* Adult Medical Unit (AMU) is a 33 bed unit and serves stable medical patients with an emphasis on, but not limited to, geriatric patients. The unit also serves a small subset of gynecological surgical patients.
* Cardiology unit is a 35 bed unit that specializes in ALL aspects of cardiac issues
* Extended Medical Care Unit is a 25 bed medical unit specializing in patients with acute care needs that require an extended length of stay.
* General Medical Unit is a 29 bed step down unit caring for a variety of medical patients including oncology, pulmonary, and renal.
* General Surgery Unit (GSU) is a 47 bed unit specializing in general surgery, vascular surgery, and a wide array of surgical subspecialties (thoracic, urological, gynecological, bariatric, etc.).
* Neurovascular Unit (NVU) is a 40 bed medical surgical unit specializing in the care of patients with neurological disorders such as Stroke, Epilepsy, Multiple Sclerosis and other inflammatory disorders of the brain and spinal cord.
* Ortho Surgical Unit (OSU) is a 24 bed medical surgical unit specializing in the care of the patient undergoing orthopedic surgical procedures such as hip fractures and total joint replacements (hip, knee, and shoulder).
Minimum Requirements:

Job Summary
The Opportunity
Mass General Brigham Health Plan is an Assessment Registered Nurse (RN) to perform initial and ongoing face-to-face, virtual, or telephonic comprehensive assessments and evaluations. The Assessment RN is responsible for ensuring the accuracy and quality of all assessments, as well as the timely submission of documentation to MassHealth.
The Assessment RN completes timely, accurate, and high-quality assessments. They utilize tools such as the Minimum Data Set (MDS), Comprehensive, and Functional Assessments to gather relevant data, enabling them to identify enrollees' concerns and unmet needs.
The Assessment RN collaborates closely with the enrollee's Interdisciplinary Care Team and other departments to facilitate timely interventions aimed at improving outcomes for members.
Essential Functions
* Conducts initial and ongoing comprehensive face-to-face assessments and evaluations for members. Outreach may occur via telephone, electronic communication, or in-person visits, depending on the enrollee's preferences and current needs.
* Travels to enrollees' homes, community locations, and residential sites to complete comprehensive assessments.
* Ability to independently manage scheduling, including coordinating face-to-face appointments with members as needed.
* Collaborates closely with primary care providers, the interdisciplinary team, and the community care management team to ensure coordinated care.
* Completes medication reconciliation
* Provides acute and chronic disease management education
* Provides analyzed assessment data to the Interdisciplinary Care Team (ICT) to support the development of a more effective individualized care plan (ICP) for the enrollee.

GENERAL SUMMARY/OVERVIEW STATEMENT:
Come join the #1 stroke program in Michigan! The Neurocritical Care Advanced Practitioner position is an employed, full-time position that includes varied shifts and weekends. Shifts include a rotation of days (8A - 9P), nights (8P - 9A), and swing (11A - midnight). In this role, the candidate will be working collaboratively with multiple providers including Trauma Surgeons, Medical Intensivists, Neurologists, Neurosurgeons, and Hospitalists.
The Neurocritical Care APP supports critically ill patients with neurological or neurosurgical conditions or who are at risk of developing neurological complications. This position also participates in the Stroke Call down process, provides input in performance improvement and nursing education. The APP will perform patient assessments, medical histories, physical exams, and plan and implement plans of care in collaboration with Neurointensivist or appropriate attending. The Neurocritical Care APP will work as a collaborative member of the care team to enhance continuity of care, facilitate and coordinate a comprehensive plan of care, participate and support ongoing research studies in the ICUs, and offer their expertise to nursing and collaborating medical teams.
QUALIFICATIONS:
* Current Michigan licensure/certification as a Nurse Practitioner or Physician Assistant granted by a Board recognized certifying organization.
* 1-3 years NP/PA experience strongly preferred
* New graduates will be considered with strong critical care and/or neuroscience nursing experience

The RN will understand the documentation differences and the specific care required for each individual swing bed patient.

Bronson is offering a Surgical Nurse Internship for licensed Registered Nurses seeking a new opportunity in surgical care (seeking candidates with 2 years of clinical nursing experience). This program is designed to prepare nurses for practice in the fast-paced and constantly evolving field of surgical nursing. Interns will take part in an extended orientation that includes classroom instruction, hands-on training in both foundational and advanced surgical skills, clinical experience, and the development of critical thinking. The curriculum follows best practices and current standards in surgical nursing. All nurses providing direct patient care are expected to demonstrate competencies specific to the surgical patient population.
As an OR RN Intern, you will participate in a full-time, day-shift orientation lasting approximately 6 to 8 months. After successful completion of orientation, you will transition into an available OR Registered Nurse position-please note that this transition may involve placement on any shift, depending on department needs (current needs include night shift).
