    Full-time Description Hospice of the Piedmont is seeking an Admission Nurse to guide patients and families through the hospice admission process. This role involves assessing eligibility, providing education, coordinating care, and promoting hospice services with compassion and excellence. Responsibilities/General Duties: • Build and maintain relationships with physicians, facilities, and referral sources to promote hospice services • Respond to referrals promptly and manage patients through admission or community referral • Present hospice services to patients and families • Collect patient information including demographics, insurance, and clinical history • Assess patient utilizing the nursing assessment and LCD criteria to identify the physical, psychosocial, and environmental needs • Develop and document the plan of care in correlation with the assessment and in compliance with regulations • Provides nursing care utilizing infection control measures (OSHA) • Provides effective communication to patients, their family, team members, and other healthcare professionals • Work with the IDT to ensure patient/family needs are met • Communicate with attending providers, hospice physicians, and other providers involved in the patient's care • Timely submission of all required paperwork (consents, DDNR, ect.) • Document visits accurately and in a timely manner • Promotes hospice's philosophy • Educate healthcare partners and the community on hospice philosophy and benefits • Support referral growth through outreach and education • Attend meetings, trainings, and participate in professional development • Maintain patient confidentiality in accordance with HIPAA Qualifications: • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing • Licensed as a Registered Nurse in the Commonwealth of Virginia • One year of experience in Hospice or Home Health is required • Requires a valid driver's license, personal automobile, and current automobile liability insurance Preferred Skills and Abilities: • Proficient in EMR systems • Strong clinical assessment, critical thinking, and decision-making abilities • Excellent time management, organizational, and multitasking skills • High attention to detail with a commitment to quality and accuracy • Strong interpersonal and teamwork skills; maintain professionalism and integrity • Flexible, resilient under pressure, and maintains confidentiality in all patient interactions Typical Physical Demands/Working Conditions: • Drive short/long distances • Generally fast-paced work environment with occasional stressful conditions to meet patient needs • Must be prepared to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens smoke, no chairs • Ability to meet patient/family individualized psychosocial needs • Must be prepared to complete EMR at bedside in varied environments • Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care Hospice of the Piedmont offers competitive wages and a comprehensive benefits package which includes Medical, Dental, Life, Short & Long Term Disability, 403(b) and Roth Retirement Plans with employer matching, Flexible Spending Accounts, Employee Assistance Program, Paid Time Off, and Paid Holidays.
