Job Title: Certified Nursing Assistant (CNA), Long Term Care (Per Diem, Varied) Department: Transitional Care Unit Hours Per Week: Per Diem Schedule: Per Diem - As Needed Certified Nursing Assistants are responsible for providing basic patient care under the direct supervision of the nurses responsible for patient care. RESPONSIBILITIES: Provide or assist residents with bathing, feeding, toileting, repositioning, exercises, and other Activities of Daily Living (ADLs) according to individualized care plans. Support residents with nourishment, fluids, and intake recording; ensure timely response to resident requests. Maintain accurate documentation of care, food/fluid intake, and shift reports while following policies and protocols. Assist in maintaining resident equipment and environment in a clean, safe, and functional manner; report and follow processes for repairs. Job Title: Certified Nursing Assistant (CNA), Long Term Care (Part-time, Evenings) Department: Long Term Care Hours Per Week: 32 Schedule: Evenings, 3pm to 11:30 pm, Every other weekend and a holiday rotation SUMMARY As a Certified Nursing Assistant, you will provide high quality, individualized patient care in collaboration with the nursing team. Your ability to take initiative while always assisting and maintaining patient care will be critical in the success of the entire team. RESPONSIBILITIES Patient Care & Service. Provide and/or assist the resident in activities necessary as per individualized care planning; transport to and from activities as needed; provide leisure activities as needed; perform evaluation and reporting; provide resident and family with emotional support Safety. Assist in maintaining a clean and safe resident environment Communication & Documentation.
