Essential Job Functions Perform delegated nursing functions within the scope of practice of an LPN under OPWDD guidelines and as per established procedures, policies, and standards as observed by the RN. Assist in data collection on individuals within assigned caseload including, but not limited to, lab results, diagnostic testing, BM data. Contribute to the development of individual's plans and assist in identifying areas of need. Adhere to physician orders accurately. Assist in observation and evaluation of nursing care administered by Direct Support Professionals. Ensure that all required documentation is completed /processed appropriately and in a timely manner Assist in maintaining effective communication with staff, physicians and Registered Nurse. Communicate information regarding individual's medical status and overall needs to the Registered Nurse, physicians and pertinent team members. Review physician summary forms to ensure information is complete, accurate, and documented in the individual's medical record, including any indicated test results and recommendations for follow up as well as resolution of a medical issue. Work in partnership with the program management to ensure that provider visits are followed upon and staff are securing follow up appointments as indicated. Ensure Emergency Room instructions and/or treatment is followed up and documented accordingly. Ensure staff are educated as needed on proper follow up and treatment. Emergency Room Documentation should be obtained by staff upon discharge. Ensure compliance with regulatory standards. Observe agency policy and procedures regarding facility security and proper identification of authorized personnel. Duties and Responsibilities Adhere to a flexible schedule that reflects, and is based on, the needs of the program. If warranted, change in program location and oversight may occur. Ensure adequate supervision is provided to the individuals based on Individualized Service Plans, Behavior Support Plans, and Nursing Care Plans. Ensure communication to RN, management, and direct support professionals any change in individual's status and/or needs observed during shift. Core Responsibilities * Ensure that documentation is accurate, contemporaneous, and effectively communicates all information required to be regulatory compliant. * Promote and oversee the safety and well-being of the children through use of written, verbal procedures, safety equipment and awareness of any potential dangers. * Attend to personal, self-care and other program needs that the children cannot independently accomplish. * Keep the nursing and administrative team advised of medical and dietary concerns. * Provide support and consultation and initiate peer support among program participants. * Promote student input in the development of program activities. * Oversee the safety and well-being of the children. * Oversee and upkeep the maintenance of equipment and supplies. * Participate in activities as part of the treatment team that may include meetings, trainings, and committees.
