This is a full-time position with 35-hour workweek and flexible/hybrid schedule.
Hope For Youth has an exciting and rare Therapist position opening in its PINS Diversion program. The Nassau County PINS (Person In Need of Supervision) Diversion program is designed to divert youth who are at risk, from PINS petitions that require court involvement. PINS Diversion services families with children up to the age of 18 in Nassau County. We are also honored to announce that the Hauppauge Industrial Association of Long Island (HIA-LI), an advocate for its member organizations since 1978, has recognized HFY as their Not-For Profit Business Award honoree for Excellence in 2025!
Families that apply for diversion services have issues including but not limited to school truancy, gang involvement, runaway behaviors, mental health and substance abuse issues.
The PINS diversion program provides short term intensive in-home, family-centered interventions with an overarching goal that will prevent out of home placements, teach problem solving skills to the families, and linkage to a community-based system of care.
A unique component to the diversion program is the incorporation of Multidimensional Family Therapy (MDFT). MDFT is an evidence based program designed to meet the needs of youth struggling with mental health and substance abuse issues. Paid training in this model provided to the hired Therapist during normal work hours at no cost.
Key Responsibilities:
MDFT Therapists function within Agency's policies and procedures as outlined in the Employee Manual.
MDFT Therapists function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
MDFT Therapists provide support to the clients and families-i.e., the knowledge, skills and tools necessary in order for them to maintain the child in their home.
MDFT Therapists achieve successful outcomes within the guidelines outlined in NYS, County and HFY policy and procedure (shared).
MDFT Therapists perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
How MDFT Therapists will generally spend time in this job:
In accordance with agency policy and procedures, the MDFT Therapists will be a Licensed Therapist (LMSW, LCSW, LMHC, LMFT) and will develop a comprehensive clinical treatment plan that includes counseling, mental health and substance abuse treatment services for youth with a High acuity level for behavior and trauma as determined by the CANS-NY. The MDFT Therapists will:
MDFT Therapists provide individual clinical services to a caseload of up to 12 clients at any time and the program will service up to 50 clinical cases total annually.
The MDFT Therapist training program will be conducted within a 3- to 6-month time frame. A full course can range between 30 and 60 sessions over 6 months, with clients seen 1 to 4 times a week as needed.
MDFT Therapists will administer a mixture of home, community or office-based interventions.
MDFT Therapists conduct collateral contacts as appropriate in school or community-based locations
MDFT Therapists prepare psychosocial assessments, treatment plans, progress notes to document all contacts, interdisciplinary team conference documentation and other such necessary case related information.
Primary physical requirements of MDFT Therapists: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, ongoing access to reliable vehicle and driving own vehicle for work. A vehicle inspection may be conducted during the interview.
Qualifications:
License Required (LMSW, LCSW, LMHC, LMFT)
Spanish and English-speaking required
Experience is preferred, and Hope For Youth will train in the MDFT model.
Valid NYS Driver's License in good standing

This opportunity is being offered at a salary set by the county that is based upon a 35-hour work week involving a combination of remote, office and field work. The hours are flexible around schedules based upon the needs of the clients.

POSITION OVERVIEW:
CFTSS Clinical services are provided to children/youth and are intended to help identify and prevent the progression of behavioral health needs through early identification and intervention. PURPOSE:
The purpose of the Brief Strategic Family Therapy (BSFT) Therapist in the Preventive Service Department is to implement the BSFT model with family with child(ren) between the ages of 6 to 17 years old who present with various behavioral problems, including school truancy, substance use, conduct problems, and delinquency. The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. All services are provided in a family's home and community.
POSITION OVERVIEW:
The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. They collaborate with other service providers to ensure a comprehensive approach to the family's care. The therapist also plays a crucial role in monitoring the family's progress, adjusting treatment plans as necessary, and working towards successful case closure.

Program Description:
Graham Windham's Community Mental Health & Wellness Clinic is an Article 31 OMH program that offers a variety of evidence-based treatment for children, youth, young adults, and their families from the age of 5-25 years of age, based on our comprehensive psychiatric and biopsychosocial evaluations. We offer individual therapy, family therapy, group therapy, psychological and psychiatric evaluations, psychopharmacology management, and support services. Our Harlem clinic offers walk-in hours for immediate mental health assessments. Each treatment plan is created in collaboration with the client based on the needs of the individual client. We also offer bilingual services in Spanish to better support the needs of our communities.
Job Responsibilities:
Provide creative arts therapy, psychotherapy and counseling. Encourage an environment that fosters and enhances creativity, socialization, and promotes physical and mental health.
Conduct goal-oriented structured therapeutic sessions that utilize creative arts techniques and principles for an assigned caseload in music, art, and/or dance.
Recognize, address and utilize therapeutic creative arts interventions to facilitate mastery, coping skills, and creative expression for children and youth who have experienced trauma.
Assist in operating, maintaining and purchasing equipment and other materials/resources necessary for creative art therapy sessions, i.e., presentation equipment, supplies, etc.
Provide, implement and coordinate comprehensive initial and ongoing assessments and treatment plan for new intake clients and then ongoing for clients on caseload.
Collaborate with client, collaterals and their multi-disciplinary clinical treatment team (i.e. psychiatrist, nurse practitioner, foster care team, care management, care coordination, etc.) in developing, reviewing and modifying the treatment plan and as issues arise. Monitor and evaluate clients' progress and modify treatment plans as needed.
Provide engagement and rapport with the purpose of fostering an individual's commitment to engage in a therapeutic relationship and attend therapy regularly. Explore motivation, resistance to therapy and attendance as needed.
Maintain electronic records and documentation according to the standards and timeframes established by OMH, Medicaid regulations and Graham policies and procedures.
Maintain expected levels of productivity and provide quality care as defined by regulatory agencies and Graham.
Manage up reportable incidents as well as complete necessary documentation.
Coordinate the discharge planning process. Identify the resources and supports needed for transition to another program and make necessary referrals, linkages for treatment, rehabilitation and support services.
Be actively self-reflective & self-aware of how clinical and administrative practice affects clients, families, and the communities we serve. Be open and willing to receive constructive feedback.

A program therapist in our Family Transitions outpatient program provides specialized services to local youth and their families who are faced with challenges related to sexual abuse. Children referred for treatment may have displayed problematic or harmful sexual behaviors and/or have endured sexual victimization. Using a family systems approach, this position places a strong emphasis on the involvement and participation of parents/families and will require skills aimed at developing a strong partnership to achieve this goal. Responsibilities:
Carry a caseload of up to 15 families through a collaborative contract with Onondaga County Department of Social Services along with the option for additional cases assigned through Child and Family Treatment and Support Services (CFTSS) for families with NYS Medicaid and/or contracts with outside counties for youth in need of specialized sexual abuse services. (Identified clients may include, child, parent, family, or a combination of such)
Participate in training relevant to specialized sexual abuse services and working with children, adolescents and families, along with training to learn about the role and implementation of CFTSS as needed.
Establish and maintain therapeutic relationships with youth and parents/caregivers.
Provide individual, parent, and family therapy.
Co-facilitate psycho-educational groups with children/adolescents and/or parents/caregivers.
Assist in providing specialized sexual abuse training to collaborative partners.

Assist in the supervision of Graduate Interns, as needed.
Develop and implement therapeutic intervention and treatment plans.

Travel and attend meetings in the community and client homes to carry out responsibilities for home visits, service plan meetings, and other meetings.

Qualifications
Master's degree in social work, Counseling, Marriage and Family Therapy or other related mental health field from an accredited institution.
Additional Eligibility Qualifications:
