    Job Requirements To be fully engaged in providing No Harm / Quality, Customer Experience, and Stewardship by therapy program involving exercise, massage, heat, water, light, and electricity. Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests. Performing interdisciplinary case management and providing appropriate age-specific therapy services. PRIMARY ACCOUNTABILITES 1. Adhere to all Health First, Inc., and Health First Home Care policies and procedures to ensure compliance with organizational standards. 2. Conduct comprehensive assessments, including evaluations of home safety, pain, nutrition, fall risk, and DME needs, to support thorough patient care planning. 3. Develop and implement individualized Plans of Care based on assessment findings and physician orders, ensuring each patient's needs are met. 4. Assist in the orientation of new associates and provide staff education as needed to support team knowledge and competency. 5. Comply with State and Federal regulations and JCAHO standards for Home Care service delivery, upholding high-quality care and compliance. 6. Actively engage in Quality Improvement processes, including peer and self-chart reviews, to enhance the quality of patient care. 7. Regularly evaluate patient outcomes, communicate any progress or changes in condition to the physician, and document changes in the Plan of Care with same-day treatment order submissions. 8. Supervise Home Health Aides at least every 14 days and oversee the Physical Therapy Assistant, ensuring annual competency to maintain care standards. 9. Accept referrals regardless of geographic territory and respond to new referrals within two calendar days to facilitate timely patient care. 10. Provide case management to ensure the Plan of Care is followed in alignment with State, Federal, and JCAHO standards, meeting or exceeding annual Press Ganey Patient Satisfaction goals. 11. Coordinate care delivery and collaborate with multidisciplinary teams to enhance patient outcomes, ensuring all visits and services are appropriately utilized and documented within two days. 12. Maintain productivity standards, utilizing supplies in a cost-effective manner, and inform patients/families of visit schedules in advance, notifying them of any changes to foster clear communication and patient satisfaction. Work Experience MINIMUM QUALIFICATIONS * Education: Graduate of a School of Physical Therapy. * Work Experience: One (1) year of acute care experience. * Licensure: o Current licensure in the State of Florida as a Physical Therapist. o Valid Driver's License and clean Motor Vehicle Record and maintained. * Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained. * Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening. * Skills/Knowledge/Abilities: o Good communication skills. o Ability to interact with other professionals, families, and patients. o Ability to decide concern appropriate therapy and patient care. o Ability to respond appropriately to life threatening situations. o Ability to assess, interpret data and educate patients and families of various ages and levels of understanding. o Ability to work autonomously. PREFERRED QUALIFICATIONS * Work Experience: Previous experience in Home Care.
