Essential Duties and Responsibilities:
Possesses knowledge of nursing principles (geriatrics, senior living, and long-term care), professional standards of nursing practice and ability to apply to resident specific circumstances
Assume self-responsibility for all direct patient care activities as indicated by the DON within the scope of the Nurse Practice Act, DHS 83 and with HSL policies and procedures
Possesses knowledge of Wisconsin state regulations for CBRFs and/or RCACs
Provide direct patient care according to the ISP and physician orders
Provide appropriate information to assist the DON in the preparation and maintenance of an effective ISP
Document detailed notes in residents' electronic medical records that reflect the therapeutic plan and specific aspects of the ISP
Oversee the 24-hour report sheet. Make routine checks on any resident who is acutely ill or has any change in condition
Attendance/punctuality: must be able to work 8- to 12-hour days
Education/Qualifications:
Valid State of Wisconsin Nursing License or Licensed Practical Nurse/Licensed Vocational Nurse
Possess knowledge of nursing principles (geriatrics, senior living, and long-term care) and professional standards of nursing practice and have the ability to apply them to resident specific circumstances
Possess knowledge of Wisconsin state regulations for CBRFs and/or RCACs
Must be in good standing on the State Board of Regulation and Licensing
Ability to speak, write, and read English
Must be flexible and able to handle multiple variations in a daily schedule
Must have basic computer knowledge, including EHR software system

Patient Assessment: Evaluate patients' health needs including use of suction machine, pulse oximeter. Maintain a high level of awareness regarding the health and medical needs of the patient supported.
Ventilator Management: Monitor, operate, and troubleshoot ventilators, ensuring proper function and patient safety.

Qualifications:
Licensure/Certification: Current/valid nursing license issued by the WI State Board of Nursing and basic life support certification. Advanced certification in ventilator and trach care is preferred.

Skills: Strong clinical skills, excellent communication, and patient education capabilities. Ability to write and correspond in a legible manner.

The LPN Navigator will be responsible for the coordination of care for all clinic patients and will interact across the enterprise to facilitate necessary services as determined by the Provider. This individual will also utilize broad-based skills in basic patient care, ancillary services and business office practices while aiding providers in accordance with the AAMA and PNHS standards and policies.
The TRIA LPN Clinical Navigator will oversee daily clinic flow through Check-in and Co-pay collection processes, standard rooming, history, and evaluation, assist with documentation, clinical procedures, after visit education, scheduling of follow-up appointments as well as facilitating overall patient care needs and coordination of care across the enterprise for our patients and families.
This role will also be responsible for ensuring medical orders/care are delivered as delegated by providers and registered nurses to support patient care. Quality of care and exceptional customer service are always delivered to our patients and families.

Required Qualifications:
* Graduate from an accredited school of practical nursing
* Must maintain skill in performing cardiopulmonary resuscitation (CPR).
* LPN must maintain a current state license of practical nursing (MN & WI).
