing experience, Inpatient and Ambulatory nursing experience preferred.
• Current Connecticut Nursing License
• BLS Certification

Position Description:
The Cardiovascular Operating Room (CVOR) provides specialized care for patients undergoing cardiac or vascular surgery. Common procedures in the CVOR include coronary artery bypass surgery, valve replacements, heart transplants, aortic artery repairs, vascular bypass, endarterectomies, and endovascular cardiac surgeries, among others. Nurses in the CVOR are integral members of a highly functioning and dynamic interdisciplinary team, working closely with cardiac and vascular surgeons, advanced practice providers, surgical technologists, perfusionists, anesthesia providers, and support staff. Additional Responsibilities Include:
The primary role will be as a circulating nurse in one of our seven cardiac or vascular operating rooms. High volume OR, averaging 30 open heart cases per week.
On-call obligation required for covering weeknights and both weekend days and nights after the orientation process.
Works as a team member in the operating room alongside the rest of the surgical team to provide patient-centered care to high acuity patients.
Ensures the presence and proper arrangement of surgical equipment and supplies in the operating room for each operation.
Assists with cleaning operating room after the operation, ensuring proper routing of equipment and used material.
Organizing and administering individualized, professional patient care to patients within the Operating Room.
Providing nursing care in accordance with the objectives, policies, procedures and protocols of the hospital and AORN, ASPAN and SGNA standards.
Participating in patient teaching; incorporating the needs and behavior of the patient's specific age/cultural groups in planning and providing care.
Participating in quality improvement activities.
After orientation there is a possibility for flexible scheduling, including evening or weekend only schedule.

Job Description:
Position includes recovering patients from anesthesia as well as managing postoperative care, including recovery from general anesthesia, regional anesthesia and IV sedation. We are a 28 bed PACU, providing service to a variety of surgical services, such as general surgery, urology, gynecology, neurosurgery including spine surgery, plastic surgery, thoracic and vascular surgery, cardiac procedures (TAVR), bariatric surgery, colorectal surgery, ENT and OMF surgery. Also included is managing the recovery of GI advanced procedure patients as well as outpatient colonoscopy and endoscopy patients. We are a fast paced unit receiving patients from 40+ operating rooms and we provide service 24/7. ICU experience is preferred although we will consider nurses with ED, Step-down and floor level experience on an individual basis. ACLS certification is required for external candidates as we frequently care for ICU and step-down patients. Nurse/Patient ratio is 2:1. We are looking for candidates with a positive outlook, an open mind and a “can-do” attitude.

• Provide a high degree of professional competence and demonstrate a high level of autonomy in the care of VAD patients, heart failure patients and patients requiring mechanical circulatory support.
• Appropriately coordinate testing/evaluation needed for pre and post implant care.
• Perform assessments of patients with VADs to establish, implement and evaluate individualized plan of care with timely and appropriate clinical advice and interventions through direct patient contact as well as telephone contact.
• Participate as an integral member of the multidisciplinary advanced heart failure team to develop standard operating protocols, clinical practice guidelines and patient and caregiver education.
• Responsible for coordination and implementation of community caregiver education related to mechanical circulatory support. Arrange all appropriate medical therapy and testing with advanced practice provider and physician guidance
• On call responsibility for outpatients on VAD support which requires some 24 hour coverage, weekends and holidays
• This position may be involved in research clinical trials, and professional continuing medical educational programs. The role of this Coordinator will evolve over time to meet the needs of the patients and of the program.

Qualifications:
Bachelor's degree required.
Preferred Minimum 3 years' experience in acute care preferred.
Cardiology, ICU or VAD experience preferred.
RegisteredNurse with a valid CT license required

Job Summary
B7E and 7E Step Down is a fast-paced, highly collaborative teaching unit serving medical patients suffering from an acute illness or injury, or experiencing an exacerbation of a chronic condition affecting one or more body systems. The unit has a strong respiratory focus but treats the full range of medical diagnoses. Step down serves critical care patients with additional interventions including, but not limited to, cardiac drips, insulin drips, Bipap, and chronic vents.
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registerednurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registerednurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.
Qualifications
Current Connecticut RN License.
Graduate of an accredited nursing school
ASN Required (Must obtain a BSN within 6 years of starting in the RN role)
BSN preferred.
BLS certification by the American Heart Association required prior to date of hire.
