We currently have five part time career opportunities available to join the HKNC team as a DirectSupportProfessional in our group home located in Port Washington, NY . The DirectSupportProfessional is responsible for providing support to the residents in the areas of communication, personal management, home management, money management, community skills, vocation, leisure and the use of behavior as communication.
This is an in-person position located out of our group home in Port Washington, NY.
Available Shifts:
16 hours per week, Sunday & Monday 4pm-12am.
16 hours per week. Saturday-Sunday. 4pm-12am
Pay Rate: $22.9440/hour.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provide consistent instruction in the following areas: personal grooming, communication, home management, wardrobe management, community skills, vocation, leisure and behavior support, under guidance of the Program Advisor and Assistant Program Advisor.
Participation in programming which consistently promotes an atmosphere conducive to the growth of all residents and encourages maximum independence and interdependence in all areas of living.
Provides consistent regular checks on all consumers during shift, ensuring safety and well-being.
Assists consumers with safe evacuation in the event of an emergency at the home, work site or in the community. Participates in regularly scheduled drills to prepare for such emergencies.
Accompanies and serves as a guide and/or a facilitator of communication for consumers and professional staff members, when assigned. To accomplish this, it may be required to use personal vehicle on occasion.
Becomes CPR and First Aid certified. Dispenses consumer medications after passing the AMAP course. Must become AMAP certified and maintain this certification as a condition of continued employment in this position.
Required to sleep-in as assigned
REQUIRED EDUCATION and/or EXPERIENCE
High school diploma
Four years of paid experience as a residence/group home aide working with individuals who have intellectual disabilities preferred
One year of experience working with individuals who are DeafBlind, blind, deaf or individuals with multiple disabilities preferred
Must possess a valid New York State Driver's license.
Familiarity with American Sign Language highly preferred.

Responsibilities:
Working with the team to develop and implement an ISP that meets the requirements of OPWDD for each participant in their service delivery area.
The DHS will also assist in the residential home and transport to one of the Lifelong Learning Centers located in either Pleasant Valley, Poughkeepsie, or Saugerties.
The Day Hab Specialist will attend to the educational, physical, social, and emotional needs of residents, while assisting the team with the daily programming, data collection, supervision and safety of residents in an assigned service delivery area
The Day Hab Specialist will work with all Anderson Center for Autism staff to ensure quality resident services are provided and supported at all times.
Requirements:

Must be 21 years or older
Must be capable at all times of initiating or assisting in all components of the crisis intervention of the agency

The goal of the DirectSupportProfessional is to empower and direct our individuals in the Day Habilitation program, assist with record keeping, and perform duties necessary for the overall care and well-being of our individuals in this program. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Supervise and positively interact with the individuals following all habilitation plans.
Work with individuals on their daily living skills.
Provide individuals with community integration opportunities.
Direct individuals in their personal routine; give individuals positive reinforcement as appropriate.
Maintain complete and accurate daily records and communication logs
Maintain the order, safety, and cleanliness of the classroom including the outdoor areas.
Attend staff meetings and/or trainings that are required.
Provide medical oversight including medication administration under the supervision of the nursing staff.
Transport individuals to a day program (to and from)
Interact with parents and professionals from outside Agencies.
Other related duties as assigned.
EDUCATION/EXPERIENCE
High School Diploma or GED equivalent
Experience working with the developmentally disabled
LICENSE/CERTIFICATION:
Valid NYS Driver's License (Required)
AMAP Certification (Preferred)
CPR Certification (Preferred)
SCIP-R Certification (Preferred)
SCHEDULE:
8-hour shift
Monday to Friday (7:30 am - 3:30 pm)
This position requires transporting individuals to and from Brooklyn on HKS Buses. Hours could vary with traffic

Assist people in developing advocacy skills, participating in advocacy efforts, and utilizing personal advocates. • Provide support as needed to meet the emotional, physical, and medical needs of each person supported. • Continually educate people receiving services on required work procedures, facility rules, policies, practices and their rights. • Facilitate person-centered, effective, positive relationships with people receiving services using positive approaches that promote self-determination in all areas of life. • Provide input and recommendations into assessment and planning processes, and development of the individual's plan. • Implement person's plan fully, within the requirements of the service definition, and document as required by the service definition and agency policy. Accurately complete all required documentation, including but not limited to, goal completion, actions toward outcomes, incident reports, timesheets, etc. • Complete daily progress notes and communication log to assure appointments, goals, and interests are met. Assist in maintaining all necessary records, daily attendance, check sheets, production sheets, etc. • Assist people receiving services with the day-to-day operation of their home such as housekeeping, meal preparation, laundry, shopping, lawn care, vehicle cleanliness, and maintenance. • Assist people receiving services with knowledge of emergency procedures and personal safety. Utilize effective judgment when dealing with safety issues, including but not limited to fire safety by conducting drills, inspecting equipment, and practicing safety procedures in the community. • Supervise, educate, and monitor (as needed) people receiving services in work settings. Settings may include community, vocational workshops, day supports, etc. • Maintain a safe working environment for employees and people receiving services by practicing safety procedures in the community and on the job. • Substitute in-house or in the community as demands occurs. Sleepovers at residential settings as needed. • Drive and Travel as required. Arrange for or provide transportation to people receiving services as required. • Cooperate with lead staff in developing a schedule, which maximizes the use of hours to ensure efficient use to time and carries out responsibilities in a manner which best meets the needs of individuals receiving services. • Assist new staff and/or current staff with orientation, mentoring, and training.
• Sleepover in a residential setting may be required. • Attend and actively participate in meetings and training as required.
