POSITION SUMMARY:
Reporting to the In-Patient Facility Manager, this position is responsible for carrying out the duties assigned in order to increase the patient's level of comfort and to maintain personal hygiene and a safe healthy environment for the patient. Although the HomeHealthAide reports to a primary manager, HEB expects the HomeHealthAide may work in more than one program or department, and assignments are given to best meet the needs of our patients.
ESSENTIAL FUNCTIONS:
Responsibilities of the HomeHealthAide include, but are not limited to, the following:
Performing personal care needs after given a patient's assignment and attending to his/her requests. If unable to perform a certain task, communicates with Case Manager.
Providing patients and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails).
Following the plan of care as written by the RN.
Demonstrating clinical care as defined in the HomeHealthAide scope of practice.
Assessing and reporting on the patient's condition and significant changes to the RN. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
Recognizing and reporting emergencies.
Keeping the patient's living area clean and orderly, as assigned.
Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
Participating in quality assessment performance improvement teams and activities.
Participating in IDT meetings.
Respectful of patient and family/caregiver environment and patient's personal needs.
Maintaining inventory of disposable supplies.
Maintaining cleanliness and infection control standards.
Performing other duties as assigned.
POSITION REQUIREMENTS:
Must be a compassionate team player who displays confidence and the ability to work independently with little or no supervision.
Must demonstrate initiative, dependability and good judgment.
Must be able to work collaboratively as a team with strong communication skills and be flexible with work assignments and professional when dealing with clients and staff.

Must be able to understand and carry out directions and instructions and maintain patient documents.
Must be physically able to perform duties as assigned and have reliable transportation.
EXPERIENCE AND EDUCATION:
Must complete the HomeHealthAide competency test with a passing score of 75% or better.
Must have completed an official HomeHealthAide training course and have a HomeHealthAide Certificate issued by the State Department of Public Health.
If there has been a 24-month lapse in furnishing services, the individual must complete another state approved HomeHealthAide program before providing services.
Valid CPR certification.

Be assigned by VITAS to any institutional setting.
Change sterile dressings.
Irrigate body cavities (such as an enema).
Irrigate a colostomy or wound.

The VITAS HomeHealthAide (HHA) is a non-exempt employee, who is trained to provide personal care and services to hospice patients to augment care given by family members or other caregivers.
ASSIGNED JOB RESPONSIBILITIES
Performs only those
personal care
activities contained in a written assignment. HomeHealthAide (HHA) assignments may include:
HomeHealthAide (HHA) helps the patient to maintain good personal hygiene.
HomeHealthAide (HHA) plans and prepare meals. Feeds or assist with feeding the patient when needed.
HomeHealthAide (HHA) assists the patient ambulation.
HomeHealthAide (HHA) assists with certain treatments as ordered by the physician, and taught and supervised by the nurse for a specific patient including: assist with the change of the colostomy bag; assist with the use of a walker or wheelchair; reinforcement of a dressing; assist with prescribed range of motion exercises; assist with prescribed ice cap or collar; measure intake and output of fluids; and assist with medications that are ordinarily self-administered.
HomeHealthAide (HHA) reads and records temperature, pulse and respiration.
OTHER RESPONSIBILITIES FOR THE HOMEHEALTHAIDE (HHA) NOT INCLUDED IN A WRITTEN ASSIGNMENT MAY INCLUDE:
HomeHealthAide (HHA) assists in maintaining a safe and healthy environment.
HomeHealthAide (HHA) promotes the patient's mental alertness through involvement in activities of interest.
HomeHealthAide (HHA) provides simple emotional and psychological support to the patient, family, and other caregivers.
HomeHealthAide (HHA) reports any changes in the patient's mental or physical condition or home situation to the VITAS Nurse and members of the interdisciplinary team.
HomeHealthAide (HHA) teaches basic personal care to the primary caregiver.
HomeHealthAide (HHA) practices basic infection control measures.
HomeHealthAide (HHA) observes, reports, and documents patient status and the care and activities provided.
HomeHealthAide (HHA) completes at least 12 hours of in-service training per calendar year.
HomeHealthAide (HHA) handles other tasks as assigned.
THE HOMEHEALTHAIDE (HHA) MAY NOT:
Be assigned by VITAS to any institutional setting.
Change sterile dressings.
Irrigate body cavities (such as an enema).
Irrigate a colostomy or wound.
Perform a gastric lavage or gavage.
Catheterize a patient.
Administer medications.
Apply heat by any method.
Care for a tracheotomy tube.
Suction a patient.
Provide any personal care that has not been assigned by the VITAS nurse or included in the plan of care.
QUALIFICATIONS
High School graduate or equivalent preferred.
Meets personnel qualifications of a HomeHealthAide as specified in 42 C.F.R. Section 484.4 and Section 484.36.
Documented evidence of 75 hours of training as specified in 42 C.F.R. Sections 484.36 (a)(1).
Documented evidence that HomeHealthAide has successfully completed a Competency Evaluation program as described in 42 C.F.R. Section 484.36(b).

POSITION SUMMARY:
Reporting to the In-Patient Facility Manager, this position is responsible for carrying out the duties assigned in order to increase the patient's level of comfort and to maintain personal hygiene and a safe healthy environment for the patient. Although the HomeHealthAide reports to a primary manager, HEB expects the HomeHealthAide may work in more than one program or department, and assignments are given to best meet the needs of our patients.
ESSENTIAL FUNCTIONS:
Responsibilities of the HomeHealthAide include, but are not limited to, the following:
Performing personal care needs after given a patient's assignment and attending to his/her requests. If unable to perform a certain task, communicates with Case Manager.
Providing patients and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails).
Following the plan of care as written by the RN.
Demonstrating clinical care as defined in the HomeHealthAide scope of practice.
Assessing and reporting on the patient's condition and significant changes to the RN. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
Recognizing and reporting emergencies.
Keeping the patient's living area clean and orderly, as assigned.
Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
Participating in quality assessment performance improvement teams and activities.
Participating in IDT meetings.
Respectful of patient and family/caregiver environment and patient's personal needs.
Maintaining inventory of disposable supplies.
Maintaining cleanliness and infection control standards.
Performing other duties as assigned.
POSITION REQUIREMENTS:
Must be a compassionate team player who displays confidence and the ability to work independently with little or no supervision.
Must demonstrate initiative, dependability and good judgment.
Must be able to work collaboratively as a team with strong communication skills and be flexible with work assignments and professional when dealing with clients and staff.

EXPERIENCE AND EDUCATION:
Must complete the HomeHealthAide competency test with a passing score of 75% or better.
Must have completed an official HomeHealthAide training course and have a HomeHealthAide Certificate issued by the State Department of Public Health.
If there has been a 24-month lapse in furnishing services, the individual must complete another state approved HomeHealthAide program before providing services.
Valid CPR certification.
