Spring students are in clinic between the dates of 12/4/25 and 5/28/26. For clinical dates before 12/4/25, the deadline has passed. Clinical Rotation Description:
This clinical rotation prepares Family Nurse Practitioner students for ambulatory care settings. MinuteClinic accomplishes this goal by not only meeting the academic standards of the school, but by providing students with a well-rounded experience that includes specialized online training, patient interaction, visibility to our EMR and the opportunity to work in a new and exciting model of health care. Our preceptors have a passion for teaching and sharing their knowledge. All of our providers have exceptional clinical skills and are experts in the operational skills required to run their own clinic. MinuteClinic?s student clinical program offers the most unique experience available, because you are doing more than just enhancing your clinical skills: Practical education including clinic operations, public relations, marketing, insurance procedures, and use of evidence based guidelines. Students will gain exposure in an ambulatory care setting and develop an understanding of how these clinics fit into the current structure of health care delivery. You?ll develop an understanding of how our corporate office relates to and supports our clinical practice. Most MinuteClinic locations are open 7 days a week and our providers? schedules are compressed, so clinical days can vary from week-to-week. Clinic locations may also vary depending on schedule of your assigned preceptor.
Required Qualifications :
To participate, you must be enrolled in a Family Nurse Practitioner program that already has an executed Affiliation Agreement with MinuteClinic. FNP students must have an RN license in the state in which they will be precepting. We will also verify with your program faculty that you are eligible for a clinical rotation at MinuteClinic.

Position Purpose
Responsible for providing a supportive home-like environment for the Villagers residing in his/her residence. Will participate in the necessary skills training for Villagers to meet their goals through the AFLS assessment process.

Completes bi-weekly hygiene checklists to ensure proper hygiene with all ADL needs.

Trains and assists Villagers on appropriate social skills, problem solving, etc.
Completes and documents for each villager all care tracking tasks in Electronic Medical Record.
Assists Villagers with ambulating across campus depending on villagers physical abilities/needs.
Is knowledgeable of and adheres to villager safety/behavior plans.
Provides watchful oversight of Villagers well-being throughout shift.

Position Purpose
Responsible for providing a supportive home-like environment for the Villagers residing in his/her residence. Provides skills training to promote independence including, but not limited to; maintaining a safe and clean room, laundry skills, personal hygiene, and interpersonal relationships with their housemates. Will participate in the necessary skills training for Villagers to meet their goals through the AFLS assessment process.
Required Qualifications
1 year of work experience and/or training or equivalent combination of education and experience preferred.
High school diploma or GED required
Must be able to past a literacy test
Must be at least 18 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.
Major Duties and Responsibilities
Assists the LPN Nurse Manager as needed to include health maintenance activities.
Is knowledgeable on the Medication Error/Progressive Disciplinary process, related to medication errors.
Transport or accompany villagers to ER/Urgent Care as needed.
Completes bi-weekly hygiene checklists to ensure proper hygiene with all ADL needs.
Maintains and adheres to the laundry and room cleaning schedule for each villager, and provides assistance/training to those who are unable to complete this task on their own.
Ensures villagers are appropriately dressed for the weather, before leaving the residence.
Trains and assists Villagers on appropriate social skills, problem solving, etc.
Completes and documents for each villager all care tracking tasks in Electronic Medical Record.
Assists Villagers with ambulating across campus depending on villagers physical abilities/needs.
Is knowledgeable of and adheres to villager safety/behavior plans.
Provides watchful oversight of Villagers well-being throughout shift.
Immediately reports incidents of inappropriate behavior or accidents involving villagers to the appropriate staff.
Completes weekly inventory of villagers ADL Supplies, clothing, and snacks.
Communicates and documents villager concerns involving villager behaviors or needs to the villager's case manager.
Assists villagers to create grocery lists with healthy food choices, per specific diets to shop in the community.
Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers.

The most essential role of the Residential DirectSupportProfessional (RDSP) is to provide the best life possible for individuals with Intellectual/Developmental disorder and individuals that are dually diagnosed with S.M.I. To provide skill building and support to promote maximum independence for each individual providing guidance and safety support in all areas ensuring that they are active and valued members of their community. This position is assigned and scheduled to any location in the Wythe, Bland or Smyth counties service area to meet a location's daily staffing needs.
ESSENTIAL FUNCTIONS:
Provide support for medical needs which includes scheduling and accompany individuals for all routine well and sick-care appointments, help the individual to explain their symptoms when sick and to understand and follow doctor's orders. Residential DirectSupportProfessionals (RDSPs) assume the responsibility for following all doctors order in providing care for individuals in service.
All RDSPs levels are responsible to maintain current certification in First Aid/CPR/AED. Provide input into and/or actively develop a person-centered service plan to support individuals served in the community. DSP IIs and IIIs are required to successfully complete the Board of Nursing approved 40-hour initial medication class and maintain an annual recertification. Additionally, DSP IIs and IIIs may be required to complete specialty training in areas of; Gastrostomy Tubes, Insulin, etc., as appropriate to the individuals residing in the home. Moreover, RDSP IIs and IIIs are responsible to administer medications as prescribed and monitor for any potential side-effects, reporting observed information to the prescribing physician.
Appropriately intervene in crisis situations and emergencies and make referrals to and seek consultation from other sources as needed. Provide a safe home environment and complete required emergency drills as required by licensing and/or other governing entities.
Provide the best life possible for all individuals served in each aspect of their lives by providing skill buildings and support to promote maximum independence for each individual according to their specific needs and in accordance with their Person Centered Plan.
Provide opportunities for each individual to explore integrative activities that are familiar as well as providing new experiences to expand their knowledge of the world in which they live promoting their development of relationships and natural supports. Provide transportation, secure funds, and facilitating positive communication by teaching skills to further their independence and to provide safety supports.
Maintain all required trainings are completed within compliance with agency policy. RDSPs are responsible for accurate, informative documentation reflecting the services and supports delivered to the individuals served. Attend scheduled meetings, assessments, and provide information that informs the development of plans for the individuals served.
RDSPs must be able to work independently and as part of a team to actively participate in daily activities to provide a loving, supportive home environment for the individuals served. RDSPs must be able to work without direct supervision.
OTHER DUTIES:
Upon hire, staff are assigned a home location within the division and assigned a supervisor. However, staff may be required to work at other group homes if coverage is warranted and at the discretion of the Group Home Coordinator or Division Director.
Direct Care Staff are subject to working varying shifts which include but are not limited to holidays, weekends, and overnights. Shifts will be determined by the need of specific locations and may vary at times.
QUALIFICATIONS:

The DirectSupportProfessional located at the Geriatric Transition Center will provide direct services to individuals with mental, behavioral, and substance abuse issues within a Transition Center unit that provides residential stabilization as well as medically-managed services. Provides direct care support and instruction, as needed, and assist treatment team members in implementing individualized recovery plans. Completes household duties to ensure cleanliness, safety, and compliance with licensure regulations.
ESSENTIAL FUNCTIONS:
Provide individualized and group services to adults meeting criteria for the Geriatric Transition Center in order to promote recovery and meet the program goal of reducing reliance on more restrictive inpatient services. Assist in implementing Individualized Recovery Plans, including support, assistance with independent living skills, social skills, personal hygiene, household skills, cooking, nutrition and meal planning education and assistance, and transportation to recreational groups and activities.
Ensure a clean and safe environment by performing such task as fire drills, fire extinguisher checks, house cleaning, grocery shopping, meal planning, and preparation.
Participate in the activities of a self-directed team, and as needed, assigned, or scheduled, to attend meetings, and trainings events.
Maintain, administer, and document medications in accordance with written physician's orders and regulatory standards, and maintain current certification in the Agency's approved medication training program. Observe persons for potential side effects of medications, and notify appropriate staff as necessary. Administer basic first aid as needed and maintain current certification in CPR, First Aid, and other required training.

Residential DirectSupportProfessionals (RDSPs) assume the responsibility for following all doctors order in providing care for individuals in service.
All RDSPs levels are responsible to maintain current certification in First Aid/CPR/AED. Provide input into and/or actively develop a person-centered service plan to support individuals served in the community. DSP IIs and IIIs are required to successfully complete the Board of Nursing approved 40-hour initial medication class and maintain an annual recertification. Additionally, DSP IIs and IIIs may be required to complete specialty training in areas of; Gastrostomy Tubes, Insulin, etc., as appropriate to the individuals residing in the home. Moreover, RDSP IIs and IIIs are responsible to administer medications as prescribed and monitor for any potential side-effects, reporting observed information to the prescribing physician.
Appropriately intervene in crisis situations and emergencies and make referrals to and seek consultation from other sources as needed. Provide a safe home environment and complete required emergency drills as required by licensing and/or other governing entities.
Provide the best life possible for all individuals served in each aspect of their lives by providing skill buildings and support to promote maximum independence for each individual according to their specific needs and in accordance with their Person Centered Plan.
Provide opportunities for each individual to explore integrative activities that are familiar as well as providing new experiences to expand their knowledge of the world in which they live promoting their development of relationships and natural supports. Provide transportation, secure funds, and facilitating positive communication by teaching skills to further their independence and to provide safety supports.
Maintain all required trainings are completed within compliance with agency policy. RDSPs are responsible for accurate, informative documentation reflecting the services and supports delivered to the individuals served. Attend scheduled meetings, assessments, and provide information that informs the development of plans for the individuals served.
RDSPs must be able to work independently and as part of a team to actively participate in daily activities to provide a loving, supportive home environment for the individuals served. RDSPs must be able to work without direct supervision.
OTHER DUTIES:
Upon hire, staff are assigned a home location within the division and assigned a supervisor. However, staff may be required to work at other group homes if coverage is warranted and at the discretion of the Group Home Coordinator or Division Director.
Direct Care Staff are subject to working varying shifts which include but are not limited to holidays, weekends, and overnights. Shifts will be determined by the need of specific locations and may vary at times.
