GENERAL SUMMARY: The pharmacist assumes responsibility for the day-to-day comprehensive, patient-oriented pharmacy services provided on his/her shift in each area of the department. The pharmacist works under the general direction of the area's administrative or clinical coordinator, and provides direction and supervision for supportive staff assigned to his/her shift. The pharmacist must be able to exercise sound management and clinical judgment in making appropriate decisions in the absence of coordinators or more senior staff. He/she assists coordinators in identifying problems which exist in his/her assigned area of practice and cooperates in the resolution of these problems. The pharmacist participates in training pharmacy interns and externs in each of the pharmacy service areas.
EDUCATION/EXPERIENCE REQUIRED: Must hold a degree from an accredited college of pharmacy; residency in hospital pharmacy strongly preferred. Should demonstrate acumen in verbal and written communication skills which enable him/her to contribute effectively in the dissemination of drug information to all levels of health care personnel in the institution. Must possess a thorough understanding of the pathophysiology and pharmacotherapy of disease states and have the application skills needed to permit proper integration of these in monitoring drug therapy. Must have basic abilities for participating in the various teaching programs including instruction and supervision of undergraduate pharmacy students, interns, and pharmacy technician trainees. Should have the skills necessary for providing informal and formal instruction of students and other health care providers on drug therapy and inpatient operational activities. Must demonstrate the ability to work with others in developing improved services for patients.
CERTIFICATIONS/LICENSURES REQUIRED: Must be licensed or eligible for licensure in the State of Michigan. If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacist position.

Position Summary The Staff Pharmacist provides pharmaceutical care to PACE participants with the highest standards of ethics and professionalism, ensuring safe, effective medication use and promoting optimal health outcomes. This hybrid role includes remote work as well as on-site responsibilities across four PACE clinics and contracted Skilled Nursing Facilities (SNFs) and Adult Foster Care (AFC) homes in Calhoun and Kalamazoo counties. Key Responsibilities Clinical Pharmacy Care
Assess, develop, and implement individualized pharmaceutical care plans.
Perform initial, semi-annual, interval, and as-needed pharmacist assessments.
Review immunization histories and order indicated vaccinations.
Provide in-person medication counseling and education for participants with complex chronic conditions (e.g., COPD, diabetes, heart failure).
Administer vaccines to participants and staff.
Medication Management & Coordination
Complete medication reconciliation and medication regimen reviews at contracted SNF and AFC facilities and remotely via electronic medical records.
Assist with medication and immunization history collection for new enrollees.
Monitor clinic pharmaceutical supplies, including dispensing levels and expiration dates.
Serve as a liaison between Senior Care Partners PACE and pharmacy partners to optimize medication preparation, dispensing, and safety.
Safety & Quality
Conduct medication incident investigations and contribute to corrective action plans.
Provide medication-related education to SNF and AFC facility staff as requested.
Maintain compliance with all PACE policies, government regulations, and professional standards.
Leadership & Collaboration
Provide non-direct oversight of staff involved in medication distribution.
Collaborate with interdisciplinary team members, families, caregivers, and external partners using a patient-family centered care approach.
Qualifications
Doctor of Pharmacy (PharmD) from an accredited school.
Current, active, unencumbered Pharmacist license in Michigan.
Current Michigan Controlled Substance license.
Current immunization administration certification or willingness to obtain.
Minimum one year of experience working with frail, elderly, or long-term care populations.
Reliable transportation, valid Michigan driver's license, and valid vehicle insurance.
Current CPR certification or ability to obtain (provided by SCP PACE).
Ability to read, write, and speak English fluently.
Proficient with computers and electronic medical records.
Ability to lift/push/pull at least 20 lbs and meet physical requirements of the role.

What you will do
Properly use dispensing and barcoding technology to ensure accurate medication selection, stocking, and order fulfillment for all patient orders. Ensure accuracy of prescription and patient data entered in various systems. Review all relevant clinical information and clarifies with pharmacist/prescriber as appropriate. Works with payers to resolve insurance-related issues, assists with financial assistance programs as needed.
Deliver excellent customer service to health care providers and patients, by greeting them in person and by phone, answering questions and requests with pharmacist supervision, and responding to messages in the Epic In Basket and Epic Secure Chat.
Provide support to pharmacists by answering telephone calls and responding to messages. Will include research of basic clinical questions and drug information and drug compatibility questions.
Will undertake clinical functions under pharmacist supervision up to level of education including but not limited to the offering and provision of counseling to patients on medications and administration of vaccinations.
Manipulate and dispense hazardous and non-hazardous medications per department procedure.
Complies with HIPAA/HITECH regulations.
Assist in the planning, implementation, and process management of technological initiatives and pharmacy workflow improvements.

Dispenses medications, related supplies, and provides all clinical pharmacy services.

Pharmacist - 40 hrs/wk - Henry Ford Hospital Detroit

GENERAL SUMMARY: The pharmacist assumes responsibility for the day-to-day comprehensive, patient-oriented pharmacy services provided on his/her shift in each area of the department. The pharmacist works under the general direction of the area's administrative or clinical coordinator, and provides direction and supervision for supportive staff assigned to his/her shift. The pharmacist must be able to exercise sound management and clinical judgment in making appropriate decisions in the absence of coordinators or more senior staff. He/she assists coordinators in identifying problems which exist in his/her assigned area of practice and cooperates in the resolution of these problems. The pharmacist participates in training pharmacy interns and externs in each of the pharmacy service areas.
EDUCATION/EXPERIENCE REQUIRED: Must hold a degree from an accredited college of pharmacy; residency in hospital pharmacy strongly preferred. Should demonstrate acumen in verbal and written communication skills which enable him/her to contribute effectively in the dissemination of drug information to all levels of health care personnel in the institution. Must possess a thorough understanding of the pathophysiology and pharmacotherapy of disease states and have the application skills needed to permit proper integration of these in monitoring drug therapy. Must have basic abilities for participating in the various teaching programs including instruction and supervision of undergraduate pharmacy students, interns, and pharmacy technician trainees. Should have the skills necessary for providing informal and formal instruction of students and other health care providers on drug therapy and inpatient operational activities. Must demonstrate the ability to work with others in developing improved services for patients.
CERTIFICATIONS/LICENSURES REQUIRED: Must be licensed or eligible for licensure in the State of Michigan. If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacist position.
